Tom provided the following: 1. While we do have a standing permit to do repairs on the 138th Av SE water main we would have to apply for f}[ an additional permit to do the line replacement. Tom said that if we submitteJ three copies of our plans q they could get the permit approved and by to us in ten l days or so. 2. The County would require us to do a full overlay of 138th Av SE. However, they would allow us to defer that overlay until next year. Our plans would have to note that the overlay would be done in 1990. 3. Tom said that compaction to 958 is required. If we can get 958 with native soil, this would be okay. If we cannot get the required 958 with native material we would have to import the backfill. .IC/kh cc: Ray Sled 3555 N.E. 2nd Street - Renton, Washington 98056 _'_ _'. m CUSTOMER NU.IBER INVOICE/STATEMENT H 731100047 BROL KING COUNTY OFFICE OF FINANCIAL MANAGEMENT AOGCOU TY ACR' '..;:s U/dD/ 6S.53Yo 653 KING COUNTY 4CM'N BLDG /)/ S /8,59.8`/. / 500 4TH AVE SEATTLE.WA 9BIDa PUBLIC WORKS-ROADS STATEMENT DATE 09/19/90 -- UTIt TY INSPECTIONS -- UTILTTY INSPECTIONS-ENGINEERIN , - ` INVOICE NUMBER 9034361 '\/\•`'_./y//,' DUE DATE 10/19/90 AMOUNT DUE #50.00 TO ENSURE PROPER CREDIT •MAKE CHECK PAYABLE TO KING COUNTY FINANCE DIVISION •RETURN THIS PORTION WITH YOUR PAYMENT •INDICATE CUSTOMER NUMBER ON CHECK TD CITY OF RENTON-ENGINEERING •ALLOW ADEQUATE TIME FOR DELIVERY - 20 MILL AVE S RENTON WA 98055 ATTN RICHARD MORENO MAIL THIS PORTIJN WITH YOUR PA IM ENT 0000000000000000000000000000000000000000000000000000000U00009034361700000500014 CUSTOMER NUMBER 31100047 GRO1 DETACH AND RETAIN THIS PORTION FOR YOUR RECORDS STATEMENTOATE pp9/19/9❑ INVOICE NUMBER PERIOD 08/01/90 - 08/31/90 PAGE 001 9034361 -- DESCRIPTION UNITS UNITS RATE AMOUNT DESCR M44470 UTILITY INSPECTION U47 UTILITY INSPECTIONS 298 RESTORATION INSPECTIO`: W 1414 69 138 AVE SE 8 SE 135 0.50 50.00 25.00 W 1414 89 138 AVE SE ¢ SE 135 0.50 50.00 25.00 TOTAL RESTORATION INSPECTION $50.00 TOTAL UTILITY INSPECTIONS $50.00 TOTAL UTILITY INSPECTION #50.00 TOTAL THIS INVOICE $50.00 s It t STATEME OF ACCO'iNT At It s PRIOR MONTH BALANCE $25.00 ADJUEESSTMENTS #$0.00 000R CURRENTSDELINAUENCY CHARGES 3.00 UNPAID INVOICES TOTAL UNPAID #n.00 TOTAL AMOUNT DUE #50.00 IAHfER 115,LAWS OF 196i r RENT Or CEFr .-A7ioh :d m M•Fy 'y udR InWry Y;a.n, ...:a1 odiE .BAR dI..•C;•Y o Ra '.•• am yNow%-iA+a 0-ir-5d CALL 296-75.iS FOR ASSISTANCE. t:ING COUNTY DEPARTMENT OF PUBLIC WORKS ROADS DIV' ION T/C 81 UTILITY INSPECTION SECTION INSPECTION REPORT UTILITY JOB NO 1 UTlury PERMIT NO 6 ¢' q UTILITY ADDRESS / ^ - —.- LOW ORG NO.5608 UTILITY NO(OPTION) l A 4-7 _. Earl Clymer, Mayor Maintenar.— Division 9;, MEMORANDUM DATE: 17 July 1989 TO: Lynn Guttmann FROM: Jack Crum,,,/-- SUBJECT- Water Main in 138th Avenue SE The city has an old four inch steel water main which lies in 138th Avenue SE (see attached map). Over the last three years we've done 25 repairs on only 2,300 feet of water main. We now make repairs right next to older repairs. Something that is discouraging to our workers. Ray and I have suggested that this main be replaced as part of the steel :rain replacement program for the past two years. However, Ron Olson has thought there would be some development going in the area and that he could get the developer to do the main replacement as part of the cost of development. Unfortunately, we are still waiting for the development and the leaks keep on keeping on. Today we were out there repairing one leak and saw two more leaks come into being as a couple of loaded dump trucks went rolling thr ugh. (To help understand the problem, we repaired another leak on the main only last Friday. ) ( Ray Sled was speaking with Dave Tibbott abc _ the problem this morning and Dave indicated that they; .(ay be some money left in one account or another, Ray thought if it was the Steel Main Replacement program, we could use it to do -.,_ tomething.9ght now. I would like to suggest, that if there is some money in the water account I would like to get authorization to purchase materials with it. The plan- would be to replace as much main as the available money allows, doing the work ourselves. Additionally, if the 138th Avenue SE main is not on the 1990 steel main replacement list, it should go on. Please let me know if there is some left over money. Once I know the dollar amount and have your OK I will take care of the rest. cc: Ray Sled 3555 N.E. 2nd Street - Renton, Washington 98056 - 1 I r r c r g S Hl G i I O 00 41 O yy ° U ' J \ I 1 a ® ----------- r 11 ,,GY H b% 1 9w Qom. 1 h W z Pan Nine 197-11-800 SFPA Roles Provided, That additional appropriations of water are exemptions arc specifically made inapplicable by one of not exempted by this subsection, the following exemptions. An environmental impact statement (EIS) must be prepared for all proposals with probable significant adverse impacts on the quslity of the environment. The purpose of this checklist is to provide information to help you and the agency identify impacts from your proposal (and to reduce or avoid impacts from the proposal, if it can be done) and to help the agency decide whether an EIS is required. Instructions for Applicants: This environmental checklist asks you to describe some basic information about your proposal. Governmemal agen- cies use this checklist to determine whether the environmental impacts of your proposal are significant, requiring preparation of an EIS. Answer the questions briefly, with the most precise information known, or give the best de- scription you can. You must answer each question accurately and carefully, to the best of your knowledge. In most cases, you should be able to answer the questions from your own observations or project plans without the need to hire experts. If you really do not know the answer,or if a question does not apply to your proposal, write 'do not know- or -does not ap- ply'.Complete answers to the questions now may avoid unnecessary delays later. Some questions ask about governmental regulations, such as zoning, shoreline, and landmark designations. Answer these questions if you can. If you have problems, the governmental agencies can assist you. The checklist questions apply to all parts of your proposal, even if you plan to do them over a period of time or on different parcels of land. Attach any additional information that will help describe your proposal or its environmental effects. The agency to which you submit this checklist may ask you to explain )our answers or provide additional in- formation reasonably related to determining if there ma:- be significant adverse impact. Use of checklist for nonproject proposals: Complete this checklist for nonproject proposals, even though questions may be answered 'does not apply.' IN AD- DITION.Complete the SCPPLEMENTAL SHEET FOR NONPROIECT ACTIONS(part D). For nonproject actions, the references in the checklist to the words 'project,' 'applicant,- and "property or site' should be read as "proposal,' -proposer,' and "affected geographic area,' respectively. An environmental impact statement (EIS) must be prepared for all proposals with probable significant adverse impacts on the quality of the environment. The purpose of this checklist is to provide information to help you and the agency identify impacts from •our proposal (and to reduce or avoid impacts from the proposal, if it can be done) and to help the agency decide whether an EIS is required. Instructions for Applicants: This environmental checklist asks you to describe some basic information about your proposal. Governmental agen• ties use this checklist to determine whether the environmental impacts of your proposal are significant, requiring preparation of an EIS. Answer the questions briefly, with the most precise information known, or give the best de- scription you can. You must answer each question accurately and carefully, to thr best of your knowledge. In most cases, you should be able to answer the questions from your own observations or project clans without the need to hire experts. If you really do not know the answer, or if a question does not app!p to your proposal, write 'do not know' or 'does not ap- ply' Complete answ-rs to the questions now may avoid unnecessary delays later. Some questions asK about governmental regulations, such as zoning, shoreline, and landmark designations. Answer these questions if you can. If you have problems, the governmental agencies can assist you. The checklist questions apply to all parts of your proposal, even if,you plan to du them over a period of time or cis different parcels of land. Attach any additional information that will help describe your proposal or its environmental effects. The agency to which you submit this checklist may ask you to explain vour answers or provide add.-ional in. formation reasonably related to determining if there may be significant adverse impact. Use of checklist for nonproject proposals.: Complete this checklist for nonproject proposals, even though questions may be answered 'does not apply.' IN AD- DITIO%,Complete the SLPPLEMVNTAL SHEET FOR NONPROIECT ACTIONS (part D). For nonproject actions, the references in the checklist to the words 'project.'
