I arrived at approximately 1417 hours. UNIT 1:TOYOTA CAMRY Upon arrival I observed a black Toyota Camry(WA/CPZ8674, Unit 1) parked in a parking stall in Parking Lot A. There was damage to Unit 1's rear passenger bumper(dents and scratches). UNIT 2: NISSAN ROGUE I observed a silver Nissan Rogue (WA/CNK0799, Unit 2) unoccupied, parked in a parking stall in Parking Lot A. There was damage to Unit 2's front passenger bumper(dents and scratches). Unit 2 was registered to Adja O. Phipps (DOB: 01/01/1977). DRIVER 1: KWAN, HERBERT H. I contacted Herbert H. Kwan (DOB: 12/25/1952) near Unit 1. Kwan was the reporting party. Kwan was positively identified by his valid Washington Driver License and established as the driver of Unit 1 by his admission. Kwan was not injured in the collision. DRIVER 2: PHIPPS,ADJA O. Adja O. Phipps, Unit 2's registered owner,was identified as the driver of Unit 2. Officer: Gorajewski, Raymond #R12399 Reviewed By: Date/Time: 09/19/2025 06:15 Collision 2025-00008154 Paqe 2 OF 5 Case Number 2025-00008154 Officer Narrative-Continued me a photograph of Unit 2's driver on his cell phone,which matched Phipp's Washington Department of Licensing photograph. I was unable to locate Phipps inside the hospital. I wasn't able to find a contact phone number for Phipps. I left my business card with the case number on Unit 2's driver window, clearly visible. STATEMENT: KWAN HERBERT H. I spoke with Kwan about the collision. The following is a summary of our conversation: Kwan drove Unit 1 in the parking lot of Valley Medical Center. Kwan reversed Unit 1 from a parking stall. As he reversed, Unit 1's rear passenger bumper collided with Unit 2's front passenger bumper as it drove in the aisle from south to north behind Unit 1. Phipps parked Unit 2 in a nearby stall and quickly exited Unit 2. Kwan approached Phipps as she walked from Unit 2 toward the hospital public entrance. Kwan asked Phipps to exchange contact and insurance information. Phipps replied something to the effect of, "I don't have time, I'm in a hurry" and walked away without exchanging any information. This concluded my conversation with Kwan. PHOTOGRAPHS Kwan took photographs of the vehicles and Phipps as she walked through the parking lot. Kwan uploaded the photographs to Axon's Evidence.com via Axon Community Request. Kwan's photograph of Phipps was clear enough for me to easily identify her by comparison with her Washington Department of Licensing photograph. This incident occurred in the sCity of Renton,King County,Washington State. !declare underpenalty of perjury under the laws of the State of Washington that the foregoing is true and correct. officer: Gorajewski, Raymond #1112399 Reviewed By: Date/Time: 09/19/2025 06:15 Collision 2025-00008154 Paqe 3 OF 5 Case Number 2025-00008154 Officer Narrative-Continued PROBABLE CAUSE I have probable cause to arrest Phipps for Hit and Run Attended, RCW 46.52.020(2)(a),which states: The driver of any vehicle involved in an accident resulting only in damage to a vehicle which is driven or attended by any person or damage to other property must move the vehicle as soon as possible off the roadway or freeway main lanes, shoulders, medians, and adjacent areas to a location on an exit ramp shoulder,the frontage road, the nearest suitable cross street, or other suitable location.The driver shall remain at the suitable Location until he or she has fulfilled the requirements of subsection (3) of this section. Moving the vehicle in no way affects fault for an accident. Phipps was the driver of a vehicle (Nissan Rogue, Unit 2) involved in an accident resulting only in damage to a vehicle (Toyota Camry, Unit 1) driven by a person (Herbert H. Kwan) and failed to remain at the suitable location (private parking lot) until she has fulfilled the requirements of RCW 46.52.020(3). However, I am not going to issue Phipps a citation for this violation. I am waiting to conduct follow up to see if she had a legitimate reason to quickly leave the scene of the collision and enter the hospital.
