I arrived at approximately 0803 hours. UNIT 1: KIA OPTIMA Unit 1, a black 2013 Kia Optima (WA Temporary /A8731091), fled the collision scene south on Edmonds Avenue NE from NE 3rd Street prior to my arrival. Unit 1 was later located, unoccupied, parked in the 3100 block of SE 4th Street. Unit 1 had moderate damage to the driver doors from the collision. UNIT 2: CHEVROLET EQUINOX I observed a black 2015 Chevrolet Equinox (WA/ BTN4017, Unit 2) stopped facing east in the westbound number one lane of NE 3rd Street just west of the intersection of Edmonds Avenue NE. Unit 2 had moderate, disabling damage to the front driver wheel. Unit 2 had minor damage (scratches and dents) to the leading edge of the front-passenger fender. Unit 2 had not been moved from its point of rest prior to my arrival. UNIT 3: CHEVROLET EXPRESS I observed a black 2006 Chevrolet Express van (WA/ C16408N, Unit 3) stopped facing west in the eastbound left-turn lane of NE 3rd Street west of Edmonds Avenue NE. Unit 3 had moderate, disabling damage to the rear-driver fender and both rear wheels. There was roadway evidence (tire friction marks) that showed Unit 3 was traveling west in the westbound number one lane of NE 3rd Street west of Edmonds Avenue NE. Unit 3 was not moved from its point of rest prior to my arrival. UNIT 4: TOYOTA SIENNA I observed a gray 2015 Toyota Sienna (WA/AUJ6778, Unit 4) stopped facing west in the westbound number one lane of NE 3rd Street west of Edmonds Avenue NE. Unit 4 had moderate damage along the entire driver side. Unit 4 had not been moved from its point of rest prior to my arrival. DRIVER 1: UNKNOWN The driver of Unit 1 is unidentified. There is no suspect description. Unit 1's driver fled prior to law enforcement's arrival and failed to fulfill the requirements of RCW 46.52.020(3). DRIVER 2: HTWAY, SA A. M. contacted Sa A. M. Htway (DOB: 09/02/1985) standing on the sidewalk near Unit 2. 1 positively identified Sa by his valid Washington Driver License and established him as the driver of Unit 2 by his admission. Sa was not injured in the collision. DRIVER 3: SANCHEZ GALVEZ, RENE I contacted Rene Sanchez Galvez (DOB: 07/06/1975) standing on the sidewalk near Unit 2. 1 positively identified Rene by his valid Washington Driver License and established him as the driver of Unit 3 by his admission. Rene was not injured in the collision. DRIVER 4: NALIVAYKO, NIKOLAY I. I contacted Nikolay 1. Nalivayko (DOB: 09/21/1950) standing on the sidewalk near Unit 2. 1 positively identified Nikolay by his valid Washington Driver License and established him as the driver of Unit 4 by his admission. Nikolay was not injured in the collision. PAGE 4 OF 8 REPORT No.` EG36345 CASE# 25-8346 COLLI SICiN TIME OF 09/26/25 07:48 COLLI NARRATIVE PASSENGERS Ni T. Chin (DOB: 10/24/1977) was the right front passenger of Unit 2. Ni complained of chest pain and was evaluated on scene by the Renton Regional Fire Authority. Ni was released at the scene. Matthew R. Meikle (DOB: 10/15/1983) was the right front passenger of Unit 4. Matthew was not injured in the collision. Liliya N. Meikle (DOB: 08/03/1980) was the right rear passenger of Unit 4. Liliya complained of left arm pain and was evaluated on scene by the Renton Regional Fire Authority. Liliya was released at the scene. WITNESS I was approached by a witness, Allison R. Ferreri (DOB: 01/07/1994), at the scene. The following is a summary of our conversation: Ferreri drove her vehicle east on NE 3rd Street toward Edmonds Avenue NE a distance behind Unit 2. Ferreri saw Unit 1 "weaving" in an out of traffic, making frequent lane changes and driving too fast for traffic conditions. Ferreri saw Unit 1 change lanes from the number one (closest to curb) into the number two lane. Ferreri believed Unit 1's driver"didn't check their blind spot". Unit 1's driver side collided with Unit 2's passenger side as Unit 2 drove in the number two lane next to Unit 1. This collision caused Unit 2 to cross the center line into westbound lanes and cause a "chain reaction" crash with Unit 3 and Unit 4. Ferreri described Unit 1 as a 1990's-model black sedan. Ferreri didn't see the driver. This concluded my conversation with Ferreri. STATEMENT: HTWAY, SA A. M. I spoke with Sa about the collision. The following is a summary of our conversation: Sa drove Unit 2 east in the eastbound number two lane of NE 3rd Street toward Edmonds Avenue NE. A "blue car" merged from the number one to the number two lane, striking Unit 2. This collision caused Unit 2 to cross the center line and collide with Unit 3 and Unit 4. Sa could only describe the other vehicle as "a blue car". This concluded my conversation with Sa. SUMMARY Unit 1 traveled east in the eastbound number one lane of NE 3rd Street toward Edmonds Avenue NE. Unit 2 traveled east in the eastbound number two lane of NE 3rd Street toward Edmonds Avenue NE next to Unit 1. Units 3 and 4 traveled westbound on NE 3rd Street west of Edmonds Avenue NE. Unit 1 changed lanes, striking Unit 2, which caused Unit 2 to cross center into westbound lanes, striking Unit 3 then Unit 4. PROXIMATE CAUSE The proximate cause of the collision was determined by statements made by Ferreri and Sa, damage observed to all involved vehicles, and tire friction marks left on the roadway surface by Unit 3. The unknown driver of Unit 1's actions caused the collision, namely their violation of RCW 46.61.140 (1), which states: A vehicle shall be driven as nearly as practicable entirely within a single lane and shall not be moved from such lane until the driver has first ascertained that such movement can be made with safety. PROBABLE CAUSE If the driver of Unit 1 can be identified, there is probable cause to arrest them for a violation of RCW 46.52.020(1). The unknown driver was the driver of a vehicle (Kia Optima, Unit 1) involved in an accident resulting in the injury to a person (Liliya N. Meikle and Ni T. Chin) that failed to immediately stop the vehicle at the scene of the accident or as close thereto, and failed to meet the requirements PAGE 5 OF 8 REPORT No.` EG36345 CASE# 25-8346 O COLLI COLLISION TIME OF 09/26/25 07:48 COLLI NARRATIVE of RCW 46.52.020(3). DISPOSITION This case is closed "RTF". There is no suspect description and no one can identify the suspect. This concluded my involvement in this case.
