Narrative
ASSIGNMENT/ARRIVAL
On 08/08/2025 at approximately 1555 hours, I was dispatched to a two-vehicle collision in the
400 block of Union Avenue NE in the city of Renton, King County,Washington.
I arrived at approximately 1625 hours.
UNIT 1: HONDA CIVIC
Upon arrival I observed a silver Honda Civic (WA/AJJ8515, Unit 1) stopped in the parking lot of
Chase Bank, 4110 NE 4th Street. Unit 1 had been moved from its point of rest prior to my arrival.
Unit 1 sustained minor damage to the front bumper.
UNIT 2:TOYOTA CAM FLY
I observed a black Toyota Camry(WA/ BUC2442, Unit 2) stopped in the parking lot of Chase
Bank, 4110 NE 4th Street. Unit 2 had been moved from its point of rest prior to my arrival. Unit 2
sustained minor damage to the front-passenger bumper.
DRIVER 1:JOHNSON ANTONE C.
I contacted Antone C. Johnson (DOB: 04/11/1962) near Unit 1. 1 identified Johnson by his valid
Washington Driver License. I established Johnson as the driver of Unit 1 by his admission.
Johnson was not injured in the collision.
DRIVER 2:TSEDENDAMBA, ODSUREN
I contacted Odsuren Tsedendamba (DOB: 09/26/1966) near Unit 2. 1 identified Tsedendamba by
her valid Washington Driver License. I established Tsedendamba as the driver of Unit 2 by her

STATEMENT:JOHNSON ANTONE C.
I spoke with Johnson about the collision.The following is a summary of our conversation:
Johnson drove Unit 1 west through the parking lot of Chase Bank toward a private driveway to
turn left (south) onto Union Avenue NE.Johnson waited while two vehicles drove north on Union
Avenue NE then turned right(east) into the private driveway of Chase Bank.
Johnson did not see any other vehicles approaching that would constitute a hazard, so he
entered Union Avenue NE to make his turn.Johnson failed to see Unit 2, which traveled north on
Union Avenue NE in the number two lane toward him. Unit 1's front bumper collided with Unit
2's front-passenger bumper.
Tsedendamba concurred with Johnson's version of events.
This concluded my conversation with Johnson.
PROXIMATE CAUSE
The proximate cause of the collision was determined by damage observed to both vehicles and
Johnson's statement.
Johnson's actions, namely his violation of RCW 46.61.205(1), which states:
The driver of a vehicle about to enter or cross a highway from a private road or driveway
shall yield the right-of-way to all vehicles lawfully approaching on said highway.
