Because the project is even numbered, the cutoff date for the progress estimates will be the on the 20th of
the month.
D. Boardwalk plans, are required. Needs to be Submitted for approval.

Payrolls are required for State funded projects to document items paid by the hour, such as Force
Account and Traffic Control Labor.
All subcontractors including trucking companies shall submit a *Request to Sublet Work (Form No.
421-12EF) and including Prime, a *Statement of Intent (Form No F700-029-000) before any work
is started or paid for on Project and shall be enforced. The prime and subcontractors including
trucking companies shall submit an *Affidavit of Wages Paid (Form No. F700-007-000) after work
is completed. Wage rage interviews shall be conducted as directed by the Project Engineer.
F. Provide required jobsite posters to the Contractor.
Where will the posters be displayed?
G. The Davis -Bacon act requires the USDOL, FHWA, and W SDOT to take an active role in investigation for
labor compliance. The Davis -Bacon act statement included in the Safety Check List has ❑ has not ❑
been signed by the Contractor.
Note that all trucking companies delivering asphalt or spreading, leveling, rolling, or otherwise participating
in the incorporation of the materials into the project are subject to prevailing wage laws and are to submit a
Statement of Intent to Pay Prevailing Wage and Affidavit of Wages Paid. All trucking companies hauling
material away from the jobsite are considered to be performing a discreet part of the work and must be
approved as subcontractors. Note also that Statements of Intent to Pay Prevailing Wage and Affidavits of
Wages Paid are required from manufacturers of non-standard items.
H. Lack of the following documents can cause delay of the initial progress payment:
1. Prime Contractor's Statement of Intent to Pay Prevailing Wage. See Standard Specifications Section
1-07.9.
2. Progress Schedule. See Standard Specifications Section 1-08.3.
Failure to submit the required reports by their due date may result in the withholding of progress estimate
payments. See Standard Specifications section 1-07.11(10)B.
I. Records are to be retained and available for inspection a minimum of three years after the Project
Completion date for all projects.

4. SUBCONTRACTORS AND AGENTS
A. Request to Sublet (Form 421-012) must be submitted and approved before the subcontractor begins work.
An Approved Washington State Department of Labor and Industries' Statement of Intent to Pay Prevailing
Wage is also needed before any payment on the subcontractor's work will be made.

5. MATERIALS DOCUMENTATION Materials Documentation Engineer
A. Please use the Record of Materials (ROM), which is delivered to the Contractor at this meeting as a guide
for materials approval submittals. Please use the Bid Item Numbers as listed on the ROM when completing
your Requests for Approval of Materials or Qualified Products Lists.
B. A list of Qualified Products proposed for installation and/or Requests for Approval of Material (RAM) (DOT
Form 350-071) must be submitted as soon as possible, and before the material is incorporated into the
project. The earlier, the better. One copy of the Qualified Products List is available from the Project Office,
or a two-year subscription may be ordered from W SDOT Engineering Publications for $15.00 by calling
(360) 705-7430 or on the Internet at:
http://www.wsdot.wa.gov/fasc/EngineeringPubiications/order. htm.
Additionally, the manual may be downloaded free of charge on the Internet at:
http://www.wsdot.wa.gov/biz/mats/qpl/gpl.cfm.
The hardcopy of the QPL is updated only once each year in January. However, the on-line QPL is updated
constantly and should be reviewed prior to submitting any materials documentation.
C. Please see the sheets entitled "Materials Documentation Requirements" which are delivered to the
Contractor at this meeting.
D. The Contractor shall ensure that commercial weighers complete and submit Scaleman's Daily Reports in a
timely manner.
E. No payment will be made for material without proper documentation.
6. TRAFFIC CONTROL AND SAFETY

B. Traffic Control Plans appear in the Contract Plans. Does the Contractor wish to adopt these standard

Contractor's plan of operation, the Contractor may propose modified plan(s). The Contractor's letter
requesting approval of any modified plan(s) must be submitted ten days in advance of need.
C. Flaggers shall wear standard flagging vests and yellow hard hats and shall use standard paddles.
Flaggers shall have a current flagging card prior to being assigned duties as a flagger. All employees on
the jobsite shall wear high visibility apparel in accordance with section 1-07.8 of the Standard Specifications
(2002).
D. Traffic Control Hours of Operation, are listed in the Contract Provisions on Page(s) 76

E. Access for Emergency Vehicles in project area (Fire, Police, Ambulance, etc.) must be maintained.
F. Construction equipment is required to follow all speed limit and other laws on the project. W SDOT will
cooperate with law enforcement agencies in the enforcement of legal load limit requirements and the
covered load and six inch freeboard regulations. The Contractor is responsible for obtaining any permits
needed for hauling material off -site.
G. Prior to work starting on the project the Contractor and the Project Engineer shall each designate an
individual responsible for construction traffic control. The State's representative will be:

4. Prior to commencement of any on site construction activities, the Contractor shall prepare a project
specific spill prevention, control and countermeasures (SPCC) plan to be used for the duration of the

D. The Department of Ecology requires registration of rock crushers in accordance with WAC 173-400.
8. WORKING RELATIONSHIP AND COOPERATION PARTICIPATING ON PROJECT
A. A final Contractor Performance Report will be prepared at the completion of work in accordance with
Section 1-05.13 of the Standard Specifications. Discuss issues regarding deficiencies in performance, and
"Interim" reporting procedures.
The Contractor will be given the opportunity to fill out an evaluation of WSDOT at the end of the contract.
WSDOT asks that the Contractor fill this report out honestly as the only way we can improve our operations
is to know what parts need fixing.
B. Discuss WSDOT / Contractor relationship concerning cooperation. Include communication of planned work
to the Project Inspector and Chief Inspector weekly.
C. To track correspondence more efficiently, all letters from WSDOT to the Contractor will be numbered
consecutively (SLN 001, SLN 002, etc). We ask that the Contractor also consecutively number their letters
to WSDOT.
