EF81589 CASE# 25-2759 F LNa O 03/27/25 13:36 �coy�isI©�ON u NARRATIVE ASSIGNMENT/ARRIVAL On 03/27/2025 at approximately 1339 hours, I was dispatched to a two vehicle collision in the intersection of SW 27th Street and Oakesdale Avenue SW in the city of Renton, King County, Washington. I arrived at approximately 1346 hours. UNIT 1: HYUNDAI SONATA I located a 2024 Hyundai Sonata (WA/ CPM9560, Unit 1) in the parking lot of 1100 SW 27th Street. Unit 1 had been moved from its point of rest prior to my arrival. Unit 1 had moderate damage to the passenger front and rear doors. The passenger side airbags were deployed. UNIT 2: VOLKSWAGEN JETTA I located a silver 2017 Volkswagen Jetta (WA/ BVK8029, Unit 2) in the intersection of SW 27th Street and Oakesdale Avenue SW in the northbound lanes of Oakesdale Avenue SW facing southeast. Unit 2 had moderate front end damage from the collision and a combination of airbags were deployed. DRIVER 1: WILEY, COLEY I contacted Coley Wiley (DOB: 11/27/1993) near Unit 1. 1 identified Coley by his Washington Driver License, which was Suspended/Revoked in the 3rd Degree effective 01/25/2025. 1 established Wiley as the driver of Unit 1 by his admission. Wiley was not injured from the collision. DRIVER 2: HEALY, MADISON K. I contacted Madison K. Healy (DOB: 07/19/2001) near Unit 2. 1 identified Healy by her valid Washington Driver License. I established Healy as the driver of Unit 2 by her admission. Healy complained of pain to her left wrist/forearm from the collision. Healy was evaluated and released on scene by the Renton Fire Authority. WITNESS: BAKER, CASEY M. I located a witness, Casey M. Baker (DOB: 01/19/1982), at the scene. The following is a summary of our conversation: Baker drove north on Oakesdale Avenue SW behind Unit 2. As they approached the intersection of SW 27th Street on a solid green, circular traffic signal, Unit 1 drove east into the intersection from SW 27th Street. Unit 2's front end collided with Unit 1's passenger side. Unit 1 drove to a nearby parking lot and stopped. This concluded my conversation with Baker. STATEMENT: WILEY, COLEY I spoke with Wiley about the collision. The following is a summary of our conversation: Wiley drove east on SW 27th Street toward Oakesdale Avenue SW. As Wiley approached the intersection his water bottle rolled under the brake pedal. Wiley drove into the intersection, unable to stop, and collided with Unit 2. This concluded my conversation with Wiley. STATEMENT: HEALY, MADISON K. I spoke with Healy about the collision. The following is a summary of our conversation: Healy drove Unit 2 north on Oakesdale Avenue SW toward SW 27th Street on a solid green, circular traffic signal. PROXIMATE CAUSE Wiley operated Unit 2 in violation of RCW 46.61.050, Obedience to and required traffic control devices, by entering the intersection of SW 27th Street and Oakesdale Avenue SW on a steady red indication. CITATIONS I authored Wiley a citation for RCW 46.61.050 (Citation #5A0189405) and Driving while License Suspended 3rd Degree (Citation #5A0189406).
