The said certification shall include a stress life curve per ASTM D-2837 . The stress regression testing shall have been done in accordance with ASTM D2837 , and the manufacturer shall provide a product supplying a minimum Hydrostatic Design Basis (HDR) of 1 , 600 psi , as determined in accordance with ASTM D 2837 . Further , the material shall be listed by PPI ( the Plastics Pipe Institute, a division of the Society of the Plastics Industry) in PPI TR-4 with a 730F hydrostatic design stress rating of 800 psi , and a 14001F hydrostatic design stress rating of 400 psi. The PPI Listing shall be in the name of the pipe manufacturer, and shall be based on ASTM D 2837 and PPI TR-3 testing and validation of samples of the pipe manufacturer' s production pipe. The MANUFACTURER' s certification shall state that the pipe was manufactured from one specific resin in compliance with these specifications. The certificate shall state the specific resin used, its source, and list its compliance to these specifications. IV. Pipe Extrusion The pipe shall be extruded using a melt homogenizing/plasticating extruder and "appropriate" die. The extruder screw design should be customized for the HDPE being processed to minimize melt fracture of the molecular structure thus reducing the molecular weight and changing some physical properties from resin to pipe . The resin should be processed at its melt temperature of 3750F to 4250F. The extruded tubular melt will be vacuum or pressure sized in downstream cooling tanks to form round pipe to specification diameter and wall thickness with a "matt-finish" surface. Pipe Pipe supplied under this specification shall have a nominal IPS ( Iron Pipe Size) OD unless otherwise specified. The SDR (Standard Dimension Ratio) , and the pressure rating of the pipe supplied shall be as specified by the engineer. The pipe shall be produced with the nominal physical properties outlined in Section III , and to the dimensions and tolerances specified in ASTM F-714 . Additionally , the pipe shall be inspected per industry accepted manufacturer standards for: * Diameter * Straightness * Wall Thickness * Ovality * Concentricity * Toe-In * Quick Burst * Overall Workmanship Pressure and Inspection on ID & OD Ductility * Print Line * Joint Length The pipe shall contain no recycled compound except that generated in the MANUFACT'URER's own plant from resin of the same specification from the same raw material. The pipe shall be homogenous throughout and free of visible cracks , holes, voids, foreign inclusions, or other deleterious defects, and shall be identical in color, density, melt index and other physical properties throughout. 2 . Pipe Performance The pipe shall be in compliance with the physical and performance requirements of Section III of this specification . Specifically, the pipe will be extruded from resin meeting specifications of ASTM D 3350 with a cell classification of PE: 345434C; and ASTM D-1248 pipe grade resin type III, Class C, Category 5, grade P34 polyethylene compound. The pipe shall exhibit the short tern tensile and compressive physical properties listed in Section III . and the pipe shall provide the long term endurance characteristics recognized by: the compressed pipe ring environmental stress crack resistance greater than 1000 hrs ; the slow crack growth resistance greater than 32 days ; and the impact strength (toughness) greater than 42 in-lb/in notch. 3 . Fittings The standard HDPE fittings shall be standard commercial products manufactured by injection molding or by extrusion and machining, or, shall be fabricated from PE pipe conforming to this specification. The fittings shall be fully pressure rated by the manufacturer to provide a working pressure equal to the pipe for 50 years service at 73 . 40F with an included 2 : 1 safety factor. The fittings shall be manufactured from the same resin type , grade , and cell classification as the pipe itself. The manufacture of the fittings shall be in accordance with good commercial practice to provide fittings homogeneous throughout and free from crack, holes, foreign inclusions, voids , or other injurious defects . The fitting shall be as uniform as commercially practicable in color, opacity, density and other physical properties . The minimum "quick-burst" strength of the fittings shall not be less than that of the pipe with which the fitting is to be used. VI . Joining Sections of polyethylene pipe should be joined into continuous lengths on the job site above ground. The joining method shall be the butt fusion method and shall be performed in strict accordance with the pipe manufacturer ' s recommendations . The butt fusion equipment used in the joining procedures should be capable of meeting all conditions recommended by the pipe manufacturer, including, but not limited to, temperature requirements of 4000F , alignment , and 75 psi interfacial fusion pressure. Butt fusion joining shall be 100% efficient offering a joint weld strength equal to or greater than the tensile strength of the pipe. Socket fusion shall not be used. Extrusion welding or hot gas welding of HDPE shall not be used for pressure pipe applications nor in fabrications where shear or structural strength is important . Flanges , unions , grooved-couplers, transition fittings and some mechanical couplers may be used to mechanically connect HDPE pipe without butt fusion. Refer to the manufacturer's recommendations. VII . Quality and Workmanship The pipe and fitting manufacturer's production facilities shall be open for inspection by the owner or his designated agents. During inspection, the manufacturer shall demonstrate that he has facilities capable of manufacturing the pipe and fittings required by this specification, that a quality control program meeting the minimum requirements of D3035, and ASTH F-714 is in use, and that facilities for performing the tests required by this specification are in use. i The engineer may request certification that the pipe produced is represented by the quality assurance data. Additionally test results from the manufacturer' s testing which show the pipe does not meet appropriate ASTM standards of manufacturer's representation, will be cause for rejection of the pipe represented by the testing. These tests may include density and flow rate measurements from samples taken at selected locations within the pipe wall and thermal stability determinations according to ASTM D 3350, 10. 1. 9 . The owner or the specifying engineer may request certified lab data form the manufacturer to verify the physical properties of the materials supplied under this specification or at his own expense may take random samples for testing by an independent laboratory. QA Deviations - If an approved supplier must supply material that does not meet all requirements of this specification, he must notify the specification engineer via a written description of the deviation with data that shows the magnitude of the deviation, the justification for the deviation from this specification, and the worst case, long term impact of the deviation on the project. The decision, prior to shipment, to accept material deviating from this specification shall be the responsibility of the specifying engineer. QA Verification - The owner or the specifying engineer may request certified lab data to verify the physical properties of the compounded materials supplied under this specification, or, may have random samples taken and have them tested by an independent laboratory. Such testing will be at expense of the party requiring verification testing. Requests for verification must be submitted in writing and mutually acceptable arrangements made. QA Rejection - Polyethylene pipe and fittings may be rejected in whole or in part by the specification engineer for failure to meet any of the requirements of this specification. QA Records - QA/QC records shall be maintained intact for a minimum of one year from date of production. VIII . Pipe Packaging, Handling, Storage The manufacturer shall package the pipe in a manner designed to deliver the pipe to the project neatly, intact, and without physical damage. The transportation carrier shall use appropriate method and intermittent checks to insure the pipe is properly supported, stacked, and restrained during transport such that the pipe is not nicked, gouged, or physically damaged. Pipe shall be stored on clean, level ground to prevent undue scratching or gouging of the pipe. If the pipe must be stacked for storage , such stacking shall be done in accordance with the pipe manufacturer's recommendations. The handling of the pipe shall be done in such a manner that it is not damaged by dragging over sharp objects or cut by chokers or lifting equipment. Sections of pipe having been discovered with cuts or gouges in, excess of 10% of the wall thickness of the pipe shall be cut out and removed. The undamaged portions of the pipe shall be rejoined using the butt fusion joining method. Fused segments of pipe shall be handled so as to avoid damage to the pipe. When lifting fused sections of pipe, chains or cable type chokers must be avoided. Nylon slings are preferred. Spreader bars are recommended when lifting long fused sections . Care must be exercised to avoid cutting or gouging the pipe. r X. Construction Practice Construction and installation shall be performed in compliance with the manufacturers Design Guidelines and Installation Guidelines, and this specification. 1. Trench Construction The trench and trench bottom should be constructed. Trenching should be done in accordance with ASTM-D-2321 - Section 7 . 2 . Embedment Material Embedment materials should be Class I, Class II, or Class III materials as defined by ASTM-D-2321-Section 6. The use of Class IV and Class V materials for embedment is not recommended and should be done only with the approval of the engineer. Class I crushed stone and Class II well graded dense aggregates are preferred, and shall have an installed density of at least 85% Standard Proctor Density through compaction or consolidation. 63 . Bedding Bedding of the pipe should be performed in accordance with ASTM-D-2321-Section 8 . Compaction rates should be as specified in ASTM-2321. Deviation from the specified compaction rates should be done only with the approval of the engineer. 4 . Haunching and Initial Backfill Haunching and initial backfill should be as specified in ASTM-D-2321-Section 9 using Class I, Class II, or Class III materials. Materials used and compaction rates shall be as specified by the engineer. In cases where a compaction rate of 85% Standard Proctor Density is not attainable, the engineer may wish to increase the SDR of the pipe to provide adequate stiffness . 5 . . Special Conditions ASTM- D -2321-Section 11 . 2 , Minimum Cover for Load Application, Section 11. 3 , Use of Compaction Equipment,and Section 11 . 4 , Removal of Trench Protection should apply unless directed otherwise by the engineer. 6. Testing Gravity flow pipelines shall be tested in accordance with the requirements and specifications o8f the engineers of record. HDPE pressure pipe shall be tested in accordance with the specification and requirements of the engineer of record and the manufacturer's recommendations, or in accordance with the guidelines and procedures taken from PPI Technical Report TR 31/9-79, at the pressure rated capability of the pipe based on the temperature at the time of hydro-test . Refer to the manufacturer' s temperature related pressure ratings, and hydro-teat the pipe at 1.5 x that pressure rating for 2 to 3 hours per TR 31/9 . Methodology. The horizontal directional drilling (HDD) method for performing crossings includes three distinct phases beginning with the drilling of the pilot hole from the surface on one side of the obstacle to be crossed. Drilling continues along a designed profile below and beyond the obstacle to exit at the surface of the other side. The second phase entails the reaming of the pilot hole to a diameter sufficiently large enough to accept the pipeline. Finally,the pipeline is pulled into place within the enlarged hole. 2. Operational Procedures. Pilot Hole — A small diameter drill string penetrates the ground at the prescribed entry point at a predetermined angle. The drill string is then advanced joint by joint using the hydraulic action of fluid-assisted drilling. The end of the drill string has a bend or bias to give the directional control needed to follow the proposed drill path. At the completion of each joint, the location of the drill head is obtained by use of a walkover system or a wireline steering tool. It is recommended that readings be taken at least every 30 feet. This information is then plotted against the proposed design plan and profile to determine the direction the next joint will be drilled. Once the drill head surfaces at the exit point, the drill head and related tooling will be removed so the reaming and pullback can commence. Reaming — In general, the final bore should be at least 1-1/2 times the outside diameter of the product line. This is necessary to allow for an annular void for the return of drilling fluids and spoils and to allow for the bend radius of the product line. Depending on the size of the desired final hole and the soil conditions, reaming may consist of one or more passes. Methodology for Installation of Direction Drill Pipe Page 1 of 2 Pullback— Once the drilled hole is enlarged, the pipe or conduit can be pulled back into the reamed hole filled with drilling fluid. The pipe that has been fabricated on the exit side can now be readied for pullback. The drill pipe is connected to the product line using a pullhead or pulling eye and a swivel. A reamer is also placed between the pullhead and the drill string to ensure that the hole remains open and to allow more lubricating fluid to be pumped into the hole during the pullback. 3. Verification of Meeting Elevations Shown on Plans Walkover Locating System —The method relies on a transmitter or sonde that is placed in a housing locating behind the drill bit. The sonde sends a signal to the surface and is picked up by a receiver held by the locator. The locater is able to monitor the path of the bore by reading and analyzing the data provided by the receiver and take drilling logs for future reference. Drilling logs are hand-written records that are kept by both the locator and the driller. These records include specific dates, times and locations, soil conditions, drilling data such as depth, angle and rate of penetration and utility crossings. From this data, profiles of the bores can be created. 5. Stand shield up, attach tag lines and install into trench property. 6.0 INSPECTION 6.1 The comperent person must r valuate the soils to assure the rated capacity of the Trench Shie'.ds is not exceeded by the lateral pressure of the soil. Soils shall be evaluated in accordance with Part 3.0. 6.2 The compdent person shall monitor all phases of the assembly, installation and use of this product to evaluate and eliminate methods which could endanger employees utilizing this product. 6.3 Daily inspections of the Trench Shield and accessories must be performed by the competent person and deficiencies corrected. 6.4 Inspections shall be conducted as necessary for hazards associated -Aath: water accumulauon, changing soil conditions, or changing site weather conditions. 6.5 Stacked shields shall be monitored to assure that each panel is sec-,red to the one below it. Four f= high stacking shields (those with only 2 spreaders) shall not be used as the bottom shield. 7.0 SAFETY SPECIFICATIONS 7.1 Employees shall be protected from loose or falling material. Trench Shields must always be used in a manner that loose or falling soil cannot enter over the top or through the end of the shield. End plates may be required. Spoil pees must be kept back from the edge of the excavation at least 2 feet. 7.2 Employees shall not enter or exit shields through unprotected areas and shall remain in shields at all times while working. 7.3 Employees shall not be in or under a shield while it is being lifted or moved 7.4 Bottom of shields may be a maximum of 2 feet above the bottom of the trench if there are no signs of deterioration of the trench face below or at the end of the shield. 7.5 Use of the spreader system for any purpose other than support for the side wall panels, or for pulling them forward is prohibited without written permission from the manufacturer. 7.6 The sides of the excavation should be cut vertical and narrow to prevent lateral movement of the shield. The width of the excavation shall be no wider than the width of the shield plus 12 inches. 7.7 A Speed Shore supplied arch spreader may be used in place of standard spreaders. Buchan Construction, Inc. which is located off E- Lake Sammamish Parkway near Redmond, Washington. The project was started on July 20, 1998 with a two to three week projected completion date. We have encountered very hard subsurface drilling conditions and a one week work stoppage for the PGA Golf Tournament. Snelson has also experienced a work stoppage that resulted from drilling fluid surfacing in several locations, including around several homes, along driveways and public streets. We have restairted drilling operations and have completed 850 feet of the pilot bore. Drilling is very slow with only 30 feet of progress on August 21, 1998, We will need to conduct two or three back-ream operations when pilot bore is completed before pull-back of the sewer line can be achieved. We cannot anticipate what further difficulties we will encounter or what production we will be able to make before this project is complete and mobilization for the NW 71h Street Sewer Project can begin. As respects; (a)work performed by the NAMED INSURED for or on behalf of the OWNER; or (b) products Sold by the NAMED INSURED to the OWNER; or (c) premises leased by the NAMED INSURED from the OWNER, the insurance afforded by this policy shall be primary insurance as respects the OWNER, or any other insured,its elected or appointed officers,officials,employees,subconsultants or volunteers; or stand in an unbroken chain of coverage excess of the NAMED INSOR.ED'S scheduled underlying primary coverage. In either event, any other insurance maintained by the OWNER, or any other insured, its elected or appointed officers, officials,employees, subconsultants or volunteers shall be in excess of this insurance and shall not contribute with it. 3. SEVERABILITY OF INTEREST. The inclusion of more than one Insured under this policy shall not affect the rights of any Insured as respects any claim, suit or judgment made or brought by or for any other Insured or by or for any employee of any other Insured.
