The Federal Emergency Management Agency (FEMA) will reimburse 75 percent of the
eligible costs associated with your approved projects. The State of Washington will pay for
12.5 percent of the eligible nonfederal share costs, leaving the City responsible for the
remaining 12.5 percent. The state share will be dispersed on any prior payments after the
contract amendments have been signed.
Please note the required conditions on the attached Project Worksheet (PW) that have
been highlighted. In addition, please note the established mandatory work completion date.
If you do not agree with the determinations that have been made regarding project eligibility,
time limits, funding, or any other determination made about the Project Worksheet, an
appeal procedure requires the City to make a written request for appeal directly to this
office. The appeal shall contain documented justification supporting the City`s position,
specifying the monetary figure in dispute and the provisions in federal law, regulation, or
policy with which the City believes the initial action was inconsistent. Two copies of any
related documentation supporting the appeal must be attached to the letter of appeal. This
request for an appeal must be made within sixty days of the date of receipt of this package.

02 May 07: Ron Straka,City of Renton, documents no chinook use this channel. The creek is too small. A survey is taken when they trap the
fish prior to cleaning out the sediment pond during maintenance. There are coho&sockeye that use the creek, but no ESA-listed species. Brian
Missildine, USFWS, said there were no issues, if chinook do not use that stream leading to the sediment pond,the fish are diverted to an overflov
channel,and they remove the fish before cleaning out the pond(02 May 07). No effect to ESA-listed aquatic species. There is an eagle nest

Renton's streets, parking lots, park trails and pathways. The city's Parks Department utilized force account labor and
equipment to collect and dispose of the flood debris to their normal disposal site.
SCOPE OF WORK
WORK COMPLETED: Several force account regular labor hours were used by the sixteen (16) Parks Department
employees in their debris collie tion and removal process with 2.5 hours of overtime by a single employee being eligible for
reimbursement. Seventeen (W pieces of equipment used in the debris collection and removal process provided 71.5 hours
service. Documentation provided by the city has been reviewed for accuracy and eligibility and is available for audit.

02 May 07: Ron Straka,City of Renton,documents no chinook use this channel. The creek is too small. A survey is taken when they trap the
fish prior to cleaning out the sediment pond during maintenance. There are coho&sockeye that use the creek, but no ESA-listed species. Brian
Missildine, USFWS, said there were no issues,if chinook do not use that stream leading to the sediment pond,the fish are diverted to an overflov
channel,and they remove the fish before cleaning out the pond(02 May 07). No effect to ESA-listed aquatic species. There is an eagle nest
approximately 0.33 miles from the pond. The activity from the adjacent golf course and the highway have acclimated this eagle pair to human
activity(verified by Brian Missildine, USFWS,24 Ap 07). No effect to bald eagles. Suzy Lutey, Environmental Specialist.
Fish and Wildlife Coordination Act(FWCA) Completed

in Appendix B, Programmatic Allowances I.K.This Allowance refers to debris removal in non-historically senstive public areas, ie.,sediment
pond.The work is 0%complete at the time of this review, but if historical or archaeological materials are discovered during debris removal
activities,the Applicant is required to stop ground disturbing activities within 50 ft.of the discovery and immediately contact FEMA and EMD.
Work may not proceed until FEMA has assessed the findings for historic significance, as required by the National Historic Preservation Act
(NHPA). Non-compliance with this condition will jeopardize the receipt of federal funding.Any change to the approved scope of work will require

Maplewood Creek drains a 1,032 acre area north of the Maplewood Golf Course. The Maplewood Creek Sediment Basin is
I ocated on the Golf Course and has a normal capacity of about 1000 cubic yards. The basin had about 496 cy of sediment
in it before the November 2006 floods. The rains and flooding from November 2 to 11, 2006, caused additional erosion in
Maplewood Creek and deposited a large amount of sediment and debris in the Maplewood Creek Sediment Basin. The
flooding deposited about 4.3ft of sediment over a 26ft x 90ft area and about 1ft of sediment over a 3390sf area. Overall the
November Floods deposited about 506 cubic yards of new sediment in the basin, filling it to capacity.
SCOPE OF WORK
Work To Be Completed: The proposed scope of work is to clean the entire sediment basin in the summer of 2007. The work
involves design, permitting, preparing construction plans and specifications, bidding, project management, inspection,
temporarily diverting the creek around the basin, removing all sediment from the basin, hauling and disposing of the
sediment off-site, restoring the basin side slopes and bottom, removing the diversion and filling the basin with water. The
work will be publicly advertised for sealed bids and will be awarded to the private contractor with the lowest responsive bid.
The FEMA Project Worksheet project cost is for the proportional costs for removing the additional 506cy deposited by the
November Floods. The costs for cleaning the sediment that was present before the November Floods will be part of the
City's maintenance costs for the basin. The cost estimate is based on costs when the City cleaned the basin in 2004.

DAMAGE DESCRIPTION AND DIMENSIONS
Maplewood Creek drains a 1,032 acre area north of the Maplewood golf course. The Maplewood Sediment basin is located on the
Golf Course and has a normal capacity of about 1000 cubic yards. The basin had about 500 cy of sediment in it before the November
2006 Floods. The rains and flooding from November 2 to 11,2006 caused additional erosion in Maplewood Creek,and deposited a
large amount of sediment and debris in the Maplewood Creek Sediment Basin. The flooding deposited about 4.3 feet of sediment over a
26 ft x 90 ft area,and about 1 ft of sediment over a 3390 sf area. Overall,the November Floods deposited about 506 cubic yards of new
sediment in the basin,filling it to capacity.
SCOPE OF WORK
The proposed Scope of Work is to clean the entire sediment basin in the summer of 2007. The work involves design,permitting,
preparing construction plans and specifications,bidding,project management, inspection,temporarily diverting the creek around the
basin,removing all sediment from the basin,hauling and disposing of the sediment off-site,restoring the basin sideslopes and bottom,
removing the diversion and filling the basin with water. The work will be publicly advertised for sealed bids,and will be awarded to the
private contractor with the lowest responsive bid.
The FEMA Project Worksheet project cost is for the proportional costs for removing the additional 506 cubic yards deposited by the
November Floods. The costs for cleaning the the sediment that was present before the November Floods will be part of the City's
maintenance costs for the basin. The cost estimate is based on costs when the City cleaned the basin in 2004.

SCOPE OF WORK
The work involved under the terms of this contract document shall be full and complete
installation of the Project, as shown on the plans and as described in the construction
specifications, including but not limited to:
• constructing a flow bypass structure and divert the creek into an existing bypass pipe
• removing approximately 1,000 cy of sediment from the Maplewood Creek Sediment Basin
and flow splitter
• hauling and disposing of sediment
• restoring flow to the sediment basin
• hand placing streambed gravel at selected locations
• and landscape planting and hydroseeding around the basin.
The estimated project cost is $36,000 to $42,000.
A total of 15 working days is allowed for completion of the project.
Any contractor connected with this project shall comply with all Federal, State, County, and
City codes and regulations applicable to such work and perform the work in accordance with
the plans and specifications of this contract document.

A.GENERAL IMPACT:
1. Identify and describe the damages that constitute a health and/or safety hazard to the general public.
Emergency response was need by the City to help protect people and public and private property from injury and damage due to flooding.
Numerous areas of public property were damaged and will need repair. Areas along the Cedar River may have the severest damage. The
exact extent of some damages can not be determined until the water recedes.
2.Describe the population adversely affected directly or indirectly by the loss of or the damage to these public facilities.
The general population is impacted by damage to public facilities including storm water facilities and public parks. People and businesses
near the Cedar River, including the Boeing 737 aircraft plant, could be affected by damage to flood protection walls along the river.
Operations at the Renton Airport seaplane base have been affect by debris and sediment.
3.What economic activities are adversely affected by the loss of the public facilities or the damage to the facilities?
Park use,the City golf course, and businesses around those area could be adversely affected. Revenue loss to the golf course. The City
Airport and Boeing 737 aircraft plant could be affected by failure of the flood wall. The Renton seaplane base has difficulty operating and
would lose customers,tenants and substantial revenues. The Elliot Spawning Channel is required as part of the USACE Cedar River 205
Project and provides mitigation to allow future project maintenance.
B.RESPONSE CAPABILITY.
1. Describe how the applicant intends to repair the damage and the schedule to accomplish the work.
Damaged areas will be assessed and repair work estimated and prioritized. Resources will need to be allocated in the City Budget for repairs.
As a result other City projects will be delayed. Repairs could take several years or more.
2.Describe the source and availability of funds to accomplish the repairs.
The City has committed some budget funding that was restricted for Law Enforcement, Safety, and the Library. The City does not have extra
capacity available for Emergency Disasters(the November Flooding).
3.How quickly can the damages be repaired without degradation of public services?
The City will prioritize repairs to protect people and property. Some public services may be affected since some repairs could take months or
years. The repairs may delay other planned projects,which could affect the public.
C.IMPACT ON PUBLIC SERVICES IF A DECLARATION IS NOT MADE (e.g.,Defer permanent repairs, impact ongoing services, improvements,etc.)
Damage repairs would have to be done and other planned improvements would have to be delayed. Due to the lack of funding impacts to
services and operations at the Renton Municipal Airport,which would impact public safety and revenues. Impact to revenues at City golf
course and recreational benefits due to Parks facility impacts.

Damage to the permeable berm structure adjacent to the Cedar River. High flows have damaged the permeable weir top berm's top elevation
and caused excessive main river sediments to deposit into the channel bottom. Damage to the function of the channel of all spawning gravel
from the base of the channel for entire length of channel. Damage to channel side slopes and planted vegetation. Fabric exposed along
channel sides.
