HomeMy WebLinkAboutCO 130 9 C.O. #13 CITY OF RENTON Page 1 of 1 Public Works Department CHANGE ORDER NO. 13 CONTRACT CHANGE ORDER AGREEMENT CONTRACT: South Lake WA Roadway Improvements (Garden Ave. Widening) CAG -12 -021 CONTRACTOR: ICON Materials DATE: March 13, 2013 SUMMARY OF PROPOSED CHANGE: Reason /Justification: After construction of the concrete pavement widening on the east side of Garden Ave. N., it was observed that the flat grade of the street created three "bird bath" locations. Curb cuts were constructed at these three locations (Sta. 40 +83, Sta. 41 +55 and Sta. 43 +85) to facilitate drainage (See RFI #20). It was determined by the City that permeable aggregate should be installed at the back of curb to disperse the runoff at these particular locations. See attached detail for proposed installation of "permeable aggregate" between N. 10th St. and Lowe's driveway. Contract Change: This work is to be performed on a force account basis with complete force account sheets signed by the City s representative on the day the work occurs. The Contractor shall submit force account sheets, corresponding invoices and required supporting documentation. Rental equipment shall be paid for by invoice and fuel shall be paid for separately. If Contractor -owned portable changable message sign (PCMS) is used, payment shall be per bid item. Add the followinq quantities to existinq bid item plan quantities in the schedule of prices.
