DL State DLExp Address Primary Phone&Type AdditionalContact Number&Type Additional Contact Number&Type Race Sex DOB- Height Weight Even Hair Collision 2025-00001058 Paqe 2 OF 4 Case Number 2025-00001058 Officer Narrative Narrative 25-1058 On 02-02-2025 at approximately 1945 hours I arrived on scene at an unknown injury accident located at 118th Ave SE and SE Petrovitsky Rd in the City of Renton, County of King, State of Washington. Call details noted a vehicle had struck a sign and was in a ditch. There were unknown injuries and the driver was walking around. Upon my arrival I observed the involved vehicle, 2017 black Dodge Durango with WA plate#BLR6058, on the north side of the street in a ditch. The vehicle had struck a fence and appeared to be high centered on some large boulder rocks. I contacted the driver of the above listed vehicle, positively identified by his WADOL photo as Sergio Fragozo-Marroquin (DOB 03-27-2007). Sergio was standing outside the vehicle with the passenger, verbally identified as Ailyn E. Heredia (DOB 03-20-2008). Sergio advised he was the driver of the vehicle. He said he was traveling from west to east on SE Petrovitsky Rd. He was attempting to turn north bound onto 118th Ave SE. He was unable to make the turn and the vehicle proceeded into the ditch on the north side of the intersection. He said he was going approximately 5 mph. It should be noted that at the time of the collision, the road was extremely icy. The road was at a slight decline as well. Based on the road conditions as well as Sergio's accounts, I believed the collision was a result of the icy road conditions. Both Sergio and Ailyn advised they were not injured and both were wearing their seatbelts. I ran Sergio's name through my mobile data computer. DOL records showed Sergio to be not licensed-eligible. I cited Sergio for No Valid Operators License with Valid ID. This was by him operating a motor vehicle when DOL records showed him to not be licensed. I provided Sergio with a business card and a case number. They were able to remove the vehicle from the ditch. During this incident, I was equipped with a body-worn camera, which records both audio and video.
