THIS AMENDMENT, dated for reference purposes only as July 10th 2025, is by and between the
City of Renton (the "City"), a Washington municipal corporation, and Bravura Counseling, PLLC
("Consultant"), a professional limited liability company. The City and the Consultant are referred
to collectively in this Amendment as the "Parties." Once fully executed by the Parties, this
Amendment is effective as of the last date signed by both parties.
Whereas, the City engaged the services of the Consultant under Agreement CAG-24-008, dated
January 22, 2024 to provide necessary services for the Wellness Check-In Program (referred to
herein as the "Agreement");
Whereas, the Parties wish to amend the Agreement to extend the time for performance in order
to utilize funds awarded through a 2025 WA State Criminal Justice Training Commission (CJTC)
grant and reflect an additional time of performance between July 2025 through December 2026;
and
Whereas, the Parties wish to amend the Agreement to modify the scope of services, to amend
the total number of wellness check-ins from 60 to 100, specifically adding 10 check-ins each
quarter between January 1, 2026 to December 31, 2026.
NOW THEREFORE, It is mutually agreed upon that CAG-24-008 is amended as follows:
1.Scope of Work: Section 1, Scope of Work, is amended to add an additional forty wellness
check-ins, taking the total number of check-ins from 60 to 100. Such additional check-ins
shall match the pace and quantity of the check-ins set forth in Exhibit A to Cag-24-008,
specifically: 10 quarterly check-ins between January 1, 2026 to December 31, 2026.
2.Time of Performance: Section 2, Time of Performance, is amended to reflect that all Work
shall be completed no later than December 31, 2026.
3.Compensation: Section 3, Compensation, is amended so that the maximum amount of
compensation payable to Consultant is increased by $30,000 from $45,000, and shall now
not exceed $75,000, plus any applicable state and local sales taxes. The additional
compensation shall be paid based upon Work actually performed according to the rate(s)
or amounts specified in Exhibit A to CAG 24-008, plus any amendments thereto. The
compensation should reflect the additional time added to this Agreement, from July 2025
to December 31, 2026.

4.All terms of the Agreement not explicitly modified herein shall remain in full force and
effect and such terms shall apply to Work performed according to this Amendment as if
fully set forth herein.
IN WITNESS WHEREOF, the Parties have voluntarily entered into this Amendment as of the date
last signed by the Parties below.
