In order to prepare the contract documents, please provide the following documents to me as soon as possible. 1. A list of names, addresses, phone numbers, and emergency phone numbers for: Responsible Officer, Job Foreman, and Bonding Agent. 2. Corporation Letter or Resolution giving names of those authorized to sign the contract and samples of their signatures. 3. A photocopy of your current State Contractor's License (not Master License). 4. A photocopy of your current City of Renton Business License or a copy of the receipt showing that one has been obtained. 5. Two (2) signed contract bonds on City of Renton forms, UNDATED. The date will be filled in when signed by the Mayor. 6. Two (2) signed contracts also UNDATED. The blank forms of the contract are enclosed. Two Certificate of Insurance for 2005, naming the City of Renton as additional insured. Language on this certificate must require 45 days written notice to the City prior to cancellation and not include language relieving the insurance company, its agents or representatives of obligations or liability to meet the 45-day requirement. If the correct language is not available, the inappropriate wording may be lined out and acknowledged by the insurance company. See enclosed sample. 8.
