Test results transmitted herein are only applicable to the actual samples tested at the location(s) referenced and are not necessarily indicative of the properties of other apparently similar or identical materials.CT0001, 10-16-13, Rev.10 Page 1 of 1 Permit No.: C19002723 GENERAL INFORMATION On site for observation of asphalt placement in the alleyway behind the existing parking garage – a 10x20 section of roadway patch was placed. The construction documents referenced for this inspection include: site plans SUMMARY OF SERVICES Observed the placement of approximately (17) tons of 1/2” HMA Asphalt 58H-2 in road of the Southport alleyway behind the existing parking garage. A Rice value of 158.6 was provided by Lakeside Paving. Asphalt Rice density tested by Mayes/Terracon on 12/16/21 (report M7211711.0006A) was obtained from a sample taken at the Issaquah batch plant, providing a Rice of 160.3. The contractor placed (2) lifts – both were 2” thick. Lakeside Paving placed asphalt by hand – (1) roller was utilized by contractor during asphalt placement, a HAMM HD-12 vibratory smooth drum roller. All lifts were compacted to achieve the minimum compaction specification of 92% or greater. Random tests were taken at various locations, each test taken exceeded the client’s minimum compaction specification of 92% or greater using Rice values supplied. SUMMARY OF TESTING All testing meets the 92% compaction rating on site. See attached Field Density Test Report for additional information. The results of observation(s) for today were reported to Lakeside Paving. COMPLIANCE STATEMENT To the best of our knowledge, the items inspected today are in conformance with approved plans and specifications.
