PURPOSE
The purpose of this Amendment is to: 1) revise the Exhibit III and Exhibit IV line-item budgets to
ensure effective expenditure of CV funding; and 2) shorten the Exhibit VII end date to 4/11/2023 (the
funds will be reallocated to Contract 6336177).

Section III Compensation and Method of Payment: Subsection D Line-Item Budget table is
deleted and replaced as follows:
Rent Assistance $11,000
Supplies for COVID-19 protection (i.e. disinfectant wipes,
masks, gloves)

Hotel/Motel Assistance (1 night/$125/each) $0
Contracted services – Cleaning Company for additional
sanitation support for office space and common areas

Section III Compensation and Method of Payment: Subsection D Line-Item Budget table is
deleted and replaced as follows:

All references in Exhibit VII to “September 30, 2023” as the termination date of that Exhibit are
deleted and replaced with “April 11, 2023”. Remaining funds will be reallocated to Contract
6336177 via Amendment.
