in order for Allpak to receive a permanent Certificate of
Occupancy. We have completed all items in accordance with Randy
Martin's letter dated May 14, 1991 and hope to have the Certificate
of Occupancy issued today.
The enclosed items, Bill of Sale and Cost Data Sheet, have been
signed and notarized (where applicable) and a check for 10% of the
overall cost is included for the City to deposit in trust as a
maintenance bond. Randy's letter only called for a maintenance
bond on the water, but someone at the City has arbitrarily added
the Street Improvement cost which increased the value by $17,000.00
We would however, like to :eceive the Certificate of Occupancy arcs
if you end up agreeing that this addition is unfair, we would ask �^
that you return the $1,7UU.00 relative to Street Improvements in a �)..
timely manner. As I am having this letter hand delivered, I would
very much appreciate a phone call from you upon receipt indicating 1p

1. Puget Power distributed copies of preliminary sketch relating to
the job. The southwest corner of the property will have a couple
of fairly large vaults. Developer/owner is asked to provide all
the excavation, restoration i.1d patch. Puget Power will put in
vaults, conduit and all the electrical work.
2. U.S. West; Dan Mulvihill, SDL, talked with Bob Steen and has a
draw+nq of how they want vaults put in.
3. Washington National Gas: Dan Mulvihill talked with them and said
they have a stub across the property line. He has drawings
swing where they will go off the stub.
4. Cable TV. None available and none required.
IN- HOUSE UTILITIES:
TRAFFIC 9ONTR9&
1. Hours are 8:30 a.m. to 3:30 p.m. for working in the right-of-way.
2. A traffic control plan is required for all lane closures. Phil
Chase, Traffic Division, would like 24 hour notice of closures.
STORM WATER
1, Will use spiral rib galvanized steel pipe coming out of oil water
separator vault.
2. Good erosion control to be exercised at all times. During time
period November l to March 31, all project disturbed soil areas
greater than 5,000 squaw feet that are to be last uncovered for
any more than 12 hours shall be covered by one of three methods;
mulch, sodding or plastic covering per Ring County Manual, Sec.
5.5, Chapter 5.
3. Pumps and catch basins 019 and 010: there is concern that an
intense storm could cause ponding due to the capacity of the
pumps. An element of the design of the pumps, because of the
sizing, is to allow some degree of surcharge in the truck docks.
A hold harmless agreement will be drawn up later for signaturee,
to hold the City harmless in case there is inconvenience because
of puddling in the truck docks.
WAJ=
1. Water utilities would like to add stub and easement to west
property line. Since there will be a road going in (extension of
oakesdale), they would like to have a stub provided to the road at
this time. This stub requires a :0"xl0- tee, gate valve and MJ
plug with blocking.

2. Contractor will install all of watermain except the connection.
All blocking must be inspected before backfilling. Pipe will be
class 52 ductile iron. Hydrants will be Iow^ type. Thrust block
must be inspected by city inspector. In strapt and in all
crossings, the City requires 5/8" minus rock backfill. Compaction
test may be required.
On services (2"), contractor to install all but meter. 0ae tapped
tee installed right in main (12"x2" tapped tee)
:. Testing/flushing/chlorinating procedure as follows:
a. Polypig flushing.
b. Pressure test 150 psi plus static head (minimum 200 psi).
C. Chlorinate for 24 hours, using chlorina solution and
chlorine pump assembly.
d. Water purity test

4. Contractor, as well as engineer shall keep as-built drawings. For
any field changes, if minor, give City a shop drawing (by Fax); if
major, need a day or two to review and approve. Field inspector
can not give approval in field for construction changes.
FIRE
1. Maintain access to the site at all times. Must be able to support
emergency equipment in all type weather. Contractor stated that
paving is scheduled for the 8th or 9th of
2. Keep now fire hydrants covered with burlap t .gs until operational.
3. Hydrostatic test 200 psi for 2 hours. Ful) volume flush at end of
hydro test. Three 2 1/2" fire hoses for flushing. Need to flush

4. Need separate plans and nermit from Fire Department if any fuel
stored on site.

6. Fire hydrant barrels - large opening must face the traveled
roadway. If on driveway, opening face the curb, if within 10' of
main roadway, face roadway. No hydrants in drive section.
Hydrants to be painted International yellow.
7. Domestic water line downstream of meter is owner's responsibility.
B. whenever water is used from hydrant, need double check valve and a
meter from the City.
9. Need ball drip on detail.
¢ANITARY SEWER
1. Wastewater: when installation exposes pipe, continue that pipe
material and slope up to first manhole. Do not change pipe type
in the run.
2. Cleanouts: if in paved area, have to be a cast iron type; if
landscaped area, can use plastic. Plugs should be screw type in

the cleanouts. PVC pipe to be beddsd in pea gravel 6" above end
6" below. contractor will be responsible for TV if required. Try
to keep trench as dry as possible.
3. Engineer needs to draft the sanitary sewer easement for the future
extension on the sanitary sawar plan.
TRANSPORTATION
1. uniform manual should be followed for street closures, routing for
traffic. Restrictive covenants will nerd property owners
signature. Any street work to be dons to Ci:r standards.
2• Construction permit will be sufficient for work done within the
right-of-way as long ac contractor is properly bonded.
3. Let inspector (Mary Burgy) know whor you plan to start work.
Contractor will provide work schedule to City for distribution.
GENERAL
1. Submittals needed for change ord3rs only.
2. Start date - November 26th.
3 overtime takes special authorisation. Pay schedule is $50 per
hour for overtime.
4. Project hours for construction on site are 7:00 a.m. to 8:00 P.M.
weekdays, Saturday by approval only and no Sundays.
E. Storm drainage; would like filter fabric underneath the gr!te when
