There were multiple issues that were observed throughout the inspection. 1. The Horizontal reinforcing steel only has .5 inches of clearance along the west foundation face. 2. Wall is missing an number 5 rebar next to the hook bar throughout the entire wall. 3. There is too much clearance between the top rebar mat and to of foundation 4. Forms are not set to 15 feet 6 inches 5. At the south end of the foundation more reinforcing steel needs to be added. Accepted inspection date Minor Deficiences noted. Re-checked prior to placement. Notes were taken of the deficiencies and will be sent to the contractor so they can make repairs. Tiffany Park Rebar and Form work Inspection. Contractor had not called in a subgrade inspection for footing of the wall. They did provide the compaction reports which can be found in the Project file. Contractor did not call for a formwork inspection prior to the installation of the reinforcing steel as previously discussed. (11:00 am) Arrived onsite with Partick DeCaro (City of Renton Inspector) performed a rebar inspection to confirm that the rebar installed was in accordance with the plans and specifications. The special inspector did not know it was a WSDOT spec job so they did not take any concrete cylinders until the 5th truck. Most trucks were right at the 2 hour window for pouring concrete into the pump. 4-2-2018 Onsite inspecting the wall with Patrick DeCaro. all deficiencies previously observed on 3-29 have been corrected and the contractor was approved to place concrete for the footing. Third party inspector report was received approving the wall prior to our inspection. 4-4-2018 12:00 pm arrived on-site and noticed that the third party inspector had not tested any of the concrete. The contractor had placed approximately 50 cubic yards of concrete into the footing starting on the south side moving to the north. I had notified the third party inspector that this project is a WSDOT job and they are required to follow the testing requirements as specified per WSDOT Standard specifications. He was unaware that this project was a WSDOT job and immediately began to test the concrete. Joe Olivieri from Rhino Construction was notified that the concrete tester on-site was unaware of the testing requirements. i notified Joe that the concrete for the wall has not been tested correctly. Concrete trucks were allowed to be placed up to two hours past batch time due the the mild weather and consistency of the concrete. After two hours the concrete trucks were notified to stop pumping into the hopper. Pat DeCaro arrived onsite and finished out the remainder of the concrete placement for inspection. Took pictures of contractor tying vertical steel rebar. Called plan reviewer and asked if they were allowed to do this. Was told to inform the site foreman and have them shut down. On site today with Justin Johnson do inspect the wall forms. They were acceptable. Contractor is installing forms for the wall. 5-7-2018 Forms were approved for setting on the wall. Contractor continues installing forms for the wall. Contractor continues installing forms for the wall. With COR Justin and Ann inspected forms today. All passed. Contractor is setting forms for the wall. AAR has passed their potion of the inspection of the forms on the wall. We will inspect it on Tuesday. Inspected the wall forms with Justin (COR) this morning at 10:30am. We noticed 3 repairs that were need. #1, #7 vertical bar moved from touching the form, #2, hook bars need 2" clearance from form, and #3, remove debris from bottom of forms. Went back at 1:30pm and checked on their progress. All repairs were done and passed. They are now ok to pour the wall. Wall contractor was preparing to pour cement concrete today, but the tester from AAR noticed the approved concrete mix design did not meet the WSDOT requirement for air entrainment. The pour was canceled and we are now waiting for a new concrete mix design. Contractor poured the wall on Monday. It was witnessed and the cement concrete was tested by the inspector from AAR as I was out sick. Waiting on cylinder breaks for cement concrete strength. Independent testing company AAR contacted the contractor and told them the 3 day break cylinders are at strength, so the wall contractor is striping the forms today. Contractor continues striping forms from the retaining wall. Inspected the bars and forms of the retaining wall today. All passed. They are now ok to continue forming the other half of the wall. Contractor continues installing retaining wall forms. Inspector from AAR has checked the retaining wall forms and found items to be addressed. The contractor will work on these and we will try and inspect tomorrow. Inspected wall forms this morning with Justin Johnson (COR). Horizontal steel at bottom of forms was off by approx. 12". Had contractor adjust steel to with 4" to 5" of wall form. Contractor has cement concrete show up on site around 11:00am. AAR testing on site for air entrainment and slump tests. The 1st truck had an air content of 4.5%, which is passing. I asked the tester to check the 2nd truck for air which came out at 3.5%, which is below acceptable standards. The truck was sent away because of low air and exceeding the time limit,(2 hours). By the 3rd truck the concrete QC was on site and the air test was below 3.0%. The QC added air entrainment but by the time they got in within specification, they had exceeded the time limit so the truck was sent away. Trucks 4 through 9 were all low on air so the QC added air entrainment to get them within passing specification. They were also at least and hour old by the time they arrived on site, and some were sent away before pouring their full load because of exceeding the time limit. Contractor has started to strip forms from the retaining wall. Contractor continues stripping forms from the retaining wall. Contractor is starting to backfill the retaining wall. I will be out the rest of the week so I informed them to call in with any work. Contractor is tieing rebar for the top of the retaining wall. AAR on site today to inspect the tieing of rebar for the top of the retaining wall. Contractor failed and are working to fix the issues. Contractor continues working on tieing rebar for the retaining wall. Inspected retaining wall today with Justin (COR). Found the following things in need of repair before we will let them proceed. Finish wall ends, remove debris, and Add crack bar at the top. Contractor continues working on tieing rebar for the retaining wall. Checked wall today with Justin (COR), after AAR said everything was good to go. Found the following defects: 6 locations where the bar clearance was more than 3 1/2 inches, need 2 vertical bars on South end of wall, substructure needs to be cleaned, remove debris, and adjust bottom horizonal bar at the South end of wall. Contractor has poured the last section of the retaining wall today. Checked face of wall today. Took pictures of the 10 cracks in the cement concrete running from the top of the wall to the bottom. Will discuss with Justin Johnson (COR) as to what we want to do next. Met on site with Justin Johnson and Ann Fowler (Both COR), so they could see the cracks in the wall. Ann is sending a letter to the engineer to ask them about what they want to do for this issue. Met on site with Ann Fowler and a structural engineer about the cracks in the wall. He said he would place a device on them that will measure if they are growing or not. Received an e-mail from Ann (COR) that stated the structural engineer who checked the cracks in the wall said they were not significant. Contractor still needs to finish the sacking of the wall. It has been added to the punch list. Contractor still needs to finish the sacking of the wall. Did not see anyone working on the site today. Contractor has finished sacking the wall.
