Narrative
ASSIGNMENT/ARRIVAL
On 05/09/2025 at approximately 1607 hours, I was dispatched to a two vehicle collision near the
intersection of Hardie Avenue SW and Renton Avenue Extension in the city of Renton, King County,
Washington.
I arrived at approximately 1607 hours.
UNIT 1: HONDA RIDGELINE
Upon arrival I observed a black Honda Ridgeline(Unit 1,WA/C61599W) parked on Hayes Place SW
south of Renton Avenue Extension. Unit 1 had been moved from its point of rest prior to my arrival.
Unit 1 had minor damage to the front driver-side bumper.
U NIT 2:TOYOTA RAV4
I observed a silver Toyota Rav4(Unit 2,WA/AYT5026) parked on Hayes Place SW south of Renton
Avenue Extension behind Unit 1. Unit 2 had been moved from its point of rest prior to my arrival.
Unit 2 had minor damage to the rear passenger-side bumper.
DRIVER 1: LY TRUNG T
I contacted TrungT. Ly(DOB:08/11/1989)standing near Unit 2. 1 identified Ly by his valid Washington
Driver License and established him as the driver of Unit 1 by his admission.
Ly was not injured from the collision.
DRIVER 2: DAO, NHI T.
I contacted Nhi T. Dao (DOB: 04/11/1993)standing near Unit 2. 1 identified Dao by her valid Washington
This incident occurred in the sCity of Renton,King County,Washington State.

Driver License and established her as the driver of Unit 2 by her admission.
Dao was not injured from the collision.
STAT_E__MENT_DAO. NHI T.
I spoke with Dao about the collision.The following is a summary of our conversation:
Dao drove Unit 2 north on Hardie Avenue SW toward Renton Avenue Extension on a green traffic signal
with the intention to turn left(west)onto Renton Avenue S. Unit 2's left turn signal was activated.
The traffic signal turned yellow as Dao approached the intersection, so she stopped just before the
intersection to not enter the intersection on a red traffic signal.
As Dao stopped Unit 2, Unit 1's front end collided with Unit 2's rear end in a "rear end" collision.
Ly heard our conversation and agreed with Dao's version of events.
This concluded my conversation with Ly.
PROXIMATE CAUSE
The proximate cause of the collision was determined by damage to both vehicles and Dao's statement.
The cause of the collision was Ly operating Unit 1 in violation of RCW 46.61.400(1), Basic rule and
maximum limits, by driving Unit 1 at a speed greater than is reasonable and prudent under the conditions
and having regard for the actual hazard that existed.
CITATION
I issued Ly a citation for RCW 46.61.400(1), Citation#5A0189409, and filed it with the Renton Municipal
Court.
