GENERAL INFORMATION
On site for concrete placement inspections of the west half of the ramp and sidewalk for the Park Avenue
north extension. The ramp and sidewalk inspected to plan and notes on C401 of project drawings. The ramp
reinforcement was placed per detail 8/C410, and the sidewalk to detail 7/C410 and is non-reinforced.
CONCRETE PLACEMENT
Observed concrete placement as follows:

TESTS PERFORMED
The following field tests were performed: Slump (4-3/4”), Air Content (4.7%), Concrete Temp. (67°F), Ambient
Temp. (37°F).
A total of (4) compressive strength specimens [1 Set] were fabricated for lab-curing.
The results of observation(s) for today were reported to the contractor.
COMMENTS
Ramp was placed via pump and mechanically vibrated for consolidation. Sidewalk was placed from truck chute
and was hand-vibrated for consolidation.
COMPLIANCE STATEMENT
To the best of our knowledge, the items inspected today are in conformance with approved plans and
specifications.
