DIRECTIONS FOR USE
Request for Approval of Source of Supply of Materials shall be prepared by the Contractor and forwarded to the
Project Engineer promptly after award of the contract. The Contractor shall provide all information required on the form
to identify the project and describe his intended sources of supply, sign the form, and submit ALL copies to the
Project Engineer.
The Project Engineer shall review all items for conformance to the contract requirements; he shall then sign the
approval making appropriate notations, and forward: (1) the White copy to the Headquarters Materials Laboratory, (2)
the Pink copy to the District, (3) the Canary copy to the Contractor and (4) retain the Goldenrod Copy, Copies 2, 3,
and 4 are information copies only. Notification of approval or rejection by the Materials Engineer will be forwarded to
the Project Engineer by copy of this completed form. The Project Engineer will inform the Contractor of the approval
action on the proposed sources.
Approval of source of supply does NOT constitute acceptance of materials for incorporation into the work. The
engineer will conduct such tests as he deems necessary on representative samples of each material before approving
such material for incorporation into the work. See Section 1-06 of the Standard Specifications.
In general, the Contractor must request approval of source of supply for ALL materials required on the work except
where he intends to use sources provided in the Special Provision for the project.
Identify items by Bid Numbers when applicable. It is not necessary that a separate approval request be made for each
different type or size of the same material or product where bid items for several different type or sizes are included
in the contract; for example, request for approval of source of culvert pipe should be combined in a single source
request unless more than one supplier will be furnishing the pipe. Request for approval of ready-mix concrete must
include the brand of cement and identification of the aggregate source.
If the Contractor desires to use a source other than that provided in the Special Provisions, the request for approval
of the alternate source must be accompanied by a letter from the Contractor requesting the change in source. The
letter should be addressed to the Project Engineer and forwarded with the District's recommendations to the
Headquarters Materials Engineer.

SUBJECT: <PANTHER CREEK and SR-167 Utility Permit No. 12682
Dear Mr. Lutz:
Attached are three sets of revised plans for the referenced project. The
Soils Conservation Service is designing enhancements to the Panther Creek
Wetlands, for construction in 1988. The design calls for plugging all
culverts under SR-167, and directing the drainage out through the P-9
Channel . As a result, the money put into the referenced project would be
wasted since these culverts are to be plugged. We have therefore revised
the plans to reflect a temporary solution prior to the S.C.S. construction.
I have met with Bob Winters and Jim Olsen to discuss the project. We have
matched inverts of the orifice with the pipe as requested by Mr. Olsen, to
address maintenance concerns.
The permit which we have for the present plans became void as of 12/31/87.
Therefore we would like to get a new permit or extend the existing one.

Dear Mr. Pape:
The City Council authorized award of the contract to your firm at the regular meeting
on November 16, 1987. You should be receiving a notice of bid award from the City
Clerk's office.
In order for us to properly process the contract for the project, we will need the
following documents submitted as early as possible:
1. Six (6) copies of the Contractor's Certificate of Insurance,
2. Six (6) copies of your current State Contractor's License,
3. Six (6) copies of your current City of Renton Business License.
The following information is also required and should be submitted prior to the
preconstruction conference:
1. A list with the name, address, telephone number, and emergency phone number for:
Responsible Officer, Job Foreman, and Bonding Agent.
2. A list of the Prime Contractors and Subcontractors along with an Intent to Pay
Prevailing Wages Form which will be posted on site per WSDOT standards.
3. A list of the Suppliers and the completed Request for Approval of Material Sources
form.
4. A Construction Progress Schedule showing the proposed dates of commencement
and completion of each of the various pay items of work.
