Door(s) Rear Collision 2025-00009965 Paqe 3 OF 6 Case Number 2025-00009965 Officer Narrative Narrative On 11/16/2025, 1 was working uniformed patrol in a marked police car as 3R23.At approximately 1838 hours, officers were dispatched to a hit and run collision at NE Sunset Blvd/Sunset Blvd NE, which is located within the city limits of Renton, county of King,Washington. Dispatch advised that approximately 3 minutes ago,there was a motor vehicle collision in the area involving 3 cars. The caller reported that 1 of the involved vehicles left the scene westbound from the location.The caller also mentioned that there were no injuries. I eventually arrived at the location. I located 2 of the involved vehicles in the collision in the eastbound lanes of NE Sunset Blvd,just west of Sunset Blvd NE, in the southernmost shoulder. The involved parties in the accident are listed as follows: • Unit 1: Unknown black car. • Unit 2: Silver 2017 Honda CR-V(WA/CMK9516,VIN/5J6RW5H37HL006366). • Driver/RO-1:Walther De J.Yotagri Uribe (DOB: 11/11/1961,verified via DOL photo). • Front Passenger/RO-2: Maria C. Henriquez (DOB: 03/26/1959,verified via DOL photo). • Unit 3: Gray 2021 Tesla Model 3 (WA/BXV2454, VIN/5YJ3E1 EB6MF925811). • Driver/RO: Jacob T. Brennan (DOB: 05/13/1993,verified via DOL photo). • Front Passenger: Lilly Ann L. Kelly(DOB: 06/19/1988,verified via DOL photo). I was able to speak to the occupants of Unit 2. They explained the following: The occupants of Unit 2 described that they were in the#2 lane of eastbound NE Sunset Blvd, going up the hill, approaching Sunset Blvd NE.They described that Unit 1 was headed westbound down NE Sunset Blvd in lane#2. The occupants of Unit 2 described that Unit 1 began to veer into the#2 eastbound lane and eventually sideswiped the entire driver's side of Unit 2. Unit 1 was described to continue westbound down the hill without stopping to exchange information. Unit 2 said that they had to swerve and move over into lane#1 as a result of the collision and inadvertently hit Unit 3s rear driver's side bumper area as a result. I took a look around Unit 2. 1 saw that there were dents and scrapes along the entire driver's side This incident occurred in the sCity of Renton,King County,Washington State. !declare underpenalty of perjury under the laws of the State of Washington that the foregoing is true and correct. I also saw a scrape mark on Unit 2's front passenger side bumper area. Both occupants of Unit 2 stated they were wearing their seat belts and reported that no airbags had deployed in their vehicle. The driver of Unit 2 reported that he had some slight back pain.The front passenger also reported that her heart was beating very fast.The fire department was called to evaluate them at their request. I then spoke to the occupants of Unit 3.They explained the following: The occupants reported a similar story to that reported by Unit 2. Unit 3 was in lane#1 of eastbound NE Sunset Blvd and was struck on the driver's side rear bumper area by Unit 2 when they had swerved over. The driver of Unit 3 reviewed his Testa cameras.They were able to show me a brief photo still of a dark vehicle in lane#2 of westbound NE Sunset Blvd that is believed to be Unit 1.There was no identifying information on the photo other than the dark colored car. I took a look around Unit 3. 1 saw a scrape mark on the rear driver's side bumper area. Both occupants of Unit 3 stated that they both had their seatbelts.They also both reported that no airbags had deployed. Both of them also reported no injuries. Fire eventually arrived and evaluated the occupants of Unit 2. All parties eventually left the location under their own power and in their own vehicles. All parties were provided with a Renton PD business card with the associated case number for their records. There is probable cause for misdemeanor hit and run -attended for the driver of Unit 1.At this time, the driver is unknown. Should a driver be identified, there is probable cause for the above. Unit 1 appears to be the proximate cause of the collision.
