Narrative
CC 25-9296
On 10/27/2025 at 1232 hours I was dispatched to a motor vehicle collision at 108th Ave SE and
SE Petrovitsky Rd in the City of Renton, King County,Washington.
Pre-Collision
Driver 2 stated that he saw Unit 1 behind him originally around the 12800 block of SE Petrovitsky
Rd and that Unit 1 was in lane#1. Driver 2 stated that Unit 1 was traveling below the speed limit
and causing a backup of traffic as they traveled Westbound. Driver 2 stated that once at the
intersection of SE Petrovitsky Rd and 108th Ave SE, he entered the#3 lefthand turn lane and
stopped for a red light facing West, and preparing to turn South onto 108th Ave SE.
Driver 1 stated that she was traveling West on SE Petrovitsky Rd and was unsure of what
happened following this.
Collision
Driver 2 stated that while stopped, Unit 1 approached from behind and the front bumper of Unit
1 collided with the rear bumper of Unit 2 while Unit 2 was stationary at a red light.
Driver 1 stated that she was not sure what happened and that she rear ended Unit 2.
Injuries
None reported.
Vehicle Disposition
Both vehicles were operational.
Proximate Cause
I determined that Driver 1 is the proximate cause of this collision because the driver of a motor
vehicle shall not follow another vehicle more closely than is reasonable and prudent, having due
regard for the speed of such vehicles and the traffic upon and the condition of the highway.
This incident occurred in the sCity of Renton,King County,Washington State.

I opted not to cite Driver 1 per RCW 46.61.145. However, due to her driving pattern reported by
Driver 2, who was an on duty fire fighter, along with her inability to recall the events leading up to
the collision, I opted to draft a DOL retest for and submit the form to DOL for their review to
Driver 1's driving privileges.
