NARRATIVE
25-4015
This incident was captured on my body worn video camera. This report is a summary of events that
occurred and is not an exact sequencing of events. Statements have been paraphrased and
summarized.
On 05/06/2025 at approximately 1826 hours I was dispatched to a non-injury blocking accident
located at the intersection of 117th Ave SE and SE Petrovitsky Rd, City of Renton, County of King,
Washington.
Upon my arrival, I confirmed there were no complaints of injury requiring immediate medical
response at the time of report. There, I was able to collect each involved party's information and
independent summary of the events leading up to the collision.
I spoke with Marcus A. Phillips (03/23/1985) who was the driver of Unit#1. Unit#1 was a light blue
2007 Lexus IS. The vehicle did not have any license plates or trip permit on it. Phillips provided me
with a bill of sale and a title for the vehicle which he stated he purchased on 05/05/2025. 1 ran the
vehicle through WACIC/NCIC, and it returned clear. Phillips provided me with a Washington ID only
as he is not licensed to drive.
I asked Phillips how the collision happened. Phillips stated he was operating Unit#1 traveling
westbound on SE Petrovitsky Rd. He stated he was traveling in lane 2 of 2 and began to merge into
the southbound turning lane to turn southbound onto 116th Ave SE. Phillips stated due to the sun
being in his eyes he could not see when he was merging which caused him to collide with the rear
passenger side of Unit#2.
Unit#2 was a grey 2010 Toyota Prius bearing WA Temporary plate A8273418. The driver of Unit#2,
Mukhammadkodir Madaminov (03/24/2004), stated the following:
Unit#2 was traveling westbound on SE Petrovitsky Rd in lane 2 of 2. Unit#2 entered the southbound
turning lane to turn southbound on 116th Ave SE. Madaminov stated he did not see the collision
happen but felt his vehicle get struck on the rear passenger side.
observed the damage to both vehicles. Unit#1 had minimal damage to the driver's side front
bumper and door. Unit#2 had minimal damage to the passenger side rear bumper. I photographed
the damage to both vehicles and uploaded the photographs to AXON.
I ran Phillips through WACIC/NCIC and learned he had two warrants out of Grays Harbor County. 1
asked Phillips to step out of the vehicle and placed him in handcuffs. 1 informed him he was being
detained for his warrants. Phillips began to complain that he felt dizzy and asked if he could sit down.
I escorted him to the front of my patrol vehicle to allow him to sit on the front of my patrol vehicle.
Phillips informed me he had just been discharged from the hospital this morning at approximately
1130 hours. He stated he his currently on dialysis 3 times a week. Dispatch informed me Grays
Harbor Sheriff's Department would not extradite Phillips on his warrants. 1 began to take him out of
handcuffs, he started to faint and began to collapse to the ground. I eased him to the ground and
requested Renton Regional Fire Authority come evaluate Phillips.
RRFA arrived on scene and took Phillips vitals. They determined he should go to the hospital and
receive care. Phillips was transported via RRFA to Valley Medical Center.
Unit#1 was driving was able to be moved off the roadway and Unit#2 was able to be driven away
from the scene.
With the above statements from both parties I was able to determine the driver of Unit#1 (Phillips)
was the proximate cause of the collision due to inattentive driving. I cited Phillips for Registration and
Display of Plates Required (RCW 46.16A.030.2), Driving Without a License (RCW 46.20.015), and
Operating a Motor Vehicle Without Insurance (RCW 46.30.020) (Sector Cite #5A0440015).
Both vehicles were able to be moved from the roadway without incident.
