Dear Grantee:
We have prepared this letter as a guideline to assist you
with preparation of the. "LPW Project Performance Report"
(ED-113A) and also to supply you with additional forms for
future use.
The ED-113A consolidates a number of routine reports into a
single format. Our experience with FD-113A's received for
the quarter ending December 31, 1977 ins'-^rotes several
common questions on the use of the form which are identified
and further clarified below.
The required reports for all Round I and Round II projects
are:
1. Quarterly Reports which are due at the
Seattle Regional Office each Jaunary 15,
April 15, July 15 and October 15.
2. The Interim Report which is used to initiate
action to amend the Letter of Credit or, if
applicable, request a Treasury Check. This
report must indicate at least 00 percent
project completion, and must include a request
for additional financing. This request may
he noted in the "remarks" section of the
ED-113A or in a cover letter. Please note
that a second Form ED-530A (Minority Business
Enterprise Report) is also reauired for all
Round II projects.
3. The Final Completion Report is to be submitted
upon 100 percent completion of the project.
In each case, the ED-113A should Le submitted in duplicate
with original signatures on each ropy. In the event addi-
tional copies of the form are needed, you may xerox a blank
form. The Regional Office will not be sending additional
forms after this date unless requeste('.
The ED-113A should not be used to replace Form CD-286 (Report
of Federal Cash Trar.sacti.ons) . The CD-286 is intended to
describe the financial status of the Letter of Credit whereas
the ED-113A describes the financial status of the entire
project by line item. The CD-2ro is still required to be
submitted quarterly to Washington, D. C. in accordance with
the Engineering and Construction GuidelineA - Letter of
Credit Proce urd es.
Construction Status (Item 3, Page 1) , All completion status
percentages and dates should reflect the entire project and
are not to be limited to particular phases. Construction
status for phases should be described in the "remarks"
section.
The remarks section, Item 4, should include as a minimum the
information required by the instruction sheet accompanying
the ED 113A form.
Regarding the Status of Funds (Item 11, Page 2) , you will
note that each applicable item (a) through (m) represents a
specific line item of the approved project budget ED-508
which was enclosed with your Grant Offer. Eligible project
costs can only be charged against the approved bue-P.t. The
Regional Office will be comparing the ED-113A with the
correct EDA approved project budget. ED-113A's which show
costs charged against nonexistent line stems may be returned
for correction perhaps delaying release of funds. A special
note is made of line (m) miscellaneous cost. This line
r�apresents the contingency line of the approved budget.
According to the Enline�erin and Construction Guidelines,
use of contingency £unas must ave EDA Regions O ce
approval. When EDA approves the use of contingency, the
approved funds are transferred by a budget revision to
another appropriate line item and the costs are then charged
against that other line item. In effect, no costs against
line (m) should be shown on the ED-113A.

We would also like to take advantage of this letter to
remind all grantees of the requirement to submit certain
project documents to the Regional Office which will enable
us to properly monitor your grant. A checklist of the
required documents was sent to you along with our initial
"welcome aboard" letter. Release of project funds may be
delayed pending our receipt of all of the necessary docu-
mentation.
If you have any questions concerning these submissions,
please contact your regional project team leader.

• - A REMINDER - •
Based on our experience to date with letters of creilit we would 31ke
to reemphasize seven points you should remember when drawing funds fob your
local Public Works project(s). :�
1. Cash drawdowns should only be initiated when actually needed for dis-
bursements on eligible costs after EDA approves the start of on site tal,or
activity. Drawdowns should be tamed so they are, in fact, rolmlwtsewont
of funds disbursed for incurred cost.
2. The voucher number on the TFS-5401, Payment Voucher on Letter Credit,
will begin with "1" for each letter of credit and will progress in rr0-
secutive order. Alphabetical designations cannot be used as part of the
voucher number. Amendments to the letter of credit will not into,tupt
the progression of the numbers assigned to the pahscnt voucher bl the
grantee.
3. The two individuals who sign form TFS-5401, must lv authorized on the
SP-1194, Authorized Signature Card, previously sul"itted. The autha
rising official who certifies the SP-1194, may sign the TFS-!,401 only It
his/her signature also appears as an "Individual authorized to Draw on the
Cited Letter of Credit" on the SP-1194. The autiorizing official is not
automatically eligible to sign the TFS-5401.
4. All blanks on the TFS-5401 must be filled-in. The complete letter of
credit number and project number must appear on every paymcnt vourher.
The name and address of the grantee's commercial bank and releral i:i.artvv
Bank or Branch must also be recorded.
5. Three copies W ie, green, and white) of the TFS-5401 must he sulmtttrd
to your commercial bank. The White copy will be fonnrded throuuh your
bank and the Federal Reserve to EDA.
6. Payment vouchers should not be drawn more frequently than daily. tuv for
less than $5,000 unless there is a balance of loom than $5,000 which is
withdrawn in its entirely. Payment vouchers should not be drawn for more
than $1,000,000.
7. When Funds a.re drawn for more then one project on a consolidated Lotter
of Credit, PLEASE, use only one voucher and list the projects and am..tmts
on the back of the white copy of the voucher.

Dear Mr. Olsen:
Our records show that construction of the above-referenced
project is nearing completion. Please be advised that the
following material must be su' fitted to the EDA Regional
Office before this project ca„ be closed out:
1. A °inal Project Performance Report (ED-113A) ,
indicating the date that the project was 100%
complete.
2. A Final Budget Revision (ED-508) , showing actual
costs.
3. A Final Inspection. Report signed by the grantee
and the architect engineer. (See paragraph 31
of the Fngineering and Construction Guidelines.)
4. A Final Acceptance P.eport signed by the pro3ect
designer, the project coordinator (if applicable) ,
the project inspector, and the grantee.
S. Two prints each of at least two photographs of
the completed project. (All photographs must
conform to the instructions in paragraph 39 of
the Engineering an3 Construction Guidelines.)
6. The date that the?ina audit will begin.
7. The duration of the project warranty.
8. Five copies of the Final Audit Report. (These
should be addressed to the Regional Audit Manager
in accordance with the Instruction!; for Audit of
Grants.)
9. The results of the Warranty Inspection. (See
paragraph 37 of the Engineering a Construction
Guidelines.

has been determined by the program evaluation analysts that further wlzds-
sion of ED-7,6 payrol forms is not required for the subject project.
Accordingly, effective imediataly, contractors and subcontractors working
on the subject project should 14;Lxntinue submission of the ED-746 payroll
foes to EDA.
Be aware, hmever, that there is no relief frog. any other responsibilities
including the submission of other required forms associated with the
project. 'lie 4di-347 Payroll Form (or its equivalent) , for example, must
continue to be ca' leted and submitted to the LIW grantee in order to
armly with the L,.vis-Sawn Act. In addition, if any other I evaluation
support activities ace associated with the subject project, these activ-

Our records show that construction of the above-referenced
project is nearing completion. Please be advised that the
following material must be submitted to the EDA Regional
Office before this project can be closed out:
1. A Final Project Performance Report (ED-113A),
indicating the Otto that the project was 100%
complete.
1. A Final Budget Revision (ED-508), showing actual
coats.
3. A Final Inspection Report signed by the grantee
and the architect/engineer. (See paragraph 31
of the Engineering and Construction Guidelines.)
4. A Final Acceptanc Report s gne y the project
designer, the project coordinator (if applicable) ,
the project inape^.tor, and the grantee.
5. Two prints each of at least two photographs of
the completed project. (All photographs must
conform to the instructions in paragraph 39 of
the Engineering and Construction Guidelies.)
6. The date that the final audit will 0911n
7. The duration of the project warranty.
8. Five copies of the Final. Audit Report. (These
should be addressed to the Regional Audit Manager
in accordance with the Instructions for Audit of
Grants.)
9. The results of the Warranty Inspection. (See
paragraph 37 of the Engineering and Construction
Guidelines.

This memo is to verify by record that the subject W-251 was
inspected prior to the end of the warranty period. There
were no deficiencies or °ailures in this particular portion
of the proiect.
Upon the end of the warranty period of W-450, a report will
be filed.
