In consideraion of the premium charged, it is hereby agreed and understood that Policy
Number issued by Insurance Company, is
amended to include the following terms and conditions as respects Contract Number
issued by the(OWNER).

claims arising from: (a)activities performed by or on behalf of the NAMED INSURED:or(b)

or used by the NAMED INSURED.
r 2. CONTRIBUTION NOT REQUIRED. As respects: (a)work performed by the NAMED

� insurance afforded by this policy shall be primary insurance as respects the OWNER,or any

3. SEVERABRLIIY OF INTEREST. The inclusion of more than one Insured under this
policy shall not affect the rights of any Insured as respects any claim,suit or judgment made
or brought by or for any other Insured or by or for a.- ,employee of any other Insured. This
policy shall protect each Insured in the same manner as though a separate policy had been
issued to each, except that nothing herein shall operate to increase the company's liability
beyond the amount or amounts for which the company would have been liable had only
one insured been named.
4 4. CANCELLATION NOTICE. The insurance afforded by this policy shall not be

SUBJECT: Dayton Ave.Loral Improvement District(BID)
As you ate aware,City Council has approved the proposed sewer LID for Dayton Ave. NE(NE
20th to N. 22nd). The Wastewater Utility has begun design work. This project is on the list
for a Public Works Trust Fund Loan. We will, therefore, be working on a schedule that is
subject to revision based on the State's time table. We arc currently projecting bidding the
, ., project L^te April or Carly May and beginning construction by the end of May.
It is our understanding that you plan to replace the water main in this area. Because the
installation of these sewers was a citizen requetst and involves a Public Works Trust Fund Loan
and an LID,we would like to ensure that tha sewer work s compieted in a timely manner. To
meet �w goal of a timely sewer project, we would Like to suggest these options for the
installation^f the new water mains.
1. We could include the watermain replacement as a separate schedule to our project. Our
contractor would coas[ruct both. Our sewer project will be responsible for the overlay
of the street. To include your work in our project, we would need plans and
1 specification materials by March 28th.
2. The Water Utility, through agreement with the Maintenance Division, could replace the
watermain prior to our anticipated beginning of construction. H Maintenance were to
install the watermain prior to our construction, we would expect full restoration of
surface improvetncnts and a permanent hot patch of the roadway prior to the start of
construction of our sewer project. Our sewer project will be responsible for the overlay
of the street at the completion of our project.
3. The Water Utility, through agreement with the Maintenance Division, could replace the
watermain after the sewer lime construction is completed. If the sever main was installed
first, the Wastewater Utility would make full restoration of surface improvements and
restore the nadway with a final patch. In order to close the IJD and the State loan in a
timely manner, the Wastewater Utility would final the project at that stage. The Water
Utility would,therefore,be responsible for the cost and placement of the final overlay.

As we discussed pm.lousily, the W.ute4atcr Utility is working on an LID for Wytor
Avennc (NE 20th to NE 22nd). You mentioned that the Shops crews could replace the
water main on this street if we could provide you with the design and materials
uttor n ition.
The Water Utility is working on the design of th,water main and will have the design to
you shortly. We. must have this main replaced and the surface restored (hot patch)
before the Sewer contractor begins worn. The tentative date for the start of the sewer
construction is the end of May
We will keep yod posted on the ptogmm of this job.

Subject: LID 337 Dayton Ave NE Sewer Improvements
project Description:
The property owners along Dayton Ave. NE, between NE 20th St and NE 22nd St.
have requested, through the formation of a local improvement dis Pict, the
installation of sanitary sewers to provide service to l,teii existing residential
neighborhood. The work involves the installation of approximately 870 lineal feet
of 8" sewer main with manhole structures and new sidesewer stubs in Dayton P.ve ,
NE (NE 20th to NE 22nd) and in NE 22nd St. (Dayton to Edmonds). This project
will include complete restoration of all affected landscaping, surface improvements,
and the roadway. The sewer main will be approximatly 8 feet deep at the deepest
points.
