NARRATIVE
On 07/26/2025, 1 was working uniformed patrol in a marked police car as 3R23. At approximately
2010 hours, officers were dispatched to an unknown injury collision at the intersection of NE 4th
St/Queen Ave NE, which is located within the city limits of Renton, County of King, Washington.
Dispatch advised that multiple callers had reported a 3-car collision at the intersection.
When I arrived at the call location, I was able to identify all involved units, and their respective
occupants as listed above. Other officers assisted me by speaking to other involved units.
I was able to speak to the driver of Unit 1. 1 was able to identify him and his 3 other occupants inside
his vehicle at this time. My conversation with the driver of Unit 1 is summarized as follows:
Unit 1 stated that he was headed eastbound on NE 4th St, approaching Queen Ave NE. Unit 1 stated
he got into the middle turn lane to prepare to head northbound onto Queen Ave NE. Unit 1 stated that
he began his turn to continue straight onto Queen Ave NE and saw that Unit 2 was in the westbound
lanes of NE 4th St approaching Queen Ave NE. Unit 1 stated that the front end of Unit 2 had collided
with the rear passenger and fender area of his vehicle, as Unit 1 had not fully completed the turn.
This resulted in Unit 1 spinning briefly before the front end of Unit 1 subsequently struck the front end
of Unit 3. Unit 3 was stopped just north of the intersection, facing southbound.
I was able to take a look around Unit 1. 1 saw that multiple airbags inside the vehicle had deployed. I
also saw that there were significant dents and scratches on the entire rear end of Unit 1. 1 also saw
that the front end of Unit 1 appeared to have come to rest on the front bumper of Unit 3 with no
obvious scratches or dents.
Unit 1's driver stated he was uninjured and was wearing his seatbelt. I also spoke to the other
passengers inside Unit 1:
Unit 1's front passenger stated he had knee pain and was wearing his seatbelt.
Unit 1's middle row driver's side passenger stated she had shoulder pain and did not recall if her
seatbelt was on.
Unit 1's middle row passenger side passenger stated he had some side pain and did not recall if his
seatbelt was on.
I was then able to meet with other officers who had spoken to the drivers of Unit 2 and Unit 3. Both
vehicles were only occupied by a single occupant, with Unit 2 having a dash camera.
Information relayed on Unit 2's recollection of the incident is summarized as follows:
Unit 2 was headed westbound on NE 4th St, approaching Queen Ave NE. Unit 2 stated the front end
of his vehicle had then struck the rear passenger side of Unit 1 as Unit 1 had begun to turn from the
center turn lane onto Queen Ave NE, facing the opposite way as Unit 2.
1 was able to view Unit 2's dash camera. The video depicted the following:
It should be noted that the time and date stamp on the dash camera appeared to be inaccurate.
Unit 2 can be seen in lane #2 (leftmost lane) of westbound NE 4th St traveling normally. As Unit 2
approaches Queen Ave NE, Unit 1 is visible facing eastbound in the center turn lane from eastbound
NE 4th St. You can also see Unit 3 facing southbound on Queen Ave NE just north of the intersection.
Unit 1 begins to turn in front of Unit 2 with what appears to be about 1-2 car lengths of space between
the two vehicles. It appears Unit 2 tries to swerve left to avoid striking Unit 1, but inevitably strikes the
rear passenger side of Unit 1. You can observe Unit 1 briefly begin to spin out in the direction of
where Unit 3 is, but the camera does not capture where the final stopping point of Unit 1 is.
I was able to take a look around Unit 2. 1 saw that his front airbags had deployed. I also saw that there
were significant dents and scratches on the entire front end of Unit 2.
Unit 2's driver stated he was uninjured and was wearing his seatbelt.
I was then able to gather the information relayed to officers about Unit 3's recollection of the incident.
His recollection is summarized as follows:
Unit 3 stated he was headed southbound on Queen Ave NE approaching NE 4th St. Just before Unit
3 entered the intersection, he stated Unit 1 abruptly collided with the front end of his vehicle as a
result of being struck by Unit 2.
1 was able to take a look around Unit 3. 1 did see that the front end of Unit 1 had impacted the front
side of Unit 3. 1 did not see any airbags deploy inside Unit 3's vehicle.

NARRATIVE
Unit 3's driver stated he felt some injury to his knee, elbow, and neck area. He also stated he was
wearing his seatbelt.
The fire department arrived and evaluated all parties at the location. The occupants inside Unit 1 and
Unit 2 all eventually left the location under their own power. The driver of Unit 3 was transported to
the hospital for further evaluation of his injuries.
Bankers tow also eventually arrived and took possession of Unit 1 and Unit 2. Unit 3 was parked at a
nearby parking lot by the driver of Unit 3 before his departure to the hospital.
All associated videos were uploaded to Evidence.com to be added to the case file.
I provided an information exchange to all parties at the location before their departure.
Based on the totality of the circumstances described above, I believe that Unit 1 is the proximate
cause of the collision.
