SUBJECT:Assignment of TAG for SurveyMonkey
This memo is in lieu of our standard contract template which allows for signature by both
parties.
Attached for your review and signature is a subscription packet for SurveyMonkey (paid via
US Bank – OneCard). The purpose of this packet is to establish a TAG number to assist in
tracking the annual renewals of our Professional subscription. The current subscription has
been paid for and there is no cost associated with this initial packet. The current
subscription term is through June 2025.
SurveyMonkey is used by Finance and Executive Services to create and analyze surveys and
forms.
