NARRATIVE
Unless otherwise noted, the following occurred within the city limits of Renton, county of King, state of
Washington.
On 08/10/2025, 1 was working as a uniformed police officer for the city of Renton. At approximately
1809 hours, I was dispatched to the intersection of 140th Way SE and Maple Valley Hwy regarding
an unknown if injury accident. Portions of my involvement in the incident were captured on my
department-issued bodycam. Any statements referred to have been paraphrased and summarized.
While enroute, dispatch relayed that multiple reporting parties were calling about the collision, with at
least two vehicles involved. One of those vehicles was described as a blue Hyundai loniq 5.
When I arrived on scene, I located the Hyundai blocking lane 2 of northbound Maple Valley Highway,
immediately south of the intersection at 140th Way SE. I identified the vehicle as WA lic/CKS2077
(VIN/ KM8KNDAF5PU213853). I got behind the vehicle with my emergency lights activated and
exited. There were multiple vehicles pulled over along the shoulder of the highway, and multiple
people were standing immediately outside of the Hyundai.
approached the group and quickly learned that this was a two-vehicle hit-and-run collision, with the
other driver fleeing southbound on Maple Valley Highway. The sole occupant and driver of the
Hyundai was identified as registered owner Santhoshi Jagadeeshan (11/16/1983 -WA DOL photo).
re-directed the other parties outside the vehicle to the shoulder, where they could speak with Officer
Turner while I gathered information from Santhoshi.
She reported to me that she was feeling some unspecified pain after the collision. Her synopsis of
what occurred, which has been paraphrased, is outlined below:
She was in lane 2 on northbound Maple Valley Hwy, stopped at the intersection at 140th Ave SE. The
traffic signal was red, so she was waiting for it to turn green before proceeding. Suddenly, she noticed
unit 1 traveling northbound on 140th Way SE at a high rate of speed. Unit 1 began to make a right-
hand turn; however, it failed to do so and struck her vehicle head on. Her vehicle was stopped during
the entirety of the incident. The whole incident happened so fast that she had no time to react or try
and get out of the way.
Renton Regional Fire Authority was called to the scene. There, they evaluated Santhoshi, who
ultimately ended up leaving the scene in a Tri-Med ambulance, which took her to Valley Medical
Center (400 S 43rd St).
Officer Turner ended up leaving the scene and rerouting to another collision near the intersection at
150th Ln SE and Maple Valley Highway. It turns out that one of the vehicles involved in that collision
was unit 1 from this collision. The two separate collisions occurred approximately 3/ of a mile apart.
All this to say, I ended up speaking with the multiple witnesses who remained on scene at 140th Way
SE. I first talked to Brittney K.W. Willey (08/08/1990), who advised of the following:
She only heard the collision, as she lives in an apartment complex next to the intersection. The noise
she heard was so jarring that it prompted her to head out to the main road on foot to check out what
happened. When she arrived, she reported seeing two vehicles involved in a head-on collision. The
two drivers involved appeared to exchange words with one another, with the driver of unit 1 possibly
yelling at unit 2. Not long after, the driver of unit 1 backed up and continued southbound on Maple
Valley Highway.
I next spoke with Jovanna A Galvizo (05/11/2000 -WA DOL photo). She reported the following:
The driver of unit 1 was a black male with "frizzy out" hair, of an average build, standing
approximately 6 feet tall, possibly a little shorter. He was supposedly wearing a white/gray sweatshirt
top that was not seated properly on his body, as well as sweatpants. The top was apparently folded
over up to his neck, exposing his stomach. She believed that when he got out of his vehicle, she
could hear him apologizing for the collision. He appeared to be a little disoriented or "out of it." In
relation to the victim's vehicle, she described being positioned next to it.
Though I regrettably did not ask her directly, I believe it is reasonable she would recognize the driver
if she saw him again, due to the extensive description she provided.
Notably, Jovanna had dashcam footage of the collision as it happened. I received the footage, which
is 0:00:58 long, and uploaded it to the case file via evidence.com. I later reviewed the footage and

observed the following:
Jovanna's vehicle (WA lic/BKG0346) is positioned behind unit 2, but in lane 1. If unit 2 were an
analog clock, with the front of it representing 12 o' clock," then her vehicle was at about the "5 o'
clock" position. Other vehicles from 140th Way SE can be seen merging onto Maple Valley Highway.
One of those vehicles ends up being unit 1, which comes on screen from the left at about 0:00:05. It
takes only about four seconds for unit 1 to then turn and strike unit 2. Seconds after the collision,
Jovanna begins merging her vehicle off to the shoulder. A human can briefly be seen getting out of
unit 1, but the quality prevents any further description from being ascertained.
Prior to speaking with one final witness, I received their contact information from Officer Watson.
Along with this witness' info, I was given a phone number for a Mohamud Osman (unk. dob). I
learned that Mohamud reportedly also had video of the collision; however, he had left the scene, and
did not return to provide supposed said footage. His phone number is (425) 598-3446. 1 did not
attempt to contact him.
Using a law enforcement database, I later queried the provided phone number to try and positively
identify Mohamud. I was able to locate a possible match, Mohamud M Osman (06/07/2000) from a
contact with King County Sheriff's Office in December of 2023. There was a WA DOL number
associated with the return. I ran that through ACCESS and was able to view a DOL image of
Mohamud. I can say with great confidence that I saw this individual at the scene of the collision. At no
point was I able to speak with him prior to his departure; however, this may be useful for future follow-
up work on this case.
The final witness remaining at the scene was Gabriel Martinez (01/05/1957). He advised of the
following:
He was in his vehicle (WA lic/MAHHA5) in lane 1, directly alongside unit 2. He described unit 1
"hauling" down the hill on 140th Way SE, specifically noting an audible screeching noise coming from
the vehicle's tires as he was attempting to turn. He believed unit 1 was going too fast. Once the
collision occurred, he saw the male driver of unit 1 get out and begin to "stumble around"; though he
specifically noted he wasn't sure if that behavior was directly correlated to the impact of the collision,
or something else. The only description of the driver he could remember was that he was a black
male with a beard. He stated that it was possible he could recognize him if he saw him again.
captured numerous photos of unit 2 and uploaded them to evidence.com.
Prior to requesting a tow for unit 2, 1 verified the vehicle would not be needed for further investigation
with Sergeant Thielman and Traffic Detective Lane. Once given the go ahead, I requested a tow and
completed an impound form for it, since Santhoshi was no longer on scene. Gene Meyer Towing
arrived on scene a short time later and towed the vehicle.
