Remarks:
MTC inspector on site, as requested, to observe the placement of approximately 7 CY of concrete with Mix Design ID #1250AF
supplied by Miles into footings, at the location noted above.
One set of five (4” x 8”) cylinders was cast for compressive strength test specimens. Refer to Report #4966 for field test results and
for more information.
Concrete was placed via truck’s chute, was mechanically consolidated with highcycle vibrator, and finished by hand.
To the best of MTC inspector's knowledge, the abovedescribed work was performed in general accordance with project
specifications and approved plans.
Work in progress. Additional inspection needed.
