Federal Aid No: N/A TIB No: N/A f 'M' Date: Construction Manager: Tani Stafford The following is documentation of the oral order given by the Resident Engineer per WSDOT Std Spec 1-04.4 The Contractor is directed to furnish and install the following in the control cabinet assembly:a 12-port wall mounted fiberoptic termination panel with loaded duplex single-mode SC coupler plates and splice tray.(Bid Item A85)The panel shall be a Corning SPH-01P with(1)CCH-CP12-59 coupler plate.This is in lieu of the LC coupler plate called for in S.P.9-29.13(10)A. Rough Cost Estimate,WSDOT Construction Manual 1-04.4,Changes. Cost Adjustments:Increase in cost is TBD. A Change Order covering this Field Directive will follow immediately. FWD 2 was sent out on 1/12/22 and a quote was requested from the Contractor on that date. The Contractor's quote for no cost was received on 2/18/22. The no cost change is accepted. Determination of-,Jditional Working Days A determination of zero additional working day was made. The change is a substitution of materials rather than added work. There was a time when Totem proposed all new vehicle signal heads. However it was later determined that the existing ones are to remain. So the actual electrical quote we are looking for is the electrical component change described best in FWD No2, attached. Let me know if that helps. The panel shall be a Corning SPH-01P with (1)CCH-CP12-59 coupler plate. This is in lieu of the LC Coupler Plate,CCH-CP49-A9,called for in the Special Provision 9-29.13(10)A(Bid Item A85). By this change order, the fiberoptic termination panel shall meet the requirements of Special Provision 9-29.13(10)A Auxiliary Equipment for NEMA Controllers noted above. Reason for Change: The Special Provisions did not include a more recent City of Renton special provision for the fiberoptic termination panel. Materials: FO Termination Panel Change Plans: No changes to the Plans. Adjustment: This Change Order includes changes as defined in 1-04.4. Specifically,the altering of specifications (e.g. FO Termination Panel). There is no cost associated with this change.
