ASSIGNMENT/ARRIVAL
On 08/05/2025 at approximately 1110 hours, I was dispatched to a two-vehicle collision in the
carwash at Brown Bear Car Wash, 621 Rainier Avenue S in the city of Renton, King County,
Washington.
I arrived at approximately 1121 hours.
UNIT 1:TOYOTA GRAND HIGHLANDER
Upon arrival I observed a blue 2024 Toyota Grand Highlander(WA/CMB7240, Unit 1) parked in
the parking lot of Brown Bear Car Wash near the vacuums. Unit 1 had been moved from its point
of rest prior to my arrival. Unit 1 was not damaged in the collision.
UNIT 2: KIA NERO
I observed a white Kia Nero (WA/BXW2684, Unit 2) parked in the parking lot south of Unit 1. Unit
2 had been moved from its point of rest prior to my arrival. Unit 2 was not damaged in the
collision.
DRIVER 1: SIDIQI MUMTAZ W.
I contacted Mumtaz W. Sidiqi (DOB: 03/22/1992) near Unit 1. 1 identified Sidiqi by his valid
Washington Driver License. I established Sidiqi as the driver of Unit 1 by his admission. Sidiqi
was not injured in the collision.

This incident occurred in the sCity of Renton,King County,Washington State.

I contacted Thavishap J.Vorachit (DOB: 1/17/1981) sitting in the driver seat of Unit 2. 1 identified
Vorachit by his valid Washington Driver License. I established Vorachit as the driver of Unit 2 by
his admission. Vorachit said his back"felt weird"from the collision.
STATEMENT:VORACHIT THAVISHAP J.
I spoke with Vorachit about the incident. The following is a summary of our conversation:
Vorachit drove Unit 2 into the car wash via the entrance.The car was automated with a track
that pulled vehicles through the wash. Attendants instructed drivers to place their vehicles in
neutral gear throughout the wash.
Just as Unit 2 entered the wash, Unit 1, which was behind Unit 2 in line, rear-ended Unit 2 at a
very slow speed.
Vorachit confronted Unit 1's driver, Sidiqi,who denied a collision.
This concluded my conversation with Vorachit.

I spoke with Sidiqi about the collision. Sidiqi denied any collision between Unit 1 and Unit 2.
SURVEILLANCE FOOTAGE
A Brown Bear Car Wash employee allowed me to watch surveillance footage on the incident. I
recorded it with my Axon body worn camera.
The footage showed as Vorachit described,with Unit 2 rapidly accelerating as it entered the

about 3 miles per hour.
