EG02680 CASE# 25-5082 OF COLLI ION 06/10/25 17:53 OF COLLISION NARRATIVE 25-5082 This incident was captured on my body worn video camera. This report is a summary of events that occurred and is not an exact sequencing of events. Statements have been paraphrased and summarized. On 06-10-2025 1 was working as a police officer in a fully marked department issued uniform and patrolling in a marked police vehicle for the city of Renton. At approximately 1758 hours I was dispatched to a non-blocking/non-injury motor vehicle collision at the intersection of Maple Ave SW/SW Grady Way, City of Renton, County of King, State of Washington. I gathered from the call notes, a two-motor vehicle collision occurred that resulted in no injuries and was not blocking traffic. The involved parties would await contact near the Buick Dealership, and the damage was over $1,000. En route to the call location, the two vehicles involved was a white 2016 Toyota Camry (license plate#BQZ2067/WA) and a gray 2011 Toyota Corolla (license plate#CGE4856/WA). I arrived on scene and observed obvious front-end damage to the Corolla, and obvious passenger side damage to the Camry. The Corolla had silver/white scuff marks on the front right bumper that was also chipped. The Corolla had an indent on the front and rear passenger side doors. There were also gray/silver scuff marks left on the front and rear passenger side doors. The driver of the Camry was later positively identified as Hong T. Dinh (DOB 03-10-1994) via WADL, and the driver of the Corolla was later positively identified as Michael R. Pacheco (DOB 10-15-1990) via WADL. Michael and Hong both advised that they had no injuries and did not need medical attention. Later both parties advised the actual collision occurred at the intersection of Lind Ave SW/SW Grady Way. I spoke with Michael, and he advised the following occurred: Michael drove northbound on Lind Ave SW towards the intersection of Lind Ave SW/SW Grady Way in the number two lane. Michael had a solid green to continue northbound on Lind Ave SW. As Michael drove into the intersection, he observed the driver of the Camry drove in front of him towards SW Grady Way. Michael attempted to avoid a collision by honking his horn, pushed on his brake, and turned his wheel to the right. Unfortunately, Michael collided with the Camry in the middle of the intersection, and later pulled over with the driver at Maple Ave SW/SW Grady Way. I spoke with Hong, and she advised the following occurred: Hong faced south in the left turn lane to turn east onto SW Grady Way from Lind Ave SW. Hong had a "flashing" yellow left turn arrow to yield to oncoming traffic. Hong said she observed the vehicles in front of her in the same left turn lane turn left at the same time continuously. Hong decided to follow them and not wait to check for oncoming traffic before she turned. As she entered the intersection she observed the Corolla, and it was too late to avoid the collision and was struck by the Corolla. The proximate cause for the collision, is that Hong failed to obey the traffic control device and failed to yield to oncoming traffic. As a result, Hong was struck by Michael's Corolla when he had the right of way, and she had to yield. Both drivers provided valid WADL's, registration paperwork, proof of insurance, and phone numbers. I provided both drivers a copy of the exchange of information sheet with their information. I issued Hong Sector citation# 5A0551298, and advised Hong that she would receive the citation in the mail. I took photos of the vehicles and uploaded them to evidence.com.
