Remarks:
MTC onsite as requested to visually inspect reinforcing steel and concrete placement at the following location(s):
1.) Castinplace concrete for the site stairs in the south west corner of the project site.
2.) Castinplace concrete for the field lighting vault slab
MTC observed that the stairs and slab noted above was reinforced per the following details:
Site stairs per detail 1/F270
Field lighting vault slab per detail 1/E9.01
Reinforcing Steel Inspection: MTC visually inspected the reinforcing steel at the locations noted for compliance with the approved
project plans and specifications. MTC observed the reinforcing steel size, grade, type, and placement. All reinforcing steel was
observed to be ASTM A615 grade 60 reinforcing steel and welded wire mesh reinforcing.
Concrete placement observation: MTC observed the placement of 19+ cubic yards of the projectapproved concrete mix
#1250AFG delivered by Miles Ready Mix for the noted areas above. Concrete was placed via placing boom pump truck and properly
consolidated by screeding. MTC took a composite sample and cast one set of (5) 4x8 cylinders to test for the required 4000 psi
design compression. Reference MTC concrete sample #5228 project set #8 for the compression strength test results. MTC notes
the site stairs required 4000 psi and the field lighting slab required 3000 psi. The contractor used a 5000 psi approved mix design.
