Attached please find the notes for pre-construction meeting Thursday for the Hazen High School Modernization Early Clear & Grade project (C25000758) at 11am onsite (meet in north parking
lot near the portables project currently under construction). Please forward to any other interested parties. Below is an updated precon items list prior to notice to proceed on April
7, 2025.

1. Issuance of Civil Construction Permit – Ongoing. Plans and TIR routed for final signatures. I will send you a copy once they are signed. All items below must be complete prior to
full issuance and notice to proceed for the permit.
2. Payment of all fees associated with Civil Construction Permit: Paid.
3. Permit bond: Received.
4. Authorization of special billing: Received.
5. Business licenses for the contractor and subs: Resolved. Note, a list of subs and confirmation of business licenses will be required at the precon meeting.
6. Insurance certificates: Complete.
7. Proof of NPDES coverage: Email from DOE provided allowing for alternative to truck discharge water offsite. NPDES coverage will be required for any site stormwater discharge.
8. Any State/DNR approvals that may be needed prior to drilling of geothermal wells: Noted.
9. Pre-construction meeting: Preconstruction meeting to be held 4/3/25 at 11am onsite. One physical copy of the engineering plans will need to be provided to the City inspector at
the pre-construction meeting.

All – Civil doesn’t want to hold up the show for getting this precon set up, so I say we stick with this Thursday’s time slot. I will check with Alan and/or Evie in our office to see
if either of them can attend, but worst case, would someone be able to call me or face time me so I can listen in? Also, I am sure everyone knows, but the precon is typically at the
job site, so would need to determine the meeting location at the site? Baseball / Practice field?

1. Issuance of Civil Construction Permit – Ongoing. Plans and TIR routed for final signatures. I will send you a copy once they are signed. All items below must be complete prior to
full issuance and notice to proceed for the permit.
2. Payment of all fees associated with Civil Construction Permit: Paid.
3. Permit bond: Received.
4. Authorization of special billing: Received.
5. Business licenses for the contractor and subs: Resolved. Note, a list of subs and confirmation of business licenses will be required at the precon meeting.
6. Insurance certificates: In process and is in review with the Risk Management department.
7. Proof of NPDES coverage: I will be meeting tomorrow with Alan to discuss. The permit has been conditioned to provide either the NPDES coverage or written confirmation from DOE that
other measures are acceptable. As discussed with Mike Sippo today 3/31/25, until the NPDES permit is obtained, Construction Runoff will be captured in on-site tanks and trucked off-site
to an approved disposal location. No discharge to the surface water storm system will occur until the NPDES permit has been approved. We understand that this will also be a permit
condition of this Early Grading Permit.
8. Any State/DNR approvals that may be needed prior to drilling of geothermal wells: Noted.
9. Pre-construction meeting: A preconstruction meeting will need to be held prior to the contractor mobilizing. Cannot schedule until the NPDES requirements are resolved. We will
need about a week notice to schedule. One physical copy of the engineering plans will need to be provided to the City inspector at the pre-construction meeting.

5. Business licenses for the contractor and subs: Noted below that the contractor is Forma.
6. Insurance certificates: In process and I will run through our Risk Management department. Note, insurance certificate expires July 2025 so will need to be updated.
7. Proof of NPDES coverage: Please provide.

9. Pre-construction meeting: A preconstruction meeting will need to be held prior to the contractor mobilizing. Wednesday April 2nd is the preferred date if construction is beginning

the pre-construction meeting.
