No additional allowance for overhead or profit shall be paid for equipment on standby under any circumstance. When equipment rented by the contractor or subcon- tractor from outside sources is ordered by the Engineer to be held at standby, payment will be computed on the basis of actual invoice cost and no allowance for overhead %nd profit will be made. (e) When breakdown of any equipment being used on force account work causes shutdown of part or all of the equipment being used, payment for such equipment that is idled shall cease. Labor that is idled and cannot be diverted to other work Shall be paid through the one-half shift during which the breakdown occurs. (f) Individual pieces of equipment not specifically covered by the schedule and having a value of one hundred (100) dollars or less shall be considered "small tools and equipment for which no rental is allowed" as specified in Section 9.o4 of the Standard Specifica- tions, Amendment No. 1. The schedule is issued after conference with representatives of the Mountain Pacific, Inland Empire, Tacoma and Oregon-Columbia Chapters of the Associated General Contractors and has their approval for the calendar year 1967 subject to either party requesting a review of the rates after a Six-month period. In preparing force account authorities, sufficient details should be shown of the varimus types of equipment to definitely establish the proper rental rate to be applicable for each piece of equipment to be used on force account work. 1967 p.la 1 ? OF HIGHWAYS Equipment Rental Agreement for Force Account Work Effective for Force Accounts Authorized After January 1, 196T The attached schedule and this agreement are to be used for establishing rental rates to be paid contractors for equipment used or force account extra work in accordance with Section 9.04, Amendment No. 1, of the Standard Specifications. Elements of cost compensated for by these rates and general governing conditions for payment are specified therein. For clarification of the general governing conditions for payment included in the standard specifications, the following criteria have been agreed to. (a) If contractor-owned equipment 1s not available and equipment is rented from outside sources, payment will be computed on the basis of actual invoice cost. Use of non-owned equipment at rates higher than those set forth horein will not be allowed, except upon special approval of the Engineer. (b) The established rates are based on 'operated costs less labor", however, when the operation of an item of equipment is an integral part of the operation which requires it to be ased intermittently at a frequency and duration which causes it to be manned and ready for use on a full time basis, payment ::hall ba made on a full time basis, For such payment, actual use .ime must exceed 50%. (c) When the operated use, of eqr " moment is icfrequent and, as determined by the nature of the force account work need not remain at the site of the work continuously, payment shall be limited to actual hours of use. The contractor Is expected to diver; such equipment to other work if practicable. When such 11 ve.-rion is not practicable as confirmed by the Engineer, and the operator cannot be used elsewhere and must be paid through the one-half shift in which he actually performed work, payment for the opera+or shall be made through the one-half shift. July 27, I19FTY—'FWR-'k-c1WnrY " The following is a summary of activities relative to the emergency service required to restore water service on the 161, supply line in the vicinity of Maple Valley Interchanio. Soma of this material is from memory and some from notes taken during the emergency construction project. The purpose of this memorandum is to provide a running account of activities surrounding the 16" water line break. ^iccures are available to document the constructlin phases of the work. The initial report on the water line breal was made by an unidentified v.ruck driver who vmtacted the street department. The water utility was then started and steps taken to shut down and close off the vicinity of the break. This occurred at approximately 2:30 P.M. July 26, 1968. At 2:e5 P.M. Vic TeGontvoort, Street Commissioner, picked me up at the Engineering Office and we proceeded to the scene, Upon initial survey of the levee, it was determined that water had forced up through the off ramp end then burst out the side of the fill from the off ramp approximately 10' or more below the grade elevation of the off ramp_ Gushing water including wash- out fill material was flowing directly to a storm drain below the break In substantial amounts. I immediato,y returned to the office and assigned Ben Peterson to locat- ing mops of the break area, Teron Gibson to check the stop line drain, Leiv Erikson to identify the area which would be out of voter and determine need earmoctions for emergency s Tpply, and Gary Case to take pictures of the scene. Wo then contacted Glen Guy at the local Highway Engineer Office, and advised his. of the break and the need for identifying the location of the Pipeline casiu.g. We also contacted the Highway maintenance department and r4wised them of the Lreak, and requested information as to who to contact for weargency clearance. I than contacted Mr. Webb of the Highway Department still obtained emergency clearance to proceed. After storm llewors wore cleared and area of outage identified, Theron Gibson was dispatched and the Fire Department contacted to obtain adequate hose for 2 - 2 1/2 inch lanes from a siamose connection under too Cedar River north bridge to the firm hydrant Immediate 1, north of the City Hall. Roturnins to the scene I discussed tho problws with Don Tay/ir, our Field and Construction Inspector, anb with Ads Rowe of the water department and determined that City equipment was not of a size and nature to be .ble to contra' , �e break. At the time of the break our major concern was to locate the daaagsd part, make a temporary or permanent repair since this waterline is the mein supply from our water sources to Mt. Olivet storage tank and Highland reservoirs which serve all of Renton served of our own water except for the downtown area. A secondary supply of this area except for '!aple Valley area is via Windsor Hill pump station and we would be able to serve this area for a length of time any place from 3 days to a week, considering no fires in the area, or shut off of electricity causing the pumps to stop or pump breakdown. In case of anything like that our water supply could only 1 hours. We have no secondary supply for the Maple Valley and the area was temporarily served via 2 fire hoses. This area would be im- possible to serve this way on a regular week-day. 'The Maple Valley area consists of 208 residential customers, the Riviera Apts, Maple Valley i;: If course and industries Ilk* Rainier Asphalt Co. , Staneway, Security Concrete City Light service Bldg. and other businesses depending on water service. To be able to esrve this area on a weekday, a temporary 6" waterline would have to be strung about 600 LF and necessary connections made. In case a rerouting as shown was done a temporary repair of the existing pipe would have to be done to be able to serve the above mentioned areas. A rerouting would take approximately 2 weeks (estimate of our water department) considering that pipe and fittings could be delivered when ordered. On 16" O.J. pipe and fittings. There is usually a waiting time ue to JJ days. It would probably also be required from the State Highway Dept. that engineered plans should be submitted for their approval. Ado Row advised that a man was stand- ing by at th hater utility and ductile pipe loeded and prepared to resrond to the scene as soon as it was needed. At approxivtely 2:4S P.M. the Mayor was given an offside status report by Theron Gibson. At 3:00 P.M. Telephone Inspector left havink determined the telephone installation was safe and rat undermined and that hand digging would disrupt it. 3.45 slight cave in occurred for which pictires were taken. Contractor obtained additional timbering and by 4:00 P.M. one additional mar cans to work for the contractor. At 4 22 P.M. two additional men came to work for the contractor. At 4:30 two of the men who had been working all day were relieved. Car A-4S, which has a radio, was kept on the scene in crse of a cave in requir- ing emergency assistance. Leiv Erikson was advised of the calling procedures relative to a cavoin or other inadent and a radio check was made at approximately 6:00 P.M. Police dispatchers were contacted throughout the night and days activities to maitstain monitoring of the radio net to receive any emergency calls from the jobsite. Two questions arc outstanding at the moment - one relative to the backflll to be procured for the embankment, contractor advises that State Highway Inspector Vic' had approved that next to the City Pit for backfill. State iosires the off ramp to be cleared by Monday morning prior to Boeing traffic. Determination will need be "de of the water problem to determine tim- ing requirements and availability of personnel and equipment. Should timely installation of the 16 ' pipe be made we would attempt to make the Monday morning deadline on the off-ramp. However, this is contingent upon the actual pipe repair. The 16" pipe controls the flow from Talbot Hill Reser- vair through the City to Mt. Olivet ieservolr. The Pump Station at Mt. Olivet then pis water to the Highlands area. An extra" water shortage could and will exist if this repair is not affected imnediatnly. At this time It is my estimste that barring additio al difficultiae the water line should be in and reedy for servief at approximately nom Sudday. At that time determination will be mad* regardink oackfilling. My recommendation Is at this point would he to proceed Immediately with ;ht backfilling. End of 6:00 P.M, repert Saturday July 27, 1968. Y The areas affected by water shortage were notified and radio and T.V. coverage was given to the water shortape resulting frnn the break. Mr. Puhich of the hater Utility checked the break location early in the evening and was on hand until the break oasis.<im. At approximately midnight the Marion Crawler shovel was coved in and r excavation proceeded to locate the pipe. In the initial attack we had excavated level at approximately 8' aoove the pipe into the identified area of the break m &W ttian dug down to the pipe, txtrema caution was exercised in working around and at the break location to prevent damage to the telephone facilities which ware located approximately 5' southerly of the 160 line. 8y 3:00 A.M. sufficient excavation and exploration had been made to , •.er- mine that the de-watering would be necessary before additional work could be performed. One man from the contractor's crew was detailed to operate the pumps throughout the rest of the night. Theron Gibson was assignee, to stand by for the City throughout the remainder of the night. All other activities were shut down teWorarily until the de-watering could be effected. [ion Taylor had left the scene at around 1:00 or 1.30 A.H. after the initial identification of the location was made and Theron Gibson case on duty at 2:00 A.M. Dick Houghton was also available and at the scene from 1:00 A.M. to 3:00 A.M. to assist in the operation and determination of action and was available for call throughout the weekend. At 3:30 A.M, Dick Houghton and I left the scene with Theron Gibson on duty. Leiv Erikson caw on duty at 8:00 A.M. and Theron Gibson vas retained until 6:00 P.M. Saturday evening for jobsite inspection. I returned to the scene at apptoximatoly 8:00 A.M. and advised Leiv [rise^ to check over certain items and to watch the scan- while Thelon Gibson had breakfast and we would deter- adne whether or not further action was necessary. Contractor 's pors�nnel were available at 8:00 A.M. one continued de-watering and preparation for mobilisation Of equipment and 'blocking, umbers, clay spade and stir compressor necessary for the tunnel work, It had been determined during the night that this tunnel with cribbing would be constructs( over and along the pipe which was broken in order to remove it without further disruption to the pavement and fill areas, The Marlon Crane cleared the excavation further to deter sliding material and the tunnelling oper- ation commenced. Lagging and tunnel cover was started at. approximately 10:00 A.M. I released uiv Erikson at approximately 10:00 A.M. and requested 1-im to return at 5:00 P.M. Theron Gibson made his report of the night's activities and advised that he would be able to continue through the day for inspection work. At 5:00 P.M. Leiv Lrikson reported to reliev- 11,vron (ibson. Discussion Of the day's activities mad a report was made by Theron Gibson. Laiv Erikson was dispatched to the jobsite for continued inspection on the site. Theron P. 2 Higbwey Department maintenance personnel. Ben Paterson had contacted the telephone company and the Gas Company relative to their utili'ies in that ►lcinity. A telephone man was dispatched and was on duty all night until 3:00 P.M. Saturday. The Gas Company employees indicated no gas lines in the immediate vicinity. There upon contacting the Mayor and requesting emergency approval, we i obtained gill Frost, local contractor, to mobilize and move in man and equip- ment to determine the extent and correct the problem. bill Frost was on the job around 5:00 P.M. and moved in equipment, men and loader with work com- mencing with the loader at approximately 7:00 P.M. Two men were working on the site for the contractor and additional men for the contractor were mobil- ising equipment at other sources to move in for repair of the break. Around 6:00 P.M. Mr. Webb of the Highway Department appeared at our office and advised that his Supervisor, Mr. Magnuson, had determined that we should delay any activities until Monday and submit a plan of attack to the Highway Department for their approval. I discussed this with Mr. Webb and then called Mr. Magnuson and obtained oral clearance from his through Mr. Webb to proceed with the exploration and repair due to the emergency nature of the condition. They indicated that they would check later that night to determine if special measures need be taken relative to sheet piling of the highway to prevent caving. Pictures identified some of the construction progress during the night sal following day activities. At 8:30 P.M. a tractor backhoe was in operation and approximately S men from Frost crew. At 9:00 P.M. it was necessary to pull the excavating equipment out and pump down the water level in order to proceed with further excavation. By this time the highway had been adequately barricaded. A utility truck of Mr. Frost's with yellow lights above had been placed in the off ramp route should someone miss the barricades. ster a marion track shovel was moved in on the jobsite avai'able for later work. Theron Gihson, Ban Paterson had been released at approximately 6:30 P.M. along with other Engineering Personnel that had been standing by. Water utility and Street Department Personnel were available and contacted us at various intervals during the night and day relative to conditioru and their activities. We have no time breakdown an the utility or street department employees as that my be obtained from their supervisors and foremen. Ads Rowe was on hand and checked back with us during the night and through the day relative to the water utility mobilization of !spare parts necessary to effect repair break. Matt Kahl also provided valuable assistance and advice during the early hours of the emergency. W. Custer, Mayor On Friday, July 26, the City of Renton experienced an emergency when a 16" water line erupted under Highway 405 off-ramp in the vicinity of the Maple Valley Highway. The response to this emergency by all City departments was exemplary. Special note should be taken of the efforts of Don Taylor, Bert McHenry, Don Miller and Adrian Rowe. In addition, recognition is due the employees in all depart- ments for their effort and perseverance in meeting this emergency. A bad situation could have been much worse but for the skill of those employees who acted quickly and decisively when confronted :with this emergency situation. Than% you for your help. D. W. Rowe, Larry Saviage plus other water and street employees. 5:30 A.N. The crow left. Three crew man remained on site working with gill Frost. Leiv Erikson was relieved At ■pproxirteiy 6:00 A.M. by Don Taylor. Don was on hand on earlier for advice and assistance. At 3:00 A.N. Sunday water line bad been recharged and water utility was again balancing the system end bleeding off air. 3ackfill had �ieen mad under, around and over the pipe and r-ompacted leaving a I- pipe from the void err to the surface. The void aroa had been filled and compacted as well as could be. It was felt that further grouting after the backfilling was complete would re- move any chance of minor voids occurring in the deeper area of excavation. The Street Department's trucks began at 8:00 A.M. tc provide backfill to the site and the Contractor and crows continued throughout the day backfilling and compactirng. Following is an excerpt from bon Taylor's report covering Surday's activities : To 8,00 A.M. 3 crewmen plus 5111 frost were working. After 8:0f A.M. one crowman plus Bill Frost remained. Finished connection at watermotn at 6:30 A.N. Hauled in about 50 yards of scrooning. COMleted bedding of pipe at 7:50 A.M. Waited I hour for State Inspector. Than went ahead with beckfil, without his. 8ackfill compacted in one foot lifts. The equipment used at this time included Car Tractor. Cat 950 Loader. Cat 941 Trackloador Vibrator, Roller. 9:30 A.N. Two crewman plus kill Frost. 10:30 A.M. Two Crewman. Those men continued throughout the day and finished. Off-ramp opened at 4:30 P.N. Sunday afternoon, Highway Crews cleered the barricades and provided servict again on off ramps. Theron Gibson's Report 7-28-68 3:00 A.M. I Informed his that this was an emergency condition and that this Amin had to be repaired ASAP. If we are to Install casing itwould be required to go back to the existing casing tie in. Be- STATUS REPORT 161, Water Win break at Maple Valley highway and Highway 40S Tim: 12:30 Monday July 29, 1968 by Part wclfenry This report continues the Saturday night 6:30 P.M. report started in is random fashion relative to the 16" water main break. Items included here are information from various notes, personal observations and other information relative to the work porformed on the watermein break. After 6:30 P.M. Saturday discussion with gill Frost, Contractor, and Vic T4Cantvoort, Street Commissioner, indicateu the probability of backfill necessity on Sunday July 28th. Tentative schedule was established to complete the repair of the broken lo" water main dur- ing the night by continuing the tunneling and replacing two IS, sections of pipe with class 4 ductile pipe. We agreed to have the street department provide the beekfill asterial at the site for the contractor to backfill and compact after completion of the watermain installation and water turnoff. Leiv Erikson was to contact the State relative to their having onsite inspection during backfilling operations. Several attempts to contact were made by telephone and at 10:30 P.M. Lely Erikson finally contacted '4r. Wissman of the :lighwny Department. He advised that everything was OK and that Mr. Webb would be at the site on Sunday morning. Following is an excerpt from the notes made by Leiv Erikson relative to work per- formed from 4:40 P.M. Saturday to 5:30 A.M. Sunday morning. One man additional by the contractor at 5:30 P.M. 6: 30 .3 men on tunneling. Expect to finish tunneling by midnight. They are making good progress t.nd I expect them to be ahead of their estimated time. Pictures taken around this time include the key ores, the total axcovation area from the top view, inside the caved area, and the card opening. 9:00 P.M. called tong distance to Mr. Wissm4n - left message to call later. 10:30 P.M. called long distance to Mr. Wissmr • everything O.K. Johansen, Right of Way Agent, State Highway Department, regarding the City's clair in the sum �7409.66 on the washout on Limited Access Highway 405, Maple Valley Highway on July 26, 1968. It was Mr. Johansen's position, at least at this stage, that the State would not pay more than one half of the City's bill. We feel that this offer should be rejected. We assume, as you had previously indicated, that there is no question but that the water line was crushed because of the excessive fill of the freeway and the failure to properly pro- tect the line at the time of construction. We also assume that it was the State 's position that the line does not have to be cased or otherwise protected. As we had suggested to you last August, it is imperative that the City seek the services of an expert to determine the proximate cause of the line breakage to substantiate the above. V would also like you to verify the fact that the Staff never :eived any permit or easement, orally- or in writing, from the City at the time of construction relative to the subject water line.
