DLState DLExp Address Primary Phone&Type AdditionalContact Number&Type Additional Contact Number&Type Race Sex DOB- Height Weight Even Hair Collision 2025-00003891 Page 2 OF 5 Case Number 2025-00003891 Officer Narrative Narrative ASSIGNMENT/ARRIVAL On 05/02/2025 at approximately 1537 hours, I was dispatched to a two vehicle collision in the 4000 block of Maple Valley Highway in the city of Renton, King County,Washington. I arrived at approximately 1546 hours. UNIT 1: FORD F-150 Upon arrival I observed a gray Ford F-150(WA/D98247F) stopped in the westbound number one lane of Maple Valley Highway west of 131 st Avenue SE. Unit 1 had damage to the driver-side taillamp assembly, the driver side of the rear bumper, and the tail gate. UNIT 2: NISSAN ARMADA I observed a silver Nissan Armada (AK/JMK810)stopped in the westbound number one lane of Maple Valley Highway west of 131 st Avenue SE just behind Unit 1. Unit 2 had moderate front end damage to the front bumper,front grille, and the hood,which was buckled upward.The radiator was compromised, as seen by coolant on the roadway surface. ROADWAY EVIDENCE I saw a liquid on the roadway surface in the westbound number lane of Maple Valley Highway about 10 feet west of the west curbline prolongation of 131 st Avenue SE.The trait of liquid lead to Unit 2's front end at its point of rest, leading me to believe it was radiator fluid. I saw plastic debris in the roadway just west of where the liquid trail started consistent with pieces of Unit 1's driver taillamp assembly. DRIVER 1: HEALY,THOMAS E. This incident occurred in the sCity of Renton,King County,Washington State. !declare underpenalty of perjury under the laws of the State of Washington that the foregoing is true and correct. Healy(DOB: 12/30/1961) near Unit 2. 1 identified Healy by his valid Washington Driver License and established him as the driver of Unit 1 by his admission. Healywas not injured from the collision. Healy stated he was Leaving the Maplewood Golf Course located at 4040 Maple Valley Highway. Healy stopped at the exit of the private drive of the golf course to turn right(west) onto Maple Valley Highway(at the intersection of 131 st Avenue SE). Healy admitted westbound traffic had a green traffic signal at 131 st Avenue SE. Healy did not see any approaching vehicles that would constitute a hazard, so he turned west onto Maple Valley Highway. Just as Healy entered the roadway he saw Unit 2 approaching in the number one lane of westbound Maple Valley Highway. Healy attempted to rapidly accelerate to avoid a collision but was unable to. Unit 2's front end collided with Unit 1's rear end. Healy admitted he shouldn't have entered the roadway and alluded to the fact he didn't notice Unit 2 driving toward him as he turned. This concluded my conversation with Healy. DRIVER 2: CASTILLO BONILLA, DORA LILIA I contacted Dora Lilia Castillo Bonilla (DOB: 12/05/1971) near Unit 2. 1 identified Dora by her valid Washington Driver License and established her as the driver of Unit 2 by her admission. Dora was not injured from the collision. Dora's statement was consistent with Healy's. PASSENGERS:UNIT 2 Tomas J.Valencia Martinez(DOB: 03/03/1968)was Unit 2's front right passenger. GabrielT.Valencia-Ring(DOB: 05/21/2022)was Unit 2's rear right passenger. Alvaro Valencia-Castillo(DOB: 04/17/1992)was Unit 2's third-row driver passenger. No passengers were injured. PROXIMATE CAUSE The proximate cause of the collision was determined by debris in the roadway, damage to both vehicles, and statements from both drivers. Healy's actions were the proximate cause of the collision, by failing to yield right-of-way to all vehicles Lawfully approaching on a highway when entering said highway from a private driveway(RCW 46.61.205). DISPOSITION_. I photographed both vehicles.All parties exchanged information. This concluded my involvement in this case.
