Here is the requested documentation listed on the Model Home Permits check off list. I talked to Tim
Cheatum on September 7t' and he informed me that he will have his paperwork completed in the "next
couple of days." I left him a message this morning asking him for this paperwork showing the detention
was signed off and I have not heard back from him. I did inform him that I was submitting these
documents to you and that you would contact hire for what is needed of him.

Sara Slatten says that the plat of Shamrock L02P0014 has been approved for inspection of the hydrants for fire flow -- for
plat approval. However we are having a hard time finding documentation to back this up. We looked under B04FO455
which doesn't seem to specificly address the approval of fire hydrants. Can you please verify if we are good to go?

King County Department of Development and Environmental Services (DDES) has changed its
requirements and process for obtaining model home permits, The Model Home Permit Program
will be administered under the provisions of Section 106.5.1 of the Uniform Building Code. The
program will be administered by the Land Use Services Division Manager, subject to the following:
6 k- 1. Building permits for preregistered plans (Basics) may be requested by the plat developer
only, and the final plat application must have been submitted and screened.
2. Lots must be clear of special conditions, restrictions, easements, NGPEs, and BSBLs that
may be subject to line changes before recording or that would require special review as part
of permit issuance. (NOTE: This is the primary judgment call required in this process.)
3. Plat engineering plans must be approved.
4. Retention/detention facilities must be in place and/or Land Use Inspection Section must
have approved for recording. Temporary erosion control must be completed. Inspection
report will indicate status.
fc 5_ Roads must be constructed to final alignment and grade such that the inspector can
determine if connecting driveways are acceptable to King County standards.
ok 6. The Health Department must have approved the final plat. If served by sewers, a letter from
the sewer district may be substituted for consideration. (Nu ries of�c
7. Commercial Inspection Section must have approved the final plat for fire flow.
ff J< 8. Plat restoration bonds must have been received. (Contact the DDES Financial Guarantee
Management Unit at 206-296-6693 regarding bonds.)
ok 9. Street and lot addressing must be complete.
10. A U.B.C. 106.5.1 application fee of $328 per lot and a $142.50 per -plat fee must be
accompanied by a $5,000 per -lot cash bond or equivalent for restoration with the
application. A plot plan for each lot is also required.
The developer's request shall include the following:

2. Building plot plans (8'/2" x 11"), preregistered basic plan number, and builder (if not

6. The developer and builder, if not the same, sign said indemnification and an
acknowledgment that he/she/they signed as a free and voluntary act.

Dear Steve VanPatten,
Our understanding of the Model Home Permit Program is that, model home permits can be
obtained for 20% of the total lots within a development or 5 lots, whichever is greater. With
this in mind, we respectfully request that CamWest Development be allowed to pull model
home permits for the Plat of Shamrock Heights, a 118 lot development. More specifically, it
is our desire to pull model home permits for Lots 48, 51, 52, 94 and 95.
We understand the risk of model home construction lies with the recording of the fmal plat.
As such, we agree to indemnity and hold King County, its successors, and assigns harmless '
for any consequences resulting from starting construction prior to recoding of the subdivision,
including cost of defense, claimed by persons within or outside of this sub division.

King County Department of Development and Environmental Services (DDES) has changed its
requirements and process for obtaining model home permits. The Model Home Permit Program
will be administered under the provisions of Section 106.5.1 of the Uniform Building Code. The
program will be administered by the Land Use Services Division Manager, subject to the following:
Building permits for preregistered plans (Basics) may be requested by the plat developer
only, and the final plat application must have been submitted and screened.
2. Lots must be clear of special conditions, restrictions, easements, NGPEs, and BSBLs that
may be subject to line changes before recording or that would require special review as part
of permit issuance. (NOTE: This is the primary judgment call required in this process.)

• 4. etention/detention facilities must be in place and/or Land Use Inspection Section must
have approved for recording. Temporary erosion control must be completed. Inspection
report will indicate status.
• _IX Roads must be constructed to final alignment and grade such that the inspector can
determine if connecting driveways are acceptable to King County standards,
Ve---The Health Department must have approved the final plat. If served by sewers, a letter from
/' the sewer district may be substituted for consideration.

Plat restoration bonds must have been received, (Contact the DDES Financial Guarantee
Management Unit at 206-296-6693 regarding bonds.)
Street and lot addressing must be complete,
A U.B.C. 106.5.1 application fee of $328 per lot and a $142.50 per -plat fee must be
accompanied by a $5,000 per -lot cash bond or equivalent for restoration with the
application. A plot plan for each lot is also required.
The developer's request shall Include the following:
1. Lot numbers (with a 5 -lot maximum or 20% of total lots, whichever is less). LA i k l 5 2— l�
Building plot plans (8%" x 11"), preregistered basic plan number, and builder (if not
developer) for the lots.
3- U.B.C. 106.5.1 application fees: $328 per lot and one $142.50 per -plat fee.
Cash bond or equivalent for restoration in the amount of $5,000 per lot.
A statement from the developer that he/she indemnifies and holds King County, its
successors, and assigns harmless for any consequences resulting from starting construction
prior to recording of the subdivision, including cost of defense, claimed by persons within or
outside of this subdivision.
V6-�The developer and builder, if not the same, sign said indemnification and an
acknowledgment that helshelthey signed as a free and voluntary act.
