THIS AMENDMENT, dated for reference purposes only as July 5, 2024, is by and between the
City of Renton (the "City"), a Washington municipal corporation, and Bravura Counseling, PLLC
("Consultant"), a professional limited liability company. The City and the Consultant are referred
to collectively in this Amendment as the "Parties." Once fully executed by the Parties, this
Amendment is effective as of the last date signed by both parties.
Whereas, the City engaged the services of the Consultant under Agreement CAG-24-008, dated
January 22, 2024, to provide necessary services for the Wellness Check-In Program (referred to
herein as the "Agreement"); and
Whereas, the Parties wish to amend the Agreement to extend the time for performance in order
to utilize additional funds awarded through a grant and reflect an additional time of performance
between January 2024 through December 2025; and
Whereas, the Parties wish to amend the Agreement to modify the scope of services, to amend
total number of check-ins from 40 to 60, specifically 10 quarterly check-ins from June 2024 to
December 2024, and also 10 quarterly check-ins from January 2025 to December of 2025, for a
total of 20 check-ins remaining in 2024 and 40 check-ins for the 2025 year;
NOW THEREFORE, It is mutually agreed upon that CAG-24-008 is amended as follows:
1.Scope of Services: Section 1, Scope of Services, is amended to add an additional twenty
check-ins, taking the total number of check-ins from 40 to 60. Such additional check-ins
shall match the pace and quantity of the check-ins set forth in Exhibit A to CAG-24-008,
specifically: 10 quarterly check-ins from June 2024 to December 2024, and also 10
quarterly check-ins from January 2025 to December of 2025. The Parties agree that a total
of 20 check-ins remain in 2024 and there shall be up to 40 check-ins for the 2025 year;
2.Time of Performance: Section 3, Time of Performance, is amended to reflect that all Work
shall be completed no later than December 31, 2025.
3.Compensation: Section 4, Compensation, is amended so that the maximum amount of
compensation payable to Consultant is increased by $15,000 from $30,000, and shall now
not exceed $45,000, plus any applicable state and local sales taxes. The additional
compensation shall be paid based upon Work actually performed according to the rate(s)
or amounts specified in Exhibit A to CAG 24-008. The increase to compensation should
