THIS APPROVAL IS TO BE AV.'.ILABLE ON THE JOB SITE AT ALL TIMES AND ITS PROVISIONS FOLLOWED BY THE PERMIT-
TEE AND OPERATOR PERFORMING THE WORK.
The persons) to whom this approval is issued may be held liable for any loss or damage 10 fish life or habitat which resulm from
failure to comply with the prows ans of this approval.
Failure to comply with the provisions of this approval is a gross misdemeanor, possibly punishable by fine tndror imprisonment.

mm[slrabve Procedure Act, chapter 34.0t RCW, to make additional restriciiins or conditions or revoke the approval when new informs
non shows such action is necessary by the department for the protection of fish life.
This department cannot be held liable for any property damage which might occur as a result of this project except where damages
are proximately cauaei by actions of the department.
This approval pertains only to the provisions of the Fisheries and Game Codes. Additional euthortration from other public aoencies
may be necessary for this project.
1. Bridge deck replacement may occur at any time.
2. Prior to the bridge deck being removed a temporary fence of heavy wire mesh shall 5e
constructed across the creek channel downstream of t)-- bridge. This will retain
bridge debris.
3. Tho bridge deck shall be removed witt, a minimum of material being dropped into the
creek.
4. No additional stream bank/abutment work shall be condLcted without consulting with the
Washington Department of Fisheries representative, Kurt Buchanan, 545-6582.
5. Sewer line installation shall be by the open-cut method.
6. Sewer line installation shall be done according to plans "Maplewood Golf Course - Golf
Course Sewer Extension,[' dated April 1986.
7. Prior to any other instream work, the stream shall be placed in a temporary )ypass
culvert.
8. This culvert shall be large enough to pass all anticipated flow and debris.

PROVISIONS:
o. The culvert shall outfall far enough downstream to prevent backwatering the work
site.
1C. Between the work site any the culvert outfall , a low rise check dam shall be built of
pea gravel-filled burlap bags.
11. Any sediment accumulated by this check dam shall be ;era„ved from the stream bed.
12. The sewer line trench shall be backfilled with rock, the surface one (1) fout shall
be clean gravels in a random size range of 1/2 - 3 inch diameter.
13. The stream bank shall be revegetated to short, brushy vegetztiun such as willc.v and
red-twig dogwood.
14. All temporary construction material shall be removed from the streambed at project
completion.

Addendum #1 to the Contract Documents for
MAPLEWOOD GOLF COURSE
S—#405 SANITARY SEWER PROJECT
TO ALL PLAN HOLDERS:
You are hereby notified of the following changes, deletions, additions,
correction and clarifications to the Plans, Specifications and other documents
comprising the contract documents for the City of Renton, MAPLEWOOD GOLF COURSE
5-405 Sanitary Sewer Project.
The golf course contains lawn sprinkler systems not shown on the plans. These
sprinkler systems will be located for the contractor in the field by the Park
Department, any damage to the marked lines shall be repaired to the satisfaction
of the Park Department by the contractor at his expense.
Any unmarked or mismarked lines damaged by the contractor shall be repaired by
the Contrac`- to the satisfaction of the Park Department dnd wil- be paid for
in accordance vith Section 1-09.6.
Addendum #1 is hereby made a part of the contract documents, and it's conditions
are fully binding on the plan holder and contractor. The contractor shall
acknowledge receipt of this Addendum #1 by signing in the spa, provided below,
attaching it to the proposals, and acknowledge the Addendum #1 in the proposal .

Sealed bids will be received until p.m. , mt the City
Clerks office and will be opened and puulicly read In the 4th floor conference
room, Renton Municipal Building, 200 Mill Ave. South.
NOTE: PRE-QUAtIFIC OF BIDDERS IS REQUIRED.

To bd considered for bid award, all prospective bidders shall submit a
*J&*-qualification submittal package to the City Clerks office no later
than 2: 30 p.m. , The pre-qualification submittals
will ce reviewed by the Engineer and the City staff for conformance with
the criteria, and the experience and general capability of the proposed
bidder to complete the work proposed. All submitting parties will be
notified by mail whether they have been approved to submit a bid on this
project.
The work to be performed within working days from the date of commencement
under this contract shall include, but not be limited to:
Furnish and install approximately 1,185 lineal feet of 15" sanitary sewer
pipe and 200 lineal feet of 8" sanitary sewer pipe together with all
required appurtenances and complete restoration as set forth in the design
drawings and specifications.
The City reserves the right to reject any and/or all bids and to waive any and/or
all informalities.
Approved plans and specifications and form of contract documents may bk. obtained
at the Public Works Department office at the Renton Municipal Building for .AC
deib+1%xa* S-L5.00 each set plus E 5.00 to cover postage , if mailed. The
deposit will be refunded upon return of the Plans and Specifications in good
condition within thirty days after bid opening. The mailing charge will not be
refunded.
A certified check or bid bond in the amount of five percent ( 5%) of the total
amount of each bid must accompany each bid.
The City' s Fair Practices and Non-Discrimination Policies shall apply.

SCOPE OF WORK
The work involved under the terms of this contract document shall be
full and complete installation of the facilities as shown on the Plans
and in the Specifications; to include, but not be limited to, the
installation of approximately 1,185 lineal feet of 15" sanitary sewer
pipe and 200 lineal feet of 8" sanitary sewer pipe together with all
required appurtenances, and the complete restoration of all public and
private property to its original condition. Any contractor connected
with this project shall comply with all Federal , State, County and City
Codes or regulations applicable to such work and perform the work in
accordance with the Plans and Specifications of this contract document.

1-07 LEGAL RELATIONS AND RESPONSIBILITIES TO THE PUBLIC
1-07.5 GAME, FISHERIES AND ECOLOGY REGULATIONS (SUPPLEMENTAL SECTION)
DELETE: Entire paragraph 3: "If wore :n 1 through 3 above differs

The Contractor shall comply with all the regulations of toe
permit issued by the Department of Fisheries regarding sewer
construction adjacent to the creek and including the encasing
pipe at the creek crossing. "
The contractor ■ay select to make the creek crorsiny by either open-cut or
jacking/boring. All labor and materials needed to effect the crossing,
including stream bank restoration and plantings shall be included in the
bid item "CREEK CROSSING WITH BORRED STEEL CASING PIPE OR STREAM
DIVERSION" per LUMP SUM, and no additional payment shall be made

Pa,-k Department property such as plants, shrubs, trees. fences, and
rockeries within the allowable construction areas shall be removed and
restored to the satisfaction of the Park Department. It shall be the
Contractor ' s responsibility to notify each Park Department when it is
necessary to remove such improvements to facilitate the Contractor 's
progress, and to remove those improvements to locations requested by the
Park Department or to restore them in their original location or as near
original condition as possible.
Such removal and restoration shall be considered inc ,dental to the project
and no further compensation will be made.

All phases of construction for emergency vehicles to the Golf Course Club
House and all other buildings within the golf course property.
7-17 SANITARY SEVERS
7-17.Si2)H SIDE SEWER CONNECTIONS (SUPPLEMENTAL SECilON)
The Contractor shall construct 8, o and a inch side sewers .rom the sewer
interceptor or lateral to serve the club house and the three residences.
The minimum grade for the side sewers shall be two percent i2U . The side
sewer connections to serve each building must be made between the soil
pipe and the septic tank.
Side sewer connection shall not start until the l5 inch sewer interceptor
and the 8 inch sewer lateral have been constructed, tested, flushed, T.V.
inspected, manholes channeled, and deemed ready for use by the Engineer,
The cost involved in providing all materials including tees, couplings,
surface cleanouts, pipe, bedding, labor , equipeent, restoration, and
testing the side sewers shall be included in the bid item price for side
sewers per Luep Sum. All restoration shall be completed to the
satisfaction of the City of Renton Parks and Recreation Department.
7-17.3(2)J METRO MANHOLE CONNECTION (SUPPL,EOENTAL SECTION
The Contractor shall acquaint himself with and comply with the regulations
contained in the permit issued by METRO and also with the regulations
contained at the back of th3 Special Provisions for cunnection to METRO
MHR1u-36. The Contractor shall replace the existing 12 inch stub with a
15 inch btub. All work involved in the compliance with the permit
regulations will be incidental to the contract pa, item.
7-17.3(2)K PUMPING AND FILLING SEPTIC TANKS (SUPPLEMENTAL SECTION)
After all side sewers are constructed, tested and accepted ready for use
by the Engineer , each septic tank must be pumped Out with the sewage
properly disposed of , the septic tank tops and bottoms must be crushed and
the septic tank filled with dirt and/or sand. There shall be no voids in
the backfill of each septic tank. Plug each septic tank outlet with a 1
foot section of murtar. All of the work fescribeo in this supplemental
section including restoration shall be included in the bid item price.
7-17. 3(2)L SURFACE CLEANOUTS (SUPPLEMENTAL SECTION)
The unit price bid for side sewers per Lump Sum, shall be full
compensation for all materials, labor and equipment necessary to construct
the 8 inch and 6 inch surface cleanouts in accordance with the Sewer

Standard Detail and restoration of the adjacent areas. the materials
shall include, but not be limited to, the wyes, plugs, and lamphole
covers.
7-17.3(3) BACKFILLING SEWER TRENCHES (SUPPLEMENTAL SECTION)
During construction of the sewer pipe from MH 5 to MH 61 the trench
backfill shall be 100%, at the conclusion of each days construction, on
all areas within the asphalt pavement. The cost for providing 100%
backfill shall be included in the bid item price for 15 inch sanitary PVC
sewer pipe.
7-17.3(5) STREAM CRCSSINf. (NEW SECTION)
Tte Creek crossing in Schedul ? B may be accomplished by either of the
following methods at the contractors option:
JACKED OR BORED CASING PIPE
Where shown on the Plans, the Contractor shall install the PVC
p.pe in a 2.0 inch diameter steel encasing pipe with a minimum
1/4-inch wall thickness. The steel encasir., pipe shall be
installed by jacking, tunneling or augernng or by a combination
of these methods.
During jacking, augerrng, or tunneling operations, care shall be
exercised to prevent caving ahead of the steel encasing pipe
which will cause voids outside the pipe. If voids occur , the
Contractor shall backpack the voids with sand and pea gravel and
fill the voids with a pumped Portland Cement grout.
If liner plates are used, they shall be assembled and installer
in accordance with the ■ancfacturer 's Instructions and
specifications and in accordance with accepted tunneling methods
using poling plates or shields of a strength equal to that of
the Inner plates.
The PVC pipe shall be skidded into position inside the casing
pipe and blocked into position The annular space between the
carrier pipe and the casing pipe or tunnel liner shall be filled
by sluicing or blowing sand or pea gravel into the space unless
otierwlse specified. Care shall to exercised to insure that the
entire space is filled and that the pipe is not d' sturbed during
the placement of the backfill between the pipe and the casing.
The Contractor shall remove the carrier pipe and reinstall It if
It will not accommodate the pipe laid to tolerances shown on the
Plans.
The unit price bid shall be full compensation for all materials,
labor and equipment necessary to construct the 20 inch diameter
encasing pipe including restoration. The carrier pipe, through
the encasing pipe, shall be paid under a separate bid item.

STREAM DIVERSION
The contractor shall provide a 48 inch culvert pipe of
sufficient len;th to carry the creek past the area of work. The
culvert pipe shad be placed in the existing creek bed and an
impervious dam shall be constructed around the up stream end,
diverting the creek into the culvert. Two filter check dams
consistin: of pea-gravel filled burlap bags shall be constructed
to filter silt and debris from the creek. One check dam shall
be located around the culvert pipe down stream of the sewer
trench, and the other shall be located 10 to 20 feet down stream
of the culvert end. These check dams shall be cleaned of all
accuaulation of debris on a daily basis during the construction
of the sewer trench, and shall not be removed until after the
bank restoration is completed and the diversion culvert is
removed.
The stream bed in the trench line shall be backfilled with
washed rack of a random size range of 112 to 3 inch diameter.
The stream bank shall be covered with loose medium rip rap rock
to a minimum thickness of 12 inches.
After all rock is placed and the diversion culvert is removed
plant the area with the following:
VACCINIUM VITIS IDAEA (Cowberrv. Linbonberrv)
Plant with one gallon size containers in a grid 30 inches
on centers covering the rip raped area from low water line
to the top of the bank.
IRIS SIBIRICA ( Siberian Iris)
Plant in groups of rhizomes in the center of each 30 inch
grid of VacciniuL Vitis Idaea.
All labor and materials required for the stream crossing ex,ci. ,
for the 18 inch carrier pipe and select trench backfi: l shall be
included in the unit price "Lreek Crossing 4ith bored steel
casing pipe or stream diversion", per Lump Sum, and no
additional pavement shall be made.

be removed prior to application of seed.
8-01.3(9) PROTECTION AND CARE Of SEEDLD AREAS iREPLACEMENI SECTION)
After all disturbed areas have been c1saned and restored to the
satisfaction of the =ngineer, and all seeding has been completed the Parks
Department shall assume the responsibility for watering, fertilizing, and

SCOPE OF WORK
The work involved under the terms of this contract document shall be full and
complete installation of the facilities as shown on the plans and in the
Specifications; to include, but not be limited to, the installation of
approximately 1,185 lineal feet of 15" sanitary sewer pipe and 200 lineal
feet of 8" sanitary sewer pipe together with all required appurtenances, and
the complete restoration of all public and private property to its original
condition. Any contractor connected with this project si:all comply with all
Federal, State, County and City Codes or regulations applicable to such work
and perform the work in accordance with the Plans and Specifications of this
contract document.

1-07 LECAL RELATIONS AND RESPONSIBILITIES TO THE PUBLIC
1-07.5 GAME FISHERIES AND ECOLOGY REGULATIONS (SUPPLEMENTAL SECTION)
DELETE: Entire paragraph 3: "If :cork in 1 through 3 above differs
little from that specified in the contract, etc."
ADD: "Work In 1 through 9 above will be incidental to contract pay
items."
The Contractor shall comply with all the regulations of the
permit issued by the Department of Fisheries regarding sewer
construction adjacent to the creek and including the encasing
pipe at the creek crossing."
The contractor may select to make the creek crossing by either open-cut or
jacking/boring. All labor and materials needed to effect the crossing,
including stream bank restoration and plantings shall be included in the
bid item "CREEK CROSSING WITH BORED STEEL CASING PIPE OR STREAM DIVERSION"
per LUMP SUM, and no additional payment shall be made
1-07. 16 PROTECTION AND RESTORATION OF PROPERTY (SUPPLEMENTAL SECTION)
Park Department property such as plants, shrubs, trees. °ences, and
rockeries within the allowable construction areas shall be removed and
restored to the satisfaction of the Park Department. It shall be the
contractor' s responsibility to notify each Park Department when it is
necessary to remove such improvements to facilitate the Contractor 's
progress, and to remove those improvements to locations requested by the
Park Department or to restore them in their original location or as near
original condition as possible.
Such removal and restoration shall be considered incidental to the project
and no further compensation will be made.
1-07.23(1 ) PUBLIC CONVENIENCE AND SAFETY (SUPPLEMENTAL SECTION)
The Contractor shall insure ingress and egress for vehicular access during
all phases cf construction for emergency vehicles to the Golf Course Club
House and all other buildings within the golf course property.
7-17 SANITARY SEWERS
','3(2)H SIDE SEWER CONNECTIONS (SUPPLEMENTAL SECTION)
T, �antractor shall construct 8, 6 and 4 inch side Sewers from the sewer

The minimum grade for the side sewers shall be two percent (2%) . The side
sev;r connections to serve each building must be made between the soil

testing the side sewers shall be included in the bid item price for side
sewers per Lump Sum. All restoration shall be completed to the
satisfaction of the City of Renton Parks and Recreation Department.
7-17.3(2)J METRO MANHOLE CONNECTION (SUPPLEMENTAL SECTION
The Contractor shall acquaint himself with and comply with the regulations
contained in the permit issued by METRO and also with the regulations
contained at the back of the Special Provisions for connection to METRO
MHR10-36. The Contractor shall replace the existing 12 inch stub with a
15 inch stub. ^.11 work involved in the compliance with the permit
regulations will he incidental to the contract pay item.
7-17.3(2)K PUMPING AND FILLING SEPTIC TANKS (SUPPLEMENTAL SECTION)
After all side sewers are constructed, tested and accepted ready for use
by the Engineer, each septic tank must be pumped out with the sewage
properly disposed of, the septic tank tops and bottoms must be crushed and
the septic tank filled with dirt and/or sand. There shalt be no voids in
the backfill of each septic tank. Plug each septic tank outlet with a 1
foot section of mortar. All of the work described in this supplemental
section including restoration shall be included in the bid item price.

compensation for all materials, labor and equipment necessary to construct
