BOX 5068 Z-VERETT, WA 98204 ATTEN: DUNCAN FALCO RE: DAMAGE AT S. 4th RENTON, WA Dear Mr. Falco: On May 2, 1994, at approximately 9:00 a.m., I heard a loud noise and looked over to see that a large van type truck had just hit one of the trees on the North side of S. 4th street I estimate the trucks speed at approximately 30 M.P.H.. I expected the driver to stop and investigate the damage, but he kept going 311 the way down So. 4th street. I took a few pictures of the damage. Two of my employee's cut up the branches and hauled them away. The major damage was breakage to a 6" diameter limb. _ The traffic was coned over to the north side of So. 4th street per the city approved traffic control plan. We provided a 14' wide lane for west bound traffic. There was more than enough room to allow truck traffic to pass by the '.rees. This particular traffic plan had been in effect for 4 days prior to May end with no eimilar problems. Numerous large trucks including semi trailers have been able to get thru just fine. In my opinion the driver as going too fast for a construction zone and he was not payir.g .i'-tention. ATTEN: JOuN HOBSON Gentlemen: On March i1 , and April 1, 1994, my crew encountered 5 separate uncharted utilities i.e; they were not shown on the plans. Three of these were not field marked, of which we broke a 2" water line and a 6" concrete pipe. Two of these were conduits for either power or communication that were field marked and ( we did not break. .a , Each of these instances cost my crew both time and extra costs to deal with the uncharted utilities. These items are compensable under section 1-04.7 of the contract. These are a type I change condition. The extra costs will be tracked with a separate costing system and submitted to you when all costs have been finalized. i While working on Logan Ave., we encountered numerous similar uncharted utilities. They also increased our costs and time. However, in the spirit of cooperation, King absorbed the extra costs, which amounted to several thousand dollars. Two extra's f were requested, one for a conflict at MH 5+72, and one for a _ water service. King can not continue to absorb these costs tt because the number of uncharted utilities is rapidly increasing. From the utility locates visible along Williams Ave. So, it is apparent that the side sewer crossings will also encounter numerous uncharted utilities. The manhole had to be extended to station 5 + 75 to allow the B" DI sewer to miss the existing storm manhole. The conflicts experienced at this location wre unanticipated by King and I assume also by the City because they are not address,d in the plans or design. King has incurred additional costs in dealing with t..ese utility conflicts while installing this manhole. I have prepaired a breakdown for these additional costs, and am including it with this letter. It took until 10:30 A.m. before the broken service line was shut off and my crew was able to start installing sewer pipe again. As you are aware, we also encountered several unknown conduit banks in this area. They did not however affect our operation as severely as the 'iroken water service which flooded our ditchline. Because of the extent of the cost for a 2 hr. shut down, I must regl.'est additional compensation for the delay caused by the _ unknown water service. However. Encicsed you will find two of the alternatives along with specifications and a sketch for which I am requesting that you submit costs. One alternative is to completely remove the existing concrete road and replace it with an asphalt surface. This work would include removal and installation of new curb and gutters, and installing the new road. The new road would consist of a 6-inch crushed rock base, a 5-inch asphalt I.eated base and a 3-inch lift of Class 8 asphalt. I � The other alternative is to crack and seat the existing concret9 and overlay it with i asphalt. This impact energy,and the striking pattem later described,will then,be used for the remainder of the project.Within the test section,coring and non- destructive deflection testing with a Falling Weight Deflectometer will be required to verify that cracks are being established full depth.The Er,ineer will obtain the necessary cores and conduct the necessary deflection tests.The Contractor shall supply water to assist in locating cracks in the concrete pavement. The water shall be applied to the surface in a fine spray. Following cracking and after the surface dries,water remaining inthe cracks will readily identify crack location. The equipment for cracking the concrete pavement shall be ap proved by the Engineer and shall be capable of producing the desired cracking without excessive displacement or spalling of the concrete. Unguided free-falling weights such as"headache or wreaking halls"will W be permitted to crack the pavement.The Contractor suall crack the existing concrete pavement in conformance with one of the following Patterns. Regardless of which pattem or pavement breaker is used,the pavement shall be cracked transversely such that adjacent cracks do not exceed 3 feet in the longitudinal direction. If a drop hammer is used(approximately 1200 lb.weight with small diameter(5")breaking head),the following pattern shall apply.The spacing between hammer blows shall not exceed 6"moving transversely across the pavement lane or lanes.For each transverse pass of the breaking machine,the pattern,hall begin one foot from the shoulder and proceed to the centerline of the pavement.The height of drop of the breaking head shall be adjusted to produce full drr,th cracking with a minimum of surface spalling. The transverse cracking pattern is not required to be in a straight line across each lane. A drop hammer which creates a slightly arc-shaped pattern across each lane is acceptable. If such a pavement breaker is used,the following nattem shall apply. The radius of the arc shall be seven feet minimum. The cracked pavement shall be seated by two passes of a pneumatic tired roller with a suitable body for ballast loading with such capacity that the gross load may be varied from 30 to 50 tons. If the pavemem is opened to traffic after the cracking and seating operations but prior to r placement of the Gist brie ninr-s course,the Contractor shall maintain the pavement by ' sweeping,asphalt patching,etc. t: ,,,e satisfaction of the engineer. We agree with you and Steve that 2-4" spalls and or recycled concrete will provide the needed foundation support required for the sewer main. Currently we have been placing 2' to 3' average depth of spalls. You have requested pricing for this extra work. I would like to track this work for a short while to allow a more precise estimate of the scope that will be involved. I will try to get propcsed pricing to you by mid to late next week.
