DLState DLExp Address Primary Phone&Type AdditionalContact Number&Type Additional Contact Number&Type Race Sex DOB- Height Weight Even Hair Collision 2025-00003883 Paqe 2 OF 5 Case Number 2025-00003883 Officer Narrative Narrative ASSIGNMENT/ARRIVAL On 05/02/2025 at approximately 1320 hours, I was dispatched to a two vehicle collision in the 200 block of S 2nd Street in the city of Renton, King County,Washington. Dispatch advised both vehicles pulled off the roadway and into the Safeway parking lot of 200 S 3rd Street for contact. UNIT 1: BMW X7 Upon arrival I observed a white BMW X7(WA/CBA0779) in the parking lot of Safeway. Unit 1 had minor damage(dents and gray paint transfer)to the trailing edge of the front driver door, and the leading edge of the rear driver door. UNIT 2: HYUNDAI SONATA I observed a gray Hyundai Sonata (WA/CMA1881) in the parking lot of Safeway parked next to Unit 1. Unit 2 had minor damage(cracks and dents)to the front passenger bumper. DRIVER 1: POLZIN,CHRISTINA M. I contacted Christina M. Polzin (DOB: 12/09/1978) next to Unit 1. 1 identified Polzin by her valid Washington Driver License and established her as the driver of Unit 1 by her admission. Polzin was not injured from the collision. The following is a summary of our conversation: Polzin drove Unit 1 west on S 2nd Street in the number two lane from Shattuck Avenue S toward the Safeway parking lot at 200 S 3rd Street. Polzin intended to turn left(south) into the private driveway of Safeway. Polzin merged left from the number two lane into the number three lane. Polzin didn't see Unit 2 traveling next to Unit 1 in the number three lane.As Polzin merged, Unit 1's driver side collided with Unit 2's front This incident occurred in the sCity of Renton,King County,Washington State. !declare underpenalty of perjury under the laws of the State of Washington that the foregoing is true and correct. This concluded my conversation with Polzin. DRIV_E__R 2 BASTIDAS UZCATEGUILUIS D. 1 contacted Luis D. Bastidas Uzcategui (DOB: 12/20/1995) next to Unit 2. 1 identified Bastidas Uzcategui by his valid Washington Driver License and established him as the driver of Unit 2 by his admission. Bastidas Uzcategui spoke only Spanish, so I used a Language Line Spanish interpreter. Bastidas Uzcategui complained of chest pain.The Renton Regional Fire Authority responded and evaluated him at the scene. Bastidas Uzcategui's statement about the collision was similar to Polzin's. Bastidas Uzcategui did not have valid insurance for Unit 2. PROXIMATE CAUSE The proximate cause of the collision was determined by damage to both vehicles and driver statements. Polzin caused the collision by violating RCW 46.61.140(1),which states: Whenever any roadway has been divided into two or more clearly marked lanes for traffic the following rules in addition to all others consistent herewith shall apply: (1) A vehicle shall be driven as nearly as practicable entirely within a single lane and shall not be moved from such lane until the driver has first ascertained that such movement can be made with safety. Polzin moved Unit 1 from lane two into lane three without first ascertaining that the movement can be made safely. DISPOSITION 1 photographed both vehicles. Both drivers exchanged information. This concluded my involvement in this case.
