Narrative
ASSIGNMENT/ARRIVAL
On 09/05/2025 at approximately 1251 hours, I was dispatched to a two-vehicle collision in the
100 block of Renton Avenue Extension in the city of Renton, King County,Washington.
I arrived at approximately 1259 hours.
UNIT 1: FORD F-150
Upon arrival I observed a gray Ford F-150 (WA/ D48851 F) stopped facing west in the two-way
turn lane of the 100 block of Renton Avenue Extension. Unit 1 had minor damage (scrapes)to
the front-passenger bumper. Unit 1 had been moved from its point of rest prior to my arrival.
UNIT 2:TESLA MODEL 3
1 observed a gray Testa Model 3 (WA/CFH5660) stopped facing west in the number three
(closest to center)westbound lane in the 100 block of Renton Avenue Extension. Unit 2 had
minor damage (scrapes and dents)to the driver-rear door and fender. Unit 2 had not been
moved from it spoint of rest prior to my arrival.
DRIVER 1:TSERKOVNYY, ROMAN G.
I contacted Roman G.Tserkovnyy(DOB: 12/31/1994) near Unit 1. 1 identified him by his valid
Washington Driver License and established him as the driver of Unit 1 by his admission.
Tserkovnyy was not injured in the collision.
DRIVER 2: POURESFANDIARY CHAM 1AMSHID
This incident occurred in the sCity of Renton,King County,Washington State.

I contacted Jamshid Pouresfandiary Cham (DOB: 04/19/1970) in the driver seat of Unit 2. 1
identified him by his valid Washington Driver License and established him as the driver of Unit 2
by his admission. Jamshid was not injured in the collision.
STATEMENT: POURESFANDIARY CHAM JAMSHID
I spoke with Jamshid about the incident.The following is a summary of our conversation;
Jamshid drove Unit 2 west on Airport Way in the westbound number two lane and stopped at the
intersection of Rainier Avenue S for a red light. Once the light turned green,Jamshid accelerated
Unit 2 through the intersection.
Unit 1's front-passenger bumper collided with Unit 2's rear-driver side just as they got through
the intersection.
There was dispute and disagreement as to who was in which lane, and who merged into who. I
was unable to determine positioning prior to, and movement during the collision.
Jamshid's Tesla was equipped with cameras but he didn't know how to operate them. I sent
Jamshid an Axon Community Request link for footage if he could download it from the Tesla.
This concluded my conversation with Jamshid.
CITATION
Tserkovnyy admitted he did not have insurance on his Ford F-150. 1 authored Tserkovnyy a
citation for Operating a Motor Vehicle without Insurance (Citation #5A0688585a)
