completed by the responsible professional engineer for the project and
returned to this department.

CONCRETE PIPE CONT.
The unit price per linmar foot Shall constitute complete COWnsstion for all labor,
material, fittings, tools, and a ipmnt necessary for pipe installation; including
excavation, placing of pipe, and mechanically taping the beckfill material. No
further coapenmattoo will be made.

The method used for testing the sanitary sower pipe for acceptance shall be Low
pressure Air Test. Refer to table in this document. If additional information is
required, see Section 62-7 of the Standard Specifications.
TESTING OF 1NTERIa'.S
The Contractor Shall provide test samples and results from a testing 1atlOratpry
approved by the Engineer of materials used on this project is follows:

INSTRUCTIONS-
A. Upon completion, and prior to the use of any project or portions thereof, a
professional engineer shall complete and sign this form certifying that the
project was constructed in accordance with the plans and specifications, and
major change orders, approved by the Department of Ecology.
B. If a pro'ect is being completed in phased construction, a map shall be attached
showing Lhat portion of the project being certified on the date given below.
Each phase of a project must be certified as it is completed. Addltional
certification forms are available from the Department of Ecology offices listed
below.
