Scope of Work
The work to be performed under this contract shall include, but not be limited to:
The construction of the East Renton Lift Station Elimination project includes construction of
approximately 75 linear feet of 12" and 950 linear feet of 15" PVC gravity sanitary sewer pipelines,
clearing and grubbing of an approximate 20,000 square foot corridor through Maplewood Park,
wetland and stream buffer mitigation within Maplewood Park corridor, temporary bypass pumping,
and decommission and demolition of existing lift station facilities. A large portion of the Work is
located within Maplewood Park, a forested, inactive park owned by King County. The rest of the
Work is located within an existing utilities site and a residential street.
A total of 100 working days will be allowed for the completion of this. Work within Maplewood Park,
excluding plantings, will be limited to June 1 to September 30, 2013.

THIS PERMIT IS FOR KING COUNTY RIGHT OF WAY ONLY.
GIVE DISTANCE BY STREET OR ROAD WITH AN ON, FROM AND TO DESCRIPTION.
ALL WATER MAIN EXTENSIONS MUST HAVE APPROVAL FROM THE FIRE MARSHAL.
ALL UNDERGROUND UTILITY LINES MUST HAVE A MINIMUM 36" COVER.
ALL WORK TO BE DONE SUBJECT TO THE APPROVAL OF THE KING COUNTY ROAD ENGINEER.
ALL WORK SHALL BE PERFORMED IN CONFORMANCE WITH KING COUNTY ROAD STANDARDS.
The undersigned agrees to comply with provisions, conditions and requirements contained in the "Standards of Good Practice for County Road Departments"
published by County Road Administration Board.
All work to be done in conformity with conditions and requirements of the King County Code 6.27, 13.24 and 14.44, and the petitioner agrees to prosecute with all

The undersigned, its successors and assigns, agrees if granted the above permit, to comply with the provisions, conditions, requirements, regulations and
recommendations herein contained and as May apply to any utility franchise granted the applicant and under whose provisions same is issued. It will respect and
protect all property contracts, persons and rights that might be affected by it.
INDEMNITY AND HOLD HARMLESS: The Permittee agrees to indemnify and hold harmless King County as provided herein to the maximum extent possible
under law. Accordingly, the Permittee agrees for itself, its successors, and assigns, to defend, indemnify, and hold harmless King County, its appointed and elected
offretals and emplovees from and against liability for all claims, demands, suits, and judgments, including cost of defense thereof, for injury to persons, death, or
property damage which is caused bv, arises out of, or is incidental to Perminee's exercise of rights and privileges granted by this permit. The Permittee's
obligations under this permit shall include: (a) Indemnification for such claims whether or not they arise from the sole negligence of either the County or the
Permittee, the concurrent negligence of both parties, or the negligence of one or more thirdparties; (b) The duty to promptly accept tender of defense and provide
defense to the County at the Permittee's own expense; (c) Indemnification of claims made by the Permittee's own employees or agents; and (d) Waiver of the
Permittee'smmuniry under the industrial insurance provisions of Title 51 RCN', which waiver has been mutually negotiated by the parties. In the event it is
necessary for the County to incur anorney's fees, legal expenses. or other costs to enforce the provisions of this section, all such fees, expenses, and costs shall be
recoverable from the Permitter. In the event it is determined that RCW 4.24.115 applies to this permit, the Permittee agrees to defend, hold harmless, and
indemnify King County to the maximum extent permitted thereunder.

2. All hard surfaced roads to be jacked or bored. Exceptions will be on a case -by -case basis with the express permission of the King County Department of
Transportation.
3 One -war traffic at all times. Signs and traffic control will be in accordance with the manual on uniform traffic control devices for streets and highways.
4. Asphalt to be neat -line cut by either saw cutting or jackhammering a continuous line 1 foot back from trench. Restoration as a minimum shall include 6.5" of
crushed surfacing material and HNIA the same thickness as the existing asphalt pavement or a minimum of'- inches, whichever is greater.
5. It is the responsibility of the grantee to notify all utility districts and private property owners when such property is liable to injury or damage through the
performance of the above work. Call 1.800.424.5555 48 hours in advance for underground utility location. This instruction does not relieve the grantee from
required notification of County inspectors as specified in paragraph 1, above.

prescribed be the County Engineer. The Count Engineer reserves the right to order its own agents or representatives to accomplish the desired change and all costs of
relocation or removal shall be bome b% the grantee.
8 All such changes. reconstruction or relocation by the grantee shall be done in such manner as will cause the (cast interference with ant of the Countr-s work. The grantee
shall have sevens -two (72) hours written notice by the Count Engineer or the Engineer's representatives of any blasting contiguous to the grantee's permit rights in
order that it mac protect its interests.
9. This grant or privilege shall not be deemed or construed to be an exclusive franchise It does not prohibit the County from granting other pemtits or franchise rights of
like nature to other public or private utilities. nor shall it prevent the Count from using arty of its roads. Strecta, public places for an, and all public use, or affect its
jurisdiction over all or anv pan of them.
10 All the provisions, conditions. requirements and regulations herein contained shall be binding upon the successors and assitms of the grantee and all privileges of the
grantee shall be given to such successors and assigns as if the, were spedfically mentioned

1. Work Subject to Approved Plans and Conditions. Work authorized by this permit is subject to the approved plans and
corrections shown thereon and the attached conditions of permit approval. Failure to comply with all conditions once
construction is begun may necessitate an immediate work stoppage until such time as compliance with the stipulated
conditions is attained.
2. Posting on the job site. This permit must be posted on the job site at all times in a visible and readily accessible location.
3. Scheduling Inspections. Inspection requests should be called in either directly to the assigned inspector or via message to
support staff at 106-286-W45._ Please provide at least 48 hours notice to schedule inspections. Scheduling and inspector
availability is subject to approved holidays.
4. Expiration. Please note the expiration date on this permit located in the upper right comer.
5. Compliance with State and Federal laws and the Endangered Species Act. The applicant is responsible for making a
diligent inquiry regarding the need for concurrent state or federal permits to engage in the work requested under this permit,
and to obtain the required permits prior to issuance of this permit. It is understood that the granting of this permit shall not be
construed as satisfying the requirements of other applicable Federal, State or Local laws or regulations. In addition this
permit does not authorize the violation of regulations. In addition, the granting of this permit does not authorize the violation
of the Endangered Species Act as set forth at 16 U.S.C. § 1531-1543, including the prohibition on the "take" of threatened or
endangered species. "Take" is defined at 16 U.S.C. § 1532(19). It is the applicants sole responsibility to determine whether
such "take" restrictions would be violated by work done pursuant to this permit, and is precluded by Federal Law from
undertaking work authorized by this pemut if that work would violate the "take" restrictions set forth at 16 U.S.C. § 1538, 50
C.F.R. §17.21, 50 C.F.R §17.31, 50 C.F.R. §223 and 50 C.F.R. §224.
6. Fees due: Enforcement. The King County Code states that fees associated with the review and inspection of projects
requiring permit applications are due at the time of application for service, or within fifteen days of receipt of an invoice from
King County's Department of Development and Environmental Services (DDES) stating that currently hourly rates are due.

The following conditions apply to the above referenced permit:
The permittee shall be responsible for all costs associated with the review and/or inspection of this permit
by responsible department staff. These costs shall be in accordance with the fees set forth in King County
Title 27: Failure to remain current with fee balances may be cause for suspension or revocation of the
permit.
Development authorized by this permit or approval may require other state and/or federal permits,
including, but not limited to, a Washington State Hydraulics Project Approval (HPA) or a U.S. Army Corps
of Engineers 404 or Section 10 permit. If such other permit(s) is/are required, this/these other permits must
be issued prior to issuance of this permit or approval. Failure to secure these other permits before beginning
work authorized by this permit or approval is a violation of this condition, and may result in suspension or
revocation of this permittapproval, and/or pursuing other enforcement actions. Should any other required
permit be suspended, revoked or in anyway. be subjected to other enforcement actions, this permit may be
suspended until all defects causing said enforcement actions have been remedied. In addition, the granting
of this permit or approval does not authorize the applicant to violate any provisions of the Endangered
Species Act as set forth at 16 U.S.C. §§ 1531-1543, including the prohibition on the "take" of threatened or
endangered species."Take" is defined at 16 U.S.C. §§ 1532(19).
General Requirements
At least 48 hours prior to the start of any work, call the Site Development Services Section to arrange a pre -
construction meeting. Please be advised this permit shall not be effective until the Notice to Proceed has
been issued pursuant to this meeting. If required, Notice of Construction Activity signs shall be in place
prior to the pre -construction meeting. The Site Development Specialist assigned the project is Ron
Ainslie and can be contacted at 206-296-7142 or by email at ron.ainslie@kingcounty.gov
The permittee shall notify the Site Development Services Section at least one day prior to the start of work
via telephone and or email. Upon completion of the work authorized by this permit, the permittee shall
notify the Site Development Specialist in writing by letter and/or email requesting a final inspection. Prior
to this request ensure that the site is stab il ized/revegetated per the approved plan with all temporary erosion
control systems removed. Failure to request a final inspection may result in unnecessary site inspections
and billings.
The boundaries of the clearing limits shown on this plan shall be clearly flagged in the field prior to
construction. The flagging shall be maintained by the permittee for the duration of construction. The
clearing limits shall be maintained by the applicant/ESC supervisor for the duration of construction.
Work shall be limited to that shown on the approved plans stamped and dated �o.luuu I by JIM
inslie of King County Land Use Services Division A copy of the approved plans, conditions and
permit must be on the job site whenever work is in progress.
Permittee is responsible to call 1-800-424-5555 not less than 48 hours before beginning excavation where

when overhead power, telephone, or utilities may be affected by this project.
All work shall comply with the provisions of King County Code Chapter 12.86, relating to noise control.

5:00 p.m. on Sunday. In the event that the permittee obtains a noise variance form the Seattle
King County Health Department, the hours of operation contained within the variance shall supersede
those contained within this condition.
During hauling operations, permittee shall provide effective dust control measures consisting of water,
asphalt treated base, chemical dust palliatives, or equivalent measures to control dust from this operation.
Dust, dirt or airborne solids from any source shall not be emitted in quantities as to adversely affect
adjacent properties and/or public traffic. Permittee shall abide by the regulations of the Puget Sound Clean
Air Agency (PSCAA). Contact PSCAA at (206)343-8800 for information.
Permittee shall be responsible for implementing all appropriate measures needed (i.e. paving, sweepers,
and/or other techniques) to keep streets and roads used as haul routes for export or import of materials
clean and free from debris, mud, etc. Rocked construction entrances shall be installed where ever
equipment, trucks, etc... will be running from disturbed areas to pavement on existing roads.
Any damage to pavement edges, sidewalk, curb and gutter, etc., resulting from operations authorized by
this permit shall be repaired immediately.
Prior to filling and/or grading of this site, the ground surface shall be prepared by removing all clearing
debris, stumps, and other unsuitable material from the site. Only earth materials which have no rock or
similar irreducible material with a maximum dimension greater than 12 inches shall be used as fill.
If the contractor chooses to dispose of excess excavated material on sites where less than 100 cubic yards
and 3 feet of fill will be placed, the contractor shall provide documentation that these sites have been
inspected by a consultant capable of identifying critical areas as defined by King County Code. The report
shall include date of inspection, name of consultant and company, an assessor's map of the parcel, the
name, address and phone number of the property owner, and the location of the fill placed on the property.
No fill shall be placed in critical areas or their buffers on these sites.
The tops and toes of cut and fill slopes shall be set back from property lines and structures as far as
necessary for adequacy of foundation support and safety of adjacent properties and to prevent damage
resulting from water runoff or slope erosion.
If the work is to be suspended for 30 or more consecutive days, the permittee shall notify the Site
Development Services Section prior to shut down. The permittee shall renotify the Section prior to
resumption of operations.
Erosion Control Requirements
The implementation of these ESC plans and the construction, maintenance, replacement, and upgrading of
these ESC facilities is the responsibility of the permittee until all construction is approved. Permittee shall
inspect the ESC facilities at the end of each working day to assure that they are in good working condition.
If these facilities require repair or maintenance, it shall be performed prior to the end of the working day.
All disturbed areas shall be promptly and thoroughly stabilized against erosion during periods of wet
weather when work is suspended.

Appropriate erosion and sediment control practices shall be used during construction to protect drainage
systems and adjacent properties. These controls shall be in place prior to site preparation or construction,
and shall be properly maintained during the entire construction process.
Stockpile and staging areas shall have TESC measures adequate to prevent any surface waters that are or
might be concentrated from carrying sediment from the site to a natural watercourse. DDES may require a

restoration plan be submitted for review and approval for any stockpiling or staging areas associated with
the permit. Staging and stockpiling areas may be located on existing access roads.
Any new fill area or other disturbed area which has been stripped of vegetation and where no further work
is anticipated for a period of 15 days or more shall be immediately stabilized with mulching, grass planting,
or other approved erosion control treatment appropriate to the time year. Grass seeding alone will be
acceptable only during the months of April through September, inclusive. Seeding may proceed outside of
the time period specified above whenever it is in the interest of the Permittee, but must be augmented with
mulching, netting, or other treatment approved by the department.
During the time period of November 1 through March 31, all project disturbed soil areas greater than five
thousand square feet (5,000 sq. ft) that are to be left unworked for a period of more than 12 hours shall be
covered with mulch or plastic or otherwise protected from erosion.
Temporary sediment control facilities shall be constructed in accordance with the details shown.
Temporary sediment control facility locations may be moved to suit field conditions subject to approval of
the engineer and applicable governmental agencies.
All ponds and ditches and other erosion -sedimentation facilities shall be maintained in good working
condition throughout construction
Where seeding for temporary erosion control is required, use the standard set forth in the current King
County Surface Water Design Manual.
Where straw mulch for temporary erosion control is required, it shall be applied at a minimum thickness of
2 inches.
The ESC facilities on inactive sites shall be inspected and maintained a minimum of once a month or
within the 24 hours following a storm event.
Site Specific Conditions
A Right of Way Use Permit will be required for work within the publicly owned right of

I. PERMIT REVOCATION: This Permit is revocable at any time by King County. The right to revoke is expressly reserved to King County.

the maximum extent permitted thereunder, and specifically for its negligence concurrent with that of King County to the full extent of Pernittee's negligence.
Permittee agrees to defend, indemnify, and hold harmless the County for claims by Permittee's employees and agrees to waiver of its immunity Order Title 51
RCW, which waiver has been mutually negotiated by the parties.
3. ANTI -DISCRIMINATION: In all hiring or employment made possible or resulting from this Permit, there shall be no discrimination against any
employee or applicant for employment because of race, color, ancestry, religion, national origin, age, sex, sexual orientation, marital status, or the presence of any
sensory, mental or physical handicap in an otherwise qualified handicapped person unless based upon a bona fide occupational qualification, and this requirement
shall apply to but not be limited to the following: employment, advertising; lay-off or termination, rates of pay or other forms of compensation, and selection for
training including apprenticeship. No person shall be denied, or subjected to discrimination in receipt of the benefit of any services or activities made possible by
or resulting from this Permit on the grounds of race, color, ancestry, religion, national origin, age (except minimum age and retirement provisions), sex, sexual
orientation, marital status, parental status, the presence of any sensory, mental or physical handicap, or the use of a trained guide -dog by a blind or deaf person.
Any violation of this provision shall be considered a violation of a material provision of this Permit and shall be grounds for cancellation, termination or
suspension in whole or in part of the Permit by the County, and may result in ineligibility for further County permits.
4. NON-EXCLUSIVE RIGHT: This Permit shall not be deemed or construed to be an exclusive right. It does not prohibit the County from granting any
other permits to other public or private entities, nor shall it prevent the County from using any public place for any and all public use or affect itsjurisdiction over
any part of them.
5. ASSESSMENTS: Permitter shall be required to pay any general or special assessments incurred by King County which are directly attributable to or
arising from any actions, occupancy, or usage authorizedherein.
6. TERMINATION: The Permittee may terminate the Permit by written notice to the Manager of Real Estate Services Section. Upon revocation,
termination, or abandonment, the Permitter shall remove at his expense all facilities placed on said property by the Permittee, and restore the premises to a
condition which is equivalent in all respects to the condition existing prior to installation of the facilities, or to a condition which is satisfactory to the County. If
the Permitter has not accomplished removal and restoration at the end'of a ninety -day period following'the'effective date of revocation, termination, expiration, or
abandonment, the County may accomplish all of the necessary work and charge all of the costs to the Permittee.
7. RESTORATION: After completion of work authorized by this Permit, the Permitter shall restore the property to a condition which is equivalent in all
respects to the condition of the property prior to starting work, or a condition satisfactory to King County. If the Permitter delays the restoration beyond
expiration of the Permit, the County may accomplish all the necessary work and charge all the costs to the Permitter.
8. REPAIRING DAMAGE BY PERMITTEE: In the 'event that damage of any kind is caused by the Permitter in the course of performing work
authorized by this Permit, Permittee;will.repair.said.damage at its sole cost and expense. Repair work shall begin without delay and continue without interruption
until completed. If damage is extensive, the time allowed for repair will be prescribed by the County agent If the County determines it is necessary, the County
may accomplish the work and charge all the costs to the Permittee.
9. ABATEMENT OF UNSAFE CONDITIONS: The, -County representative may at any time, do, order, or have done all work considered necessary to
restore to a safe condition any area described in Permit left by the Permittee in a condition dangerous to life or property. The Permitter shall pay, upon demand, to
the County all costs of such work, materials, etc. Nothing in this section shall relieve the Permittee of duties under Terns and Conditions No. 2 above.
10. RIGHTS RESERVED TO COUNTY - CONFORMANCE AND PAYMENT OF COST REQUIRED: The County -reserves the right to use, occupy,
and enjoy its property for such purposes as it shall desire'including; but not limited to, constructing or installing structures and facilities on the property, or
developing, improving, repairing or altering the property. The Permittee, upon written notice, will at his own cost and expense remove, repair, relocate, change or
reconstruct such installations to conform with the plans of work contemplated or -ordered by the County according to a time schedule contained in the written
notice.
II. NOTICE: Permittee agrees to obtain information from other utility operators regarding the location and current status of their installations before
starting work Property owners adjoining, or in proximity to, the project as described herein shall be notified by Permittee when such property is exposed to the
possibility of injury or damage through performance of work on the project authorized by this Permit. Permitter shall make all advance arrangements necessary to
protect such property or utility from injury or damage.
12. OTHER APPLICABLE LAWS: Issuance of this Permit does not in any way relieve the Permittee from complying with any other applicable laws in
performing the work subject to this Permit.
13. RE-ENTRY: After completion of work authorized by this Permit, if the Permittee desires to re-enter upon the property described herein for any
reconstruction, notice shall be provided in advance to King County.together with the plans and specifications for the work proposed, and shall not be permitted

15. SPECIAL TERMS AND CONDITIONS:
a. All use is restricted to the construction, operation and maintenance of a 15-inch sewer
line as shown in the July 2011 60% plans.
b. The Permittee shall not make any additional use of, or improvements or alterations to
the corridor that is not specifically authorized in this permit. Any additional use of
the corridor, including but not limited to new utility lines, landscaping or parking,
must be approved in writing by King County. Such approval shall be made an
amendment to this Permit and contain appropriate conditions.
c. The Permittee will be responsible to secure and provide proof of all necessary permits
prior to starting any construction on the corridor of ways as authorized by this permit.
d. The 60% plans submitted as part of this application dated July 2011 are considered
preliminary. Final plans must be approved by King County Parks prior to the start of
construction.
e. Following authorized construction activities on the corridor, the Permittee shall
restore park property and trail shoulders altered by the construction to original or
better condition, including the restoration of any drainage systems. The Permittee will
not dump or dispose of construction debris and surplus material on parkland and any
time.
f. The Permittee will coordinate site access with Dan Perzynski, Park District
Maintenance Coordinator, (206) 255-6676.
g. The Permittee will contact Robert Nunnenkamp, Property Agent at (206) 263-6207 to
coordinate any on -site inspections, prior to and following any work on parkland.
h. The Permittee shall provide sets of as-built/record drawings in DWG and PDF
formats.
i. King County reserves the right to set reasonable additional terms and conditions as
unforeseen conditions may warrant in the future.

15j. INSURANCE:
By the date of issuance of this permit, the permittee shall procure and maintain, for the duration of this permit,
insurance or coverage against claims for injuries to persons or damages to property which may arise from and in
connection with the rights and privileges granted by this permit and/or the performance of work hereunder by the
permittee, his agents, representatives, employees and/or subcontractors.
By requiring such minimum insurance coverage, the County shall not be deemed or construed to have assessed
the risks that may be applicable to the Contractor under this contract. The Contractor shall assess its own risks
and, if it deems appropriate and/or prudent, maintain greater limits and/or broader coverage.
Nothing contained within these insurance requirements shall be deemed to limit the scope, application and/or
limits of the coverage afforded, which coverage will apply to each insured to the full extent provided by the
terms and conditions of the policy(s). Nothing contained within this provision shall affect and/or alter the
application of any other provision contained within this Agreement.
For All Coverages: The cost of such insurance shall be paid by the permittee. Each insurance policy shall be
written on an "Occurrence Form."
1. Minimum Scope and Limits of Insurance
Coverage shall be at least as broad as:
General Liability:
Insurance Services Office form number CG 0001 (Ed. 11-88) covering COMMERCIAL GENERAL
LIABILITY. The permittee shall maintain limits no less than $1,000,000 combined single limit per occurrence
for bodily injury, personal injury and property damage.
AUTOMOBILE LIABILITY: Insurance Services Office form number (CA 00 01 Ed. 12-90 or its equivalent)
covering BUSINESS AUTO COVERAGE, symbol 1 "any auto', or the combination of symbols 2, 8 and 9.
Limits shall be no less than $1,000,000. Combined Single Limit Bodily Injury and Property Damage.
WORKERS COMPENSATION: Workers Compensation coverage, as required by the Industrial Insurance
Act of the State of Washington;
EMPLOYERS LIABILITY or "Stop -Gap": The protection by the Workers Compensation policy Part 2
(Employers Liability) or, in states with monopolistic state funds, the protection provided by the "Stop -Gap"
endorsement to the General Liability policy. Limits shall be no less than $1,000,000.
2. Deductibles and Self -Insured Retentions
The deductible and/or self -insured retention of the policies shall not limit or apply to the permittee's liability to
the County and shall be the sole responsibility of the permittee.

3.Other Insurance Provisions
The insurance policies required in this permit are to contain, or be endorsed to contain the following
provisions
a. All Liability policies except Workers Compensation:
1. The County, its officers, officials, employees and agents are to be covered as insured as respects
liability arising out of activities performed by or on behalf of the permittee in connection with this
permit.
2. Insurance coverage shall be primary insurance as respects the County, its officers, officials,
employees and agents. Any insurance and/or self-insurance maintained by the County, its officers,
officials, employees or agents should not contribute with the permittee's insurance or benefit the
permittee in any way.
3. The permittee's insurance shall apply separately to each insured against whom a claim is made and/or
lawsuit is brought, except with respect to the limits of the insurer's liability.
b. All Policies:
Coverage shall not be suspended, voided, cancelled, reduced in coverage or in limits prior to the
expiration date of this special use permit, unless forty-five (45) days prior notice, return receipt
requested, has been given to the County.
4. Accentability of Insurers
Unless otherwise approved by the County, insurance is to be placed with insurers with a Bests' rating of no
less than A: VIII, or, if not rated with Bests', with minimum surpluses the equivalent of Bests' surplus size
VIII.
5. Verification of Coverage
The permittee shall furnish the King County Real Estate Services Section with certificates of insurance and
endorsements required by this permit. The County reserves the right to require complete, certified copies of
all required insurance policies at any time.
6. Municipal or State Agency Provision
If the Permittee is a Municipal Corporation or an agency of the State of Washington and is self -insured for
any of the above insurance requirements, a certification of self-insurance shall be attached hereto and be
incorporated by reference and shall constitute compliance with this section.

Project Name. East Renton Lift Station Elimination UTILITY SYSTEMS
Project Description: Convert a portion of the City of Renton sewer system from a pumped system
to a gravity sewer system; includes a temporary culvert and a new conduit
crossing in the upper reach of Stewart Creek.
PROVISIONS
1. TIMING LIMITATIONS: The project may begin immediately and shall be completed by
February 20, 2017, provided work within the ordinary high water line (OHWL) of Stewart Creek
shall occur only between June 1 and September 30.
2. NOTIFICATION REQUIREMENT: The Area Habitat Biologist (AHB) listed below (e-mail to
fisheldf@dfw.wa.gov) and the Enforcement Program Officer (e-mail to capeljlc@dfw.wa.gov) shall
receive e-mail notification from the person to whom this Hydraulic Project Approval (HPA) is issued
(permittee or authorized agent) no less than three working days prior to start of work, and again
within seven days of completion of work to arrange a compliance inspection. The notification shall
include the permittee's name, project location, starting date of work or completion date of work, and
the control number of this HPA.
3. Work shall be accomplished per plans and specifications approved by the Washington
Department of Fish and Wildlife entitled, "PROPOSED: East Renton Lift Station Elimination -
remove existing pump station and install gravity sewer line", dated July 2011, and "EAST RENTON
LIFT STATION ELIMINATION Stream and Buffer Mitigation Plan", dated July 2011, except as
modified by this HPA. A copy of these plans shall be available on site during construction.
4. A temporary culvert may be used as a bypass to divert flow around the work area and shall be in
place prior to initiation of other work in the wetted perimeter.
5. A sandbag revetment or similar device shall be installed at the bypass inlet to divert the entire
flow through the bypass.
6. A sandbag revetment or similar device shall be installed at the downstream end of the bypass to
prevent backwater from entering the work area.

7. The bypass shall be of sufficient size to pass all flows and debris for the duration of the project.
8. Prior to releasing the water flow to the project area, all instream work shall be completed.
9. Upon completion of the project, all material used in the temporary bypass shall be removed from
the site and the site returned to preproject or improved habitat conditions.
CONDUIT INSTALLATION
_I-;10: Conduit,alignment shall be as nearly perpendicular to the stream as possible.
11. The conduit shall be installed at sufficient depth so that subsequent disturbance of the
streambed is avoided.
12. Trenches shall be backfilled with native streambed materials, and the streambed shall be
returned to preproject or improved habitat conditions.
13. Excess spoils shall be disposed of so they will not re-enter the stream.
14. The conduit approach trench shall be isolated from the stream until laying of the conduit across
the stream occurs.
COMMON PROVISIONS
15. Disturbance of the streambed and banks and their associated vegetation shall be limited'to that
necessary to perform the'project. Affected streambed and bank areas -shall be restored to
preproject or improved habitat configuration. Prior to December 31 of the year of conduit
installation, the disturbed areas of vegetation shall be revegetated with native or other woody
species per the approved plans (Provision 3). Plantings shall be maintained as necessary for three
or more years'to ensure 80 percent or greater survival of each species o'r a contingency species
approved by the AHB.
16. If at any time, as a result of project activities, fish are observed in distress,'a fish kill occurs, or
water quality problems develop (including equipment leaks or spills), immediate notification shall be
made to the Washington Emergency Management Division at 1-800-258-5990, and to the AHB.
17. Erosion control methods shall be used to prevent silt -laden water from entering the stream.
These may include, but are not limited to, straw bales, filter fabric, temporary sediment ponds,
check dams of pea gravel -filled burlap bags or other material, and/or immediate mulching of
exposed areas.
18. Prior to starting work, the selected erosion control methods (Provision 17) shall be installed.
Accumulated sediments shall be removed during the project and prior to removing the erosion
control methods after completion of work.

19. Wastewater from project activities and water removed from within the work area shall be routed
to an area landward of the ordinary high water line to allow removal of fine sediment and other
contaminants prior to being discharged to the stream or wetlands associated with the stream.

Contractor installed the 12" sewer between Manholes 79 & 80. Manholes core drilled
with core-n-seal boots. Contractor had to bring in quarry spalls for foundation material
for this run.
Contractor was allowed to use some of the "native" material out of the existing trench
line for backfill as long as it was clean and "dry". The native was used in the middle part
of the trench. Import was used toward the bottom of the trench as it would be less
susceptible to impact from the water at that level.
Manhole 80 has the isolation valve for the lift station installed inside it. The contractor
was told that, when they modify that manhole, to leave the valve in place, close it, cut off
the valve stem and fill structure with concrete as specified (don't worry about filling
concrete into the outflow sewer).
Approved using a quick set concrete for the bottom of manholes 79 & 80 so they can
bring in the pea gravel the same day.

subject East Renton Lift Station Landscape Final Inspection
ESA completed a final inspection of the installed plants at the East Renton Lift Station site within Maplewood
Park on October 3, 2013.
All plantings installed were reviewed and appear to meet the punch list accounting for the areas that were
modified at the direction of the City. Since the plantings have been installed, heavy browsing by deer has been
observed, primarily on the Indian plum and Ninebark plantings. It appears that the browse is primarily of the
leaves and the bark has not been affected. Because the affected plants are entering their dormancy period, we
recommend keeping the plants in place and observing their health in the spring. All of the plantings should
continue to be monitored and replaced, if needed, during the plant establishment period. At this time it is our
opinion they are meeting the specification.
