DLState DLExp Address Primary Phone&Type AdditionalContact Number&Type Additional Contact Number&Type Race Sex DOB- Height Weight Even Hair Collision 2025-00007431 Paqe 2 OF 5 Case Number 2025-00007431 Officer Narrative Narrative ASSIGNMENT/ARRIVAL On 08/25/2025 at approximately 1435 hours, I was dispatched to a two-vehicle collision in the 2300 block of Maple Valley Highway in the city of Renton, King County, Washington. I arrived at approximately 1441 hours. UNIT 1:TOYOTA PRIUS Upon arrival I located a red Toyota Prius (WA/CTT3099, Unit 1) stopped in the parking lot of the Riviera Apartments, 2201 SE Maple Valley Highway, Renton. Unit 1 had been moved from its point of rest prior to my arrival. Unit 1 had minor damage (scrapes)to the rear-passenger bumper. UNIT 2: ISUZU NPR(BOX TRUCK) I located a blue Isuzu NPR box truck(WA/C75051 Y, Unit 2) stopped in the parking lot of the Riviera Apartments parked near Unit 1. Unit 2 had been moved from its point of rest prior to my arrival. Unit 2's front bumper was ripped off in the collision. DRIVER 1: KEYS, NICOLE M. I contacted Nicole M. Keys (DOB: 01/10/2003) near Unit 1. 1 identified Keys by her valid Washington Driver License. I established Keys as the driver of Unit 1 by her admission. Keys was not injured in the collision. Keys said the following about the collision: Keys drove Unit 1 east on Maple Valley Highway in the number 2 (closest to center) lane. Keys saw an ambulance driving west in the two-way turn lane with the emergency lights and siren This incident occurred in the sCity of Renton,King County,Washington State. !declare underpenalty of perjury under the laws of the State of Washington that the foregoing is true and correct. Keys intended to yield to the right. Keys signaled her turn with Unit 1's turn indicators, checked her mirrors,visually checked her blind spot,then merged into the number 1 lane. Unit 1's rear-passenger bumped collided with Unit 2's front bumper. Keys didn't see Unit 2 prior to the collision. Keys admitted she was a new driver and did not have insurance on the vehicle. DRIVER 2:WASMUND AUSTIN P. I contacted Austin P.Wasmund (DOB: 08/10/1996) near Unit 2. 1 identified Wasmund by his valid Washinton Driver License. I established Wasmund as the driver of Unit 2 by his admission. Wasmund was not injured in the collision. PASSENGER Brian E. Skeen (DOB: 12/11/1988)was a front passenger in Unit 2. Skeen was not injured in the collision. PROXIMATE CAUSE The proximate cause of the collision was determined by damage observed to both vehicles and Keys' statement. Keys' actions were the proximate cause of the collision, namely her violation of RCW 46.61.140(1), which states: Whenever any roadway has been divided into two or more clearly marked lanes for traffic the following rules in addition to all others consistent herewith shall apply: This incident occurred in the sCity of Renton,King County,Washington State. !declare underpenalty of perjury under the laws of the State of Washington that the foregoing is true and correct. CITATIONS I authored Keys a citation for the violation of RCW 46.61.140(1) and a violation of RCW 46.30.020(1)(a), Insurance Required, Citation #5A0249334.
