Description of Test Procedure, Conditions, Results.
CMGRT-100 measures resistance by applying a voltage at 1.689KHZ- And indirectly measuring
resulting current then divides the voltage by the current to derive the resistance, I used a test
stance loop prior to ground grid test to assure unit was functioning correctly.

10/9/2008 Welds are visually acceptable.
The remaining 6 anchor bolt chairs have been installed and those welds are as per approved plan
(drawing S08 - sheet 20 of 34), and are visuaiiy acceptable.
Floor plate lap joint fillet wells are complete and visually acceptable. The code required vacuum leak
tests were completed on the remaining sections, and we now state that the entire floor plate has
been leak tested and found to be acceptable.
Tank structural welding is complete and has been applied in accordance with approved project
specifications and documents.

Bob Schaefer Provided continuous monitoring of the CJP (complete joint penetration) shell plate vertical seams
6/25/2008 (8 places) on shell course #11.
The plate thickness was verified as being 7/ E;" thick per drawing S01.
The vertical seams are being welded with the semi-auto/manual FCAW Process.
Joint fit -up is within code tolerance.
The fair -up and tracking of the horizontal seam which connects shell courses #10 and #11, which
occurs concurrently with the welding of vertical seams. This joint is ready for welding.

Bob Schaefer Visual examination of completed vertical and horizontal welds at shell course #11 is acceptable.

Northwest Inspection was on site in the morning to perform code mandated RT (radiographic testing)
inspection of the CJP (complete joint penetration) welds on course #11. All indications were
removed — RT okay. (See attached Radiographic Examination report for results).
The paint contractor mobilizes to site and begins sandblast operations on roof assembly which is still
staged in the lay down area.
The cable tray attachment fillet welds at the western sector of the tank are complete and visually
acceptable to course #9.
Erection of course #12 begins at 2:30 pm.
Shell thickness was verified prior to erection.

The painters, City of Renton, and Lincoln Electric representatives were on site for a pre -construction
meeting and show and tell.
The fillet welds on the spiral stairs and tile stair landing at course #8 are complete and visually
acceptable.
The cable tray assembly was being erected and welded today.
After everyone visiting the site left, the CJP welding resumed. Visual testing and RT (radiographic
testing) is scheduled for Monday, 6/30/2008.

door after 4t" course installation) was welded and the entire weld was leak tested again. Welds are

After successful tests on the above reported fillet welds, the anchor bolt chairs began being fit and
tacked. This work is in progress and proceeding normally.

Bob Schaefer Arrived at site on our client's behalf to monitor tank steel erection activities. At the end of the day, the
4/17/2008 floor plates are installed and tacked in place. The annular ring complete joint penetration (CJP)
welds have been radiographed, and all 14 splice welds are acceptable, and the 1.2" first course of
the tank shell has been erected and tacked/dogged into position for welding.
Upon review of the project specifications, an apparent discrepancy has arisen. Paragraph 5.12 of
Division 5 "Metals" states that all welders shall be WABO certified. However, the governing code for
the project, AWWA D100-05, mandates that all welders be qualified IAW ASME BPVC 1X. T. Bailey,
Inc., being a tank erection shop, has qualified the welders to the ASME requirements, while WABO
uses AWS D1.1 as the qualification standard. This was reported verbally to RH2 Engineering, Karla
Secrest. Request Engineer's approval of the ASME certifications. At this time, the three ASME
certified welders at the site are: Chris Morse, Jim Hoyt, and Cesar Ajeto.

Performed continuous monitoring of the horizontal circumferential seam welding which joins the 1s
4/30/2008
and 2"d shell plate courses. Welding of the inside portion completed at the end of day. One welder
assigned to do weld "pick-up", as crew begins to set up the saw process welding machine on the
outside of the seam.
One welder begins FCAW semi -auto fillet welds on the anchor bolt chairs, per detail shown on
drawing S08, sheet #20. This work is in progress.

Jeff Rabe 1. The writer attended an on site meeting regarding the pending mass concrete placement for
3/12/2008 the mat foundation.
2. There will be two pumps, placing a total of about 1600 cubic yards of concrete on 3/15/2008.
Sampling frequency is four 6'x12" cylinders every 50 cubic yards. The sampling frequency
might be reduced to every 100 cubic yards, depending on the consistency of the delivered
product. Air and slump tests will be required at the time the samples are made. In addition,
every truck will be tested for air until the air content is consistent. The engineer of record will
be on site to help determine when reductions in air testing and sample frequency might be
appropriate.
3. The first concrete is scheduled to hit the ground at 9 a.m., and the placement will finish at
about 8 p.m. ORA staff should plan on arriving by 8 a.m. that morning.
4. The writer briefed ORA dispatch on these details and suggested manpower requirements of
a one placement inspector at the end of each hose and three ACI technicians per pump to
sample and test the concrete. The intent is to have enough testing personnel as not to hold
up placement. This will be fairly labor intensive due to the projected rate and volume of the
placement. A second, relief shift of inspectors should be considered as fatigue may become
an issue. Lab support staff have also been briefed and intend to begin pre -positioning cure
boxes and supplies.
5. Writer notes that the contractors are in process of expanding the pump and truck delivery
areas, however the site may still be congested and the lay down area might consist of an un-
even 2" to 4" diameter rock spalls. Sheets of plywood and dunnage may be required to
provide a flat stable area to perform testing and level the cure boxes.
6. A final reinforcing steel inspection is scheduled for Friday, 3/14/2007, in the aftemoon.
7. Stoneway has volunteered to provide a maturity meter to monitor the core and surface curing
temperatures.

to the approved project plans and specifications for size, spacing, lap, length and clearance.
4. Ironworkers were in progress of setting the standees in preparation for installing the top mat.

appropriate times during the progression of the work.

Tuesday, March 4, and would like to schedule a resteel inspection at that time. The writer advised the
contractors to please schedule all inspection through our dispatch at the main office number and noted
that our dispatch was requesting 48 hours notice at this time.
3. The engineer said there were no special ductility requirements above ASTM D-615 such as are
sometimes seen in the construction of high rise buildings. The writer examined some of the rebar that
had already been delivered to the site and noted it was grade 60 from Seattle's NuCor plant.
4. The soil bearing capacity and sub grade preparation has been inspected and approved by others.
5. There will be several layers of reinforcement in the mat foundation. Contractors will schedule
inspections, as appropriate, with our office. A 1600 cubic yard concrete placement is scheduled for
Saturday, March 15. Sampling will be required every 50 cubic yards. A pre -placement meeting will
occur prior to the placement.
6. Entry to the site is off a small gravel access road immediately to the north of Hazen High School (off

Roof assembly operations resume in staging area.
At the horizontal seam, a lamination type indication was revealed while examining the film for the
vertical weld. This occurred at course #9, weld #3 (C9-V3) at the northern sector of the tank.
Indication is at the west side of C9-V3, view 0-1. The area was excavated by air carbon arc, for a
distance of approximately 3'-0", to determine its length. Also, further west from the RT indication,
several sections, 2" long, were excavated to ensure the indication does not extend further. Weld was
re -shot after re -weld. RTOK.

Bob Schaefer Shell plate course #14 is welded complete and the vertical (8) and horizontal UP (complete joint
7/11/2008 penetration) welds are visually acceptable.
Erection of final shell plate course #15 begins in AM. After acceptance of each weld joint, welding
begins. Welds for course #15 incomplete by end of shift.
Northwest Inspection is tentatively scheduled for next Tuesday, 7/15/2008 — AM.
Thicknesses of rolled plate for shell course verified as being %4' thick, per 501, sheet 15 of 34.
Spiral access stairs laid out between courses 8 & 11 and tacked in place. Welding incomplete.
Stainless waster sleeve at interior inlet pipe, floor level, welded inside and out prior to setting inlet
piping to the welded flange. This also occurred at the outlet check valves stainless steel flanged
sleeve. Both welded with S309 & S308 SMAW as required.
Welds visually acceptable.

Bob Schaefer With the exception of the of the area adjacent to the temporary access door at the southeast
8/4/2008 quadrant of the tank (approximate 10'-0" radius), the tank bottom fillet welds were tested in
accordance with AWWA D100-05 section 11.10 "testing", paragraph 11.10.1.2. This air
pressure/vacuum testing was conducted with vacuum pressures of at least 2 psi (pounds per square
inch). The remainder of the floor will be tested at a future date when the access door is reinstalled,
welded after blast, and paint operations.
Fillet welds inside and outside of tank on roof to tank attachment joints are per project plan and are
visually acceptable.
Handrail/guardrail assembly at tank edge - roof, is installed; complete and welds are visually
acceptable.
Anchor bolt chairs at access door remain incomplete, also.
Conforms with the exception of the chairs at access doors.

Bob Schaefer Shell welds for radiographic testing is now substantially complete. The T. Bailey, Inc, crew sets up
7/15/2008 staging on the outside of the tank at course #15 in order to fit -up the roof assembly. See attached
Radiographic Examination Report for results.
The 2" x 2" X'/," rim angle fit; in place is the 19' activity (reference: 1/S03) to take place prior to
setting the completed dome roof assembly atop the shell.
