EG12394 CASE# 25-6167 OF COLLISION 07/17/25 16:40 OF CbLLI510N NARRATIVE 25-6167 ACCINJ On 7/17/2025 1 was working as a uniformed patrol officer and driving a marked patrol vehicle for the City of Renton. I was dispatched to a report of a collision with injuries in the 19100 block of 108th Ave SE Renton/King/WA. Numerous callers were reporting a two-vehicle collision with several injured parties. I arrived on scene and located both vehicles blocking the sorthbound and middle turn lanes. RRFA personnel were on scene and evaluating injuries. I spoke with the driver of a 2013 Cadillac XTS WA/BTN2295 (Unit 1), identified by her WADL as Tianne M Israel DOB: 9/13/1979. Tianne said that she was driving northbound on 108th Ave SE in the #2 lane, she said that she crossed through the intersection of SE 192nd Ave and 108th Ave SE. She said that a red truck turned left from the center turn lane into the gas station in front of her and she veered to avoid it. She said that was when she struck Vehicle 2 which was behind the red truck also in the center turn lane. The Cadillac had damage to the front bumper and front passenger quarter panel area. There was front airbag deployment as well. Tianne's two passengers, Jantae I White DOB: 10/25/2005 (left rear), and Durae W Israel DOB: 10/10/2016 (right rear) were not injured. Tianne had a possible broken left arm and was evaluated by RRFA at the scene, she was transported to VMC for further evaluation. I spoke with the driver of the 2022 Toyota Rav4 WA/CDJ2594, identified by his WADL as Hilario P Valdez DOB: 9/21/1958. Hilario spoke minimal English, what he said was that he was in the center turn lane to turn east onto SE 192nd Ave, he said that traffic had just started moving when the Cadillac veered out of her lane and struck him. Hilario said that he believed the Cadillac was speeding. I asked him about a red truck, and he just shook his head. Hilario had a complaint of pain and was transported to VMC for further evaluation. His front passenger, identified by her WADL as Corazon B Valdez DOB: 3/13/1957 had complaint of head, neck, and back pain and was also transported to VMC for evaluation. The Toyota had heavy damage to the front bumper area and had a combination of airbags deployed. Gene Meyers Towing removed both vehicles from the scene. I called witness Bailey R Chalker DOB: 4/11/2003 and asked if she witnessed the collision, she said yes. She explained that she was driving north on 108th Ave SE and the light at SE 192nd had just turned green. She said that the "older Asian couple" pulled out of the gas station onto 108th Ave SE and struck the "Chevy" (referring to the Cadillac) that was also travelling northbound. I confirmed with Bailey several times that the "older Asian couple" was leaving the gas station, and she said yes. Based on the damage to both vehicles I do not believe this is possible. I called several other witnesses and did not receive any answer. I went to VMC and gave all paperwork including exchange of information forms to both Tianne and Hilario. I took photographs of the vehicles and uploaded them to Axon Evidence. A DOL check that the Cadillac had an expired registration as of 5/17/2024, 1 checked the tab on the Cadillac rear plate, and it was black in color and showed 2024. 1 completed Sector Citation #5A0563871 for Failure to Renew Expired Registration Greater than 2 Months RCW 46.16A.030.5.0.
