GENERAL NOTES:
1. UNLESS OTHERWISE SPECIFIED HEREIN, ALL CONSTRUCTION
SHALL BE IN ACCORDANCE WITH THE 1996 EDITION OF THE
"STANDARD SPECIFICATIONS FOR ROAD, BRIDGE, AND
MUNICIPAL CONSTRUCTION" PREPARED BY WSDOT, AND THE
AMERICAN PUBLIC WORKS ASSOCIATION (APWA), AS
AMENDED BY THE CITY OF RENTON.
2. ALL WORK SHALL COMPLY WITH THE EASEMENT AND UTILITY
RELOCATION AGREEMENT BETWEEN THE CITY OF RENTON AND
OLYMPIC PIPE LINE COMPANY, DATED APRIL 1, 1998.
IN ACCORDANCE WITH THIS AGREEMENT, THE CONTRACTOR
MUST COMPLY WITH ALL APPLICABLE REGULATORY PERMIT
REQUIREMENTS, INCLUDING BUT NOT LIMITED TO THE
SEATTLE PUBLIC UTILITIES, WASHINGTON STATE DEPARTMENT
OF FISH AND WILDLIFE HYDR
AULIC PROJECT APPROVAL
(HPA), AND THE CORPS OF ENGINEERS NATIONWIDE PERMIT
NO. 3. COPIES OF THESE PERMITS ARE AVAILABLE AT THE
CITY OF RENTON. OLYMPIC PIPE LINE COMPANY SHALL OBTAIN
APPROVAL FROM SEATTLE PUBLIC UTILITIES PRIOR TO
CONSTRUCTION.
3. BEFORE ANY CONSTRUCTION OR DEVELOPMENT ACTIVITY
OCCURS, A PRE -CONSTRUCTION MEETING MUST BE HELD
WITH THE OLYMPIC PIPE LINE COMPANY AND THE CITY OF
RENTON DEVELOPMENT SERVICES DIVISION, PLAN REVIEW
PROJECT MANAGER.
4. THE PROJECT SHAD_ BE CONSTRUCTED IN ACCORDANCE WITH
THIS PLAN. ANY DEVIATION FROM THE APPROVED PLANS
WILL REQUIRE WRITTEN APPROVAL FROM THE CITY OF
RENTON DEVELOPMENT SERVICES DIVISION.
5. A COPY OF THIS PLAN MUST BE ON THE JOB SITE
WHENEVER CONSTRUCTION IS IN PROGRESS. DATUM IS
BASED ON NAVD 1988.
6. THE CONTRACTOR SHALL BE RESPONSIBLE FOR PROVIDING
ADEQUATE SAFEGUARD, SAFETY DEVICES, PROTECTIVE
EQUIPMENT, FLAGGERS, AND ANY OTHER NEEDED ACTIONS
TO PROTECT THE LIFE, HEALTH, AND SAFETY OF THE
PUBLIC, AND TO PROTECT PROPERTY IN CONNECTION WITH
THE PERFORMANCE OF WORK COVERED BY THE CONTRACT.
7. CONTRACTOR SHALL OBSERVE WEIGHT LIMIT RESTRICTIONS ON
OLYMPIC PIPE LINE ACCESS ROAD. GROSS AXLE WEIGHT
SHALL NOT EXCEED 32,000 LBS. (HS 20-44).
8. ALL SUBMITTALS TO OLYMPIC PIPE LINE SHALL BE COPIED
TO THE CITY OF RENTON FOR REVIEW. EXCEPT AS NOTED
HEREIN, ALLOW 2 WORKING DAYS FOR REVIEW PRIOR TO
BEGINNING WORK ASSOCIATED WITH THE SUBMITTAL.
9. CONTRACTOR IS RESPONSIBLE FOR NOTIFYING ALL UTILITY
PURVEYORS AT LEAST 48 HOURS IN ADVANCE OF ALL WORK
IN VICINITY OF UTILITY. U11UTY LOCATIONS SHOWN ON THE
PLANS ARE APPROXIMATE AND WERE DEVELOPED FROM
RECORD DRAWINGS AND SUPPLEMENTED WITH APPROXIMATE
FIELD MEASUREMENT OF SURFACE FEATURES. CONTRACTOR
SHALL CONTACT UTILITY LOCATION CENTER (1-800-424-5555)
TO LOCATE AND MARK ALL UTILITIES AT LEAST 2 WORKING
DAYS PRIOR TO COMMENCEMENT OF ANY WORK.
10. NOTE THERE ARE OVERHEAD 115 KV POWER LINES IN THE
WORK AREA. THE CONTRACTOR IS REQUIRED TO MAINTAIN A
15-FOOT MINIMUM CLEARANCE FROM THESE POWER LINES AT
ALL TIMES. CONTRACTOR MAY, AT ITS EXPENSE, COORDINATE
WITH PUGET SOUND ENERGY TO HAVE PHONE LINES
TEMPORARILY SHUTDOWN.
11. CONTRACTOR SHALL OBTAIN AND REVIEW THE FOLLOWING
SUBSURFACE INFORMATION FROM THE CITY-
GEOTECHNICAL STUDY CONDUCTED BY HWA GEOSCIENCES,
INC., DATED APRIL 1988.
SEDIMENT AND SOIL CHARACTERIZATION STUDY PREPARED
BY HERRERA ENVIRONMENTAL CONSULTANTS, INC., DATED
APRIL 13, 1988.
AVAILABLE GROUNDWATER MONITORING DATA. GROUNDWATER
MONITORING DATA IS RELATIVE TO NATIONAL GEODETIC
VERTICAL DATUM (NGVD) OF 1929 WHICH IS THEORETICALLY
3.58 FEET LOWER THAN NAVD.
12. AT LEAST 15 DAYS PRIOR TO COMMENCEMENT OF WORK
CONTRACTOR SHALL SUBMIT PLAN FOR INSTALLATION OF
BOX CULVERT IN ACCORDANCE WITH MANUFACTURERS
INSTRUCTIONS AND IN ACCORDANCE WITH THE GEOTECHNICAL
REPORT AND THESE NOTES. THE PLAN SHALL CLEARLY
DESCRIBE THE FOLLOWING: (1) METHODS FOR DEWATERING,
(2) THE PLAN FOR PROVIDING A FIRM FOUNDATION FOR THE
BOX CULVERT IN ACCORDANCE WITH THE RECOMMENDATIONS
IN THE GEOTECHNICAL REPORT, (3) METHOD OF PROTECTION
AND TEMPORARY SUPPORT OF EXISTING UTILITIES, AND (4)
MONITORING THE EXISTING SEATTLE WATER DEPARTMENT
FACILITIES IN ACCORDANCE WITH APPROVED PERMITS. THE
PLAN SHALL BE PREPARED BY AND STAMPED BY A
PROFESSIONAL ENGINEER REGISTERED IN THE STATE OF
WASHINGTON.
13. AS NOTED IN THE GEOTECHNICAL REPORT, BOX CULVERT
FOUNDATION SUPPORT SHALL BE EITHER 12" DEPTH LEAN OR
TREMIE CONCRETE SLAB OR 24" DEPTH CRUSHED ROCK
PAD FOR CRUSHED ROCK OPTION, EXCAVATION SHALL BE
ADEQUATELY DEWATERED AND DRY BEFORE PLACING.
CRUSHED ROCK SHALL MEET THE REQUIREMENTS OF
CRUSHED SURFACING BASE COURSE, SECTION 9-03.9(3) OF
THE STANDARD SPECIFICATIONS. FOR LEAN CONCRETE
OPTION, LEAN CONCRETE SHALL HAVE A MINIMUM 28 DAY
COMPRESSION STRENGTH OF 2,000 PSI. TREMIE CONCRETE
SHALL HAVE A MINIMUM 28 DAY COMPRESSION STRENGTH
OF 3,000 PSI.
14. IF SOFT COMPRESSIBLE SOILS SUCH AS PEAT ARE
ENCOUNTERED AT THE SUBGRADE ELEVATION FOR CONCRETE
SLAB/CRUSHED ROCK PAD, UNSUITABLE SOILS SHALL BE
OVER -EXCAVATED AND REPLACED. REPLACEMENT OF
UNSUITABLE FOUNDATION MATERIAL SHALL BE WITH CRUSHED
ROCK OR LEAN CONCRETE. OVER -EXCAVATION PROCESS
SHALL BE OBSERVED BY A GEOTECHNICAL ENGINEER. A
SOIL STABILIZATION GEOTEXTILE SHALL BE PLACED ON
SUBGRADE IN THE AREA OF REMOVED COMPRESSIBLE SOILS
PRIOR TO BACKFILLING.
15. EXCAVATION BACKFILL SHALL BE BANK RUN GRAVEL FOR
TRENCH BACKFILL IN ACCORDANCE WITH 9-03.3.19 OF THE
STANDARD SPECIFICATIONS. THE BACKFILL SHALL NOT BE
BULLDOZED INTO THE EXCAVATION OR DROPPED DIRECTLY
GENERAL NOTES: (CONT'D
ON THE STRUCTURE. HEAVY VIBRATORY EQUIPMENT SHALL
NOT BE USED OVER THE BOX CULVERT UNTIL A MINIMUM 3
FEET OF BACKFILL HAS BEEN PLACED. BACKFILL SHALL BE
PLACED IN HORIZONTAL LIFTS NOT EXCEEDING 8 INCHES IN
THICKNESS AND COMPACTED TO AT LEAST 95 PERCENT OF
MODIFIED PROCTOR MAXIMUM DRY DENSITY AS DETERMINED
BY ASTM D1557. WHEN SMALLER COMPACTION EQUIPMENT
IS USED, SOIL SHALL BE PLACED IN LIFTS NOT EXCEEDING 4
INCHES.
16. MATERIAL EXCAVATED WITHIN THE EXISTING CHANNEL
BOTTOM AT THE UPSTREAM SIDE OF THE BOX CULVERT
SHALL BE EXCAVATED AND HAULED TO A LANDFILL FACILITY
OPERATING UNDER A CURRENT PERMIT MEETING CURRENT
WAC 173-51 (E.G., CEDAR HILLS). OTHER MATERIAL
EXCAVATED WITHIN THE EXISTING BERM AND AT THE BOX
CULVERT OUTLET SHALL BE DISPOSED OF IN A LEGAL
MANNER TO BE DETERMINED BY THE CONTRACTOR. REFER
TO THE SOIL SEDIMENT AND CHARACTERIZATION STUDY, BY
HERRERA ENVIRONMENTAL CONSULTANTS.
17. CONTRACTOR SHALL SUPPLY AS -BUILT ELEVATION AND
LOCATION INFORMATION TO THE CITY OF RENTON AND
OLYMPIC PIPE LINE.
18. EXCAVATION PLAN SHALL BE DESIGNED BY THE
CONTRACTOR. THE CONTRACTOR SHALL SUBMIT AN
EXCAVATION/SHORING/PROTECTION OF EXISTING UTILITIES
PLAN DESIGNED, SIGNED, AND STAMPED BY A LICENSED
PROFESSIONAL ENGINEER IN THE STATE OF WASHINGTON.
ALL CRIBBING, SHEETING, AND SHORING SHALL MEET THE
REQUIREMENTS OF WAC 296-155. SURCHARGE LOADS FROM
CONSTRUCTION EQUIPMENT, MATERIALS, SOILS, ETC., SHOULD
ALSO BE CONSIDERED IN SHORING DESIGN. THE DESIGN
WILL NOT BE REVIEWED FOR ITS STRUCTURAL ADEQUACY.
THE DESIGN OF ANY CRIBBING, SHEETING, OR SHORING IS
THE SOLE RESPONSIBILITY OF THE CONTRACTOR. THE PLAN
SHALL INCLUDE SPECIFIC MEASURES FOR TEMPORARY
SUPPORT AND PROTECTION OF EXISTING UTILITIES. IF
CONTRACTOR OPTS FOR UNSUPPORTED SLOPra
EXCAVATIONS, TEMPORARY CUTS IN EXCESS OF 4 FEET IN
DEPTH SHOULD BE SLOPED IN ACCORDANCE WITH PART N
OF WAC 296-155.
19. ROCK RIPRAP, WHERE REQUIRED, SHALL BE OF SOUND
QUARRY ROCK PLACED TO A DEPTH OF 18 INCHES AND
MUST MEET THE FOLLOWING SPECIFICATIONS. 4"-8"
ROCK/40%-70% PASSING, 2"-4" ROCK/30%-40% PASSING,
AND 2" ROCK/10%-20% PASSING.
20. ROCK SURFACING SHALL INCLUDE 2-INCHES OF CRUSHED
SURFACING TOP COURSE UNDERLAIN BY 4-INCHES OF
CRUSHED SURFACING BASE COURSE. CONTRACTOR SHALL
INSTALL A SEPARATION GEOTEXTILE (SECTION 9-33.1 OF
THE STANDARD SPECIFICATIONS) FOR WEED BARRIER UNDER
CRUSHED SURFACING FOR OLYMPIC PIPE LINE ACCESS ROAD.
21. ALL DISTURBED AREAS EXCEPT RIPRAP AND STREAMBED
AREAS AND ACCESS ROADS SHALL BE SEEDED. GRASS
SEED SHALL BE APPLIED BY HYDROSEEDING. THE GRASS
SEED MIXTURE, OTHER THAN CITY OF RENTON APPROVED
STANDARD MIXES, SHALL BE SUBMITTED BY A LANDSCAPE
ARCHITECT AND APPROVED BY THE DEVELOPMENT SERVICES
DIVISION.
22. ALL SEDIMENTATION/EROSION FACILITIES MUST BE IN
OPERATION PRIOR TO CLEARING AND CONSTRUCTION, AND
THEY MUST BE SATISFACTORILY MAINTAINED UNTIL
CONSTRUCTION IS COMPLETED AND THE POTENTIAL FOR
ON -SITE EROSION HAS PASSED. CONTRACTOR SHALL SUBMIT
AN EROSION CONTROL PLAN WITH THE FLOW DIVERSION PLAN.
DIVERSION AND CARE OF WATER:
1. CONTRACTOR SHALL SUBMIT A FLOW DIVERSION PLAN AT
LEAST FIVE DAYS PRIOR TO COMMENCEMENT OF WORK. THE
PLAN SHALL BE STAMPED BY A PROFESSIONAL ENGINEER
REGISTERED IN THE STATE OF WASHINGTON. THE SW 23RD
STREET DRAINAGE CHANNEL IS A YEAR-ROUND STREAM
THAT CONVEYS RUNOFF FROM A LARGE AREA. FLOODING
CAN OCCUR FROM HIGH FLOWS IN THE CHANNEL AS WELL
AS FROM HIGH WATER LEVELS IN SPRINGBROOK CREEK
BACKING UP INTO THE CHANNEL. THE ANTICIPATED PEAK
WATER LEVEL AND FLOW FOR THE 2-YEAR FLOOD ARE
APPROXIMATELY ELEVATION 13.5 (NAVD 88) AND 25 CFS
RESPECTIVELY. AT A MINIMUM, THE CONTRACTOR SHALL
PROVIDE THE FOLLOWING:
TEMPORARY DIVERSION DAMS UPSTREAM AND DOWNSTREAM
OF WORK AREA CAPABLE OF WITHSTANDING WATER LEVELS
UP TO ELEVATION 13.5.
TEMPORARY DIVERSION PIPING, TWO 18-INCH DIAMETER
SMOOTH WALL PIPE MINIMUM.
IN ADDITION TO THE TEMPORARY DIVERSION PIPING, HAVE
EQUIPMENT PREPARED TO PUMP AND/OR DIVERT FLOWS UP
TO A TOTAL OF 30 CFS ON STANDBY.
BOX CULVERT STRUCTURAL NOTES:
1. PRECAST 4-SIDED BOX CULVERT CONSTRUCTED TO THE
OVERALL DIMENSIONS SHOWN ON THE DRAWINGS SHALL MEET
THE REQUIREMENTS OF ASTM C789 EXCEPT AS MODIFIED BY
THESE PLANS AND NOTES. SHOP DRAWING SUBMITTALS FOR
BOX CULVERT SHALL INCLUDE STRUCTURAL CALCULATIONS
STAMPED AND SIGNED BY A REGISTERED PROFESSIONAL
ENGINEER IN THE STATE OF WASHINGTON.
2. TONGUE AND GROOVE JOINTS SHALL BE SEALED WITH A
ONE-PIECE ELASTOMERIC SEAL SPECIFICALLY DESIGNED FOR
BOX CULVERTS. (OMNI-FLEX GASKETS OR EQUAL.)
3. ACCESS RISER SHALL BE LARGE ENOUGH TO ACCOMMODATE
A 36 INCH OPENING FRAME AND GRATE. INCLUDE LADDER
ASSEMBLY. CONTRACTOR SHALL SUBMIT SHOP DRAWINGS
FOR REVIEW AND APPROVAL.
