Upon arriving on scene, 1 saw two vehicles with heavy damage. Unit 1 was a white 2014 Honda Accord bearing the WA license plate of (CPY6201) with heavy front passenger damage which was seen in the Shell Gas Station parking lot, over 200 feet away from the scene of the accident. In the intersection of Oaksdale Ave SW and SW Grady Way was Unit 2 which was a green 2002 Toyota Tacoma bearing the WA license plate of (C55485E) with significant front end damage. The damage ranged from the driver door and around the front of the vehicle. The damage was so significant it was difficult to tell which side of the vehicle had the original impact from the collision. Renton Fire Department was called on scene to check on all involved occupants due to multiple airbag deployments in both vehicles. Officer Swan spoke to the driver of Unit 2 which was the Toyota Tacoma. The driver who identified himself as PUENTES, ARMANDO (DOB 07-19-1970) stated he was traveling northbound on Oaksdale Ave SW. Other witnesses on scene were unsure of the direction of travel for Puentes however stated they were certain the second involved, white 2014 Honda Accord (CPY6201) was traveling at an incredibly high rate of speed eastbound on SW Grady Way through a stale red light. Statements were collected from the witnesses who all again stated that the reckless manner in which the driver of the white 2014 Honda Accord was operating the vehicle, was the cause of the collision. None of the witnesses could place either of the male subjects as the driver. Please see Officer Deighan's, Officer Engels and Officer Swans reports for further witness details and statements. PUENTES, ARMANDO (DOB 07-19-1970) provided me with a Medical Release Form and a statement stating he was traveling northbound on Oaksdale Ave SW, through a stale green light. Due to his condition post collision, Renton Fire Department suggested he go to the hospital. His family who was on scene, stated they would drive him as they believe he is not acting normal. I proceeded to speak to the occupants of Unit 1. The driver of the vehicle identified himself as MORALES VARGAS, FREDIS (DOB 05-06-2004). The front right passenger was the registered owner of the vehicle and identified himself as BAUTISTA HERNANDEZ, YAHIR ALEXANDER (DOB 02-23-2006) both verified via WA DOL. Bautista's injuries were consistent with being in the passenger seat. His injuries consisted of cuts to his face and right elbows. The passenger's right-side window was shattered, and the front of the windshield was cracked. Both had blood on the surrounding airbags. Driver Morales stated he was unharmed and was visibly not hurt which was supported by the visual lack of vehicle damage to the driver side, except steering wheel airbag deployment. Renton Fire Department checked both individuals. Bautista was advised to go to the hospital. Speaking to both driver Morales and passenger Bautista, they were unwilling to clearly state their direction of travel. They first stated they were unsure, both looking at each other to provide me with an answer. Morales spoke up stating they were traveling westbound on SW Grady Way through intersection of Powell Ave SW and SW Grady Way which is where the collision occurred. After the impact, the other vehicle fled the scene and they pulled over into the shell. Speaking to both Morales and Bautista, they stated they were approaching the intersection, unit 2 drove out into the intersection faster than they anticipated, as they were attempting to drive through a yellow light. They misjudged the speed of the light and vehicle speeds of unit 2. It should be noted during my discussion with Morales and Bautista the intersection of Powell Ave SW was to east of our location. The active collision with Renton Fire Department responding was to the west of our location. I pointed out the involved truck, at the intersection of Oaksdale and Grady, to which they stated they believed the male attempted to drive off however were again, unsure. Due to the inconsistencies in the story both subjects provided me with statements, signed under the PAGE 3 OF 6 REPORT NO.` EG22051 CASE# 25-7027 O COLLI COLLISION TIME OF 08/12/25 22:04 COLLI NARRATIVE penalty of perjury, regarding their positioning in the vehicle and direction of travel. It should be noted that when providing me with a statement, Bautista was read the penalty of perjury statement and became uncomfortable not answering yes to the statement. It was after he was read the sentence "Is the information given in this statement true to the best of your knowledge", Bautista emphasized that only to the best of his knowledge and then stated yes to the perjury statement. Both continued claiming their westbound direction of travel despite me showing them the second involved vehicle at the intersection of the eastbound travel lane of Oaksdale and Grady. Both subjects also completed Medical Release information paperwork. Both involved vehicles were photographed and as they were inoperable, towed at the owner's expense by Banker's towing. It should be noted that there is a traffic camera on the intersection of Oaksdale Ave SW and SW Grady Way. At a later time, I reviewed the footage and observed the following. 08/12/2025 - 22:02:58 eastbound traveling light on SW Grady Way, turns red. 08/12/2025 - 22:03:03 visible lights from Unit 2 approach from the northbound traveling lane, approaching intersection and driving out after a green light. 08/12/2025 - 22:03:04 Unit 1 collides into Unit 2 at an increasingly high rate of speed. This video has been uploaded to the case. This video, which includes time stamps, proves that Unit 2 was traveling through the intersection due to having the right of way. Unit 1 was the proximate cause of the collision due to recklessly driving through an intersection with a stale red light, as shown in the video. As MORALES VARGAS, FREDIS (DOB 05-06-2004) admitted to driving and then provided me with a written statement admitting so, he will be charged with RCW 46.61.500 Reckless driving. Due to the collected statements, observations from the collision and collected and reviewed camera footage, Morales was operating the vehicle in willful or wanton disregard for the safety of persons or property as statements from witnesses claimed Unit 1 which Morales was operating, was passing vehicles at a heightened levels of speed and running a stale red light which was confirmed by the camera footage, which ultimately caused severe damage to Unit 2 and victim PUENTES, ARMANDO (DOB 07-19- 1970) causing him to go to the hospital due to the extent of his injuries. Neither of the two parties, registered owner HERNANDEZ, YAHIR ALEXANDER (DOB 02-23-2006) nor driver MORALES VARGAS, FREDIS (DOB 05-06-2004) were able to provide me with insurance information. Due to this MORALES VARGAS, FREDIS (DOB 05-06-2004) will also be charged with RCW (46.30.020) operating a motor vehicle without proof of insurance, as both parties were given multiple opportunities to provide insurance for the vehicle and were unable to. As Morales provided me a statement stating he was driving, he will be cited for both of the above. This concludes my involvement with this case. My involvement in this case was captured on my department-issued Axon body camera. This report is a summary of events that occurred and is not an exact sequencing of events.
