ASSIGNMENT/ARRIVAL
On 10/14/2025 at approximately 1442 hours, I was dispatched to a two-vehicle, non-injury
collision in the parking lot of 801 SW 16th Street in the city of Renton, King County,Washington.
This location was a warehouse, and the collision occurred in the loading dock.
I arrived at approximately 1450 hours.
UNIT 1: FREIGHTLINER M2
Upon arrival I located a white Freightliner M2 (WA/ D73644H, Unit 1) box truck operated by
We-Haul Moving Services, LLC, USDOT 3918815. Unit 1 was backed into loading bay 51 and the
driver was unloading items into the warehouse.
Unit 1 was not damaged in the collision.
UNIT 2: FORD F250
I located a gray Ford F-250 (UT/Z250KG, Unit 2), towing a utility trailer, operated by Terra
Vertical Communications, LLC. Unit 2 was backed into loading bay 52. Unit 2 was unoccupied at
the time of the collision.
Unit 2 had minor damage from the collision: damage to the front-passenger bumper,
front-passenger headlamp assembly, and the leading edge of the front-passenger fender.
DRIVER 1: PEREZ VALENCIA FAUBRISIO 1.
I contacted Faubrisio I. Perez Valencia (DOB: 04/10/1995) unloading items from Unit 1. 1
positively identified Perez Valencia by his valid Washington Driver License and established him
as the driver of Unit 1 by his admission. Perez Valencia was not injured in the collision.
This incident occurred in the sCity of Renton,King County,Washington State.

Perez Valencia said he was backing Unit 1 into loading bay 51, Unit 2 was parked in loading bay
52 just to the south. There wasn't much room to back, and Unit 1's rear-driver bumper collided
with Unit 2's front-passenger bumper as Perez Valencia backed.
DRIVER 2:WIGGINS DONALD W.
I contacted Donald W.Wiggins (DOB: 11/12/1980) near Unit 2,who was the responsible party for
Unit 2. Wiggins said he was unloading items from the trailer being towed by Unit 2 when he felt
the slight bump from the collision.
DISPOSITION
Both parties exchanged information.
This concluded my involvement in this case.
