EE98285 CASE# 24-7529 OF COLLISION 07/19/24 01:00 OF CbLLI510N NARRATIVE On 07/19/2024 at approximately 0100 hours, I was working as a Police Officer for the City of Renton. While working I was dispatched to a collision at the intersection of S 2nd ST and Rainier Ave S, in the City of Renton, King County, Washington. I arrived on scene and found that it was a two-vehicle collision, both vehicles had sustained heavy damage and airbag deployment, so fire was sent to the scene to evaluate the drivers. At the time of the collision, the Crowd Strike outage was present affecting internet-based systems across the entire country. I did not have access to WA state DOL, I could not run anything through WACIC/NCIC. I could not pull a case number or create an arrest report as all programs were down. Unit 1 was identified as a white 2014 Mercedes E250 (WA LIC: BPP5634), driven by Daisy-Lynn A. Quitugua (DOB: 09/06/1984). Daisy's identity was later verified through her WA DOL photo when programs were restored. Upon running Daisy-Lynn through WA DOL I found that her license was suspended in the 3rd degree and that an ignition interlock device was required in her vehicle. Unit 2 was a black 2013 Toyota Corolla (WA LIC: BUH6305), the driver was identified as Matthew J. Campbell (DOB: 03/14/1998). Unit 1 had extensive damage to the front end of their vehicle along with airbag deployment, the vehicle was incapable of driving under its own power, so it was towed. When looking inside of the vehicle, I observed no ignition interlock device present. Unit 2 had extensive damage to the rear of the vehicle, the trunk had been heavily crumpled in as though it was hit at a high rate of speed. The front end of the vehicle also appeared to have minor damage. Inside of the vehicle, the driver's seat was twisted and contorted from the collision. Unit 2 was no longer able to drive under its own power, so it was towed. Officer S. Tamaivena took photos of the collision and uploaded them to Evidence.com. I was unable to interview Matthew as he was being placed into an ambulance and transported to the hospital for a suspected concussion. I later spoke with Matthew via phone on 07/20/2024, Matthew stated that he was in the middle-left turn lane stopped at a redlight on S 2nd ST preparing to turn southbound on Rainier Ave S. Matthew stated that he observed a set of headlights approaching from behind, he looked back up at the mirror and observed that the light was green. Before he was able to make his turn Matthew was struck by Daisy-Lynn and launched forward through the intersection. Matthew was pushed with so much force that his vehicle was completely pushed through the intersection and struck the concrete barrier located at the opposite side of the intersection. Matthew stated that he had went to the emergency room after the crash. They determined that he had a bruised collar bone. Matthew stated that he was extremely sore and was most likely going to start physical therapy. I spoke with Daisy-Lynn at the scene, I observed that Daisy-Lynn had watery, bloodshot eyes, slurred speech, and had an obvious scent of intoxicants coming from her person. I asked Daisy-Lynn for her driver's license, she stated that she had an I.D. Daisy-Lynn began looking around for her I.D and could not locate it. She then stated "I have um I.D, I just don't have any. I have an I.D." I asked Daisy- Lynn where it was at, she stated in the car, I looked in the vehicle and did not locate an I.D. I asked Daisy-Lynn if she had insurance, she stated that it was expired. I asked if she had anything to drink, she stated that she had nothing to drink. I notified Daisy-Lynn that her speech was slurred, her eyes were bloodshot and glossy, and that she smelled of intoxicants. I notified Daisy-Lynn that due to the circumstances I would not be able to complete a DUI investigation as all the resources that I needed to do so were not working. I notified Daisy-Lynn that she would be receiving criminal charges for the incident. I asked Daisy-Lynn how the collision happened, she stated that she was trying to turn southbound onto Rainier Ave S from S 2nd ST when she got hit by Matthew's vehicle. I notified Daisy-Lynn that due to the damage present on the vehicles there was no way that she could have been hit. Daisy stated the following "So if he hit me from behind, why would he be from behind when I'm in front." I asked Daisy-Lynn where she was before this and she stated that she was at J. P's barroom, a local bar located less than a mile away. I notified Daisy-Lynn that she would be receiving criminal traffic citations as well as traffic infractions PAGE 3 OF 5 REPORT NO. EE98285 CASE# 24-7529 OF COLLISION 07/19/24 01:00 OF CbLLI510N NARRATIVE in the mail. Daisy-Lynn was taken home by her boyfriend at the scene. Currently I find that the proximate cause of the collision is a combination of numerous things. I believe that Daisy-Lynn was intoxicated by alcohol which inhibited her driving capabilities. I believe that Daisy -Lynn was traveling too fast for conditions as the collision pushed Matthew's vehicle from one end of the intersection all the way to the opposite side of the intersection, the impact of the collision also bent the driver's seat inside of Matthew's vehicle. I believe that Daisy-Lynn was inattentive to her surroundings as she failed to see Matthew's vehicle stopped at the light and slow down or come to a stop. I have cited Daisy for the following Traffic Infractions under citation#4AO577102: Driving without Insurance (RCW 46.30.020). As Daisy-Lynn did not have valid insurance with her and stated that her insurance was expired. Inattentive Driving (RMC 10-12-25). Daisy-Lynn failed to pay attention to her surroundings, she failed to see Unit 2 stopped at the light and rear ended him. Failure to renew registration greater than two months (RCW 46.16A.030). Daisy-Lynn's Mercedes registration expired on 06/15/2023. 1 have developed probable cause for the following criminal traffic offenses: Negligent Driving in the 1 st degree (RCW 46.61.5249). This is based upon Daisy-Lynn's driving capabilities being inhibited due to her intoxication from alcohol. Daisy Lynn had bloodshot, glossy eyes, slurred speech, she had an obvious scent of intoxicants coming from her person, she could not make coherent sentences, and she stated that she had just come from a bar. Reckless Driving (RCW 46.61.500) The posted speed limit on S 2nd ST is 25mph. Based upon my training and experience I know what a collision at 25 mph looks like. This vehicle collision looked to be approximately 50mph. The impact of the collision was so great that it pushed Matthew's vehicle all the way through the intersection, it also bent the driver's seat of his vehicle. Driving While License Suspended 3rd degree (RCW 9A.20.021). This is based upon Daisy-Lynn driving with a license that is suspended in the 3rd degree according to her WA DOL profile. Driving without an Ignition Interlock Device (RCW 46.20.740). Daisy-Lynn's DOL return showed that she was supposed be driving with an ignition interlock device. While looking inside the vehicle I found no such device present. I have cited Daisy-Lynn for these offenses under citation#4AO577101 and forwarded the charges to the prosecutor's office for review.
